About this file

This is a Quality Assurance Surveillance Plan (QASP) for Contract #36C24626P0617, a Water Monitoring Service contract awarded to support Salem Veterans Affairs Medical Center (VAMC) in Salem, Virginia. The QASP establishes the systematic framework by which the Government will evaluate contractor performance throughout the contract period, defining roles, responsibilities, performance standards, and monitoring procedures.

The Government's oversight is managed by the Contracting Officer (Harold L. McQuillia, Jr., 757-315-3430, harold.mcquillia@va.gov) and a Point of Contact (POC) to be designated, who will conduct surveillance activities and maintain a quality assurance file. Three primary performance objectives with established Acceptable Quality Levels (AQLs) are outlined: (1) adherence to all Performance Work Statement (PWS) Section B.2 requirements with a 95% AQL using CO/POC periodic surveillance; (2) compliance with Inspector Qualifications and Performance Standards per PWS Section B.2.5 with a 95% AQL using observation and audit methods; and (3) full compliance with Reporting Requirements in PWS Section B.2.4 with a 100% AQL using observation and audit methods. Surveillance methods include direct observation, complaint review, and user surveys. Performance ratings are categorized as positive (no more than 1 incident of non-compliance), neutral (no more than 3 incidents), or negative (more than 3 incidents). Documented unacceptable performance triggers Contract Discrepancy Reports (CDRs) requiring corrective action plans, with the POC conducting monthly performance assessment meetings with the contractor and providing written assessments to the Contracting Officer.

The contractor shall acknowledge receipt of the CDR in writing. The CDR shall specify if the contractor is required to prepare a corrective action plan to document how the contractor shall correct the unacceptable performance and avoid a recurrence. The CDR shall also state how long after receipt the contractor has to present this corrective action plan to the CO and POC. The Government shall review the contractor's corrective action plan to determine acceptability.

Any CDRs may become a part of the supporting documentation for any contractual action deemed necessary by the CO.

a. Frequency of Measurement.

During contract performance, the POC will periodically analyze whether the frequency of surveillance is appropriate for the work being performed.

b. Frequency of Performance Assessment Meetings.

The POC shall meet with the contractor monthly to assess performance and shall provide a written assessment.
