EVALUATION
Below are specific questions relating to the Contractor’s performance in the areas of quality, timeliness, cost control, business relations and customer satisfaction. Please rate the Contractor for each question on a scale as either Acceptable, Unacceptable or Neutral. Indicate your rating by placing an “X” in the box of your selected rating for each question. Please type comments in the section provided below each question to support the rating given. Please email the completed questionnaire to the IRS Procurement Office, Bridgette.Garcia@irs.gov.
ACCEPTABLE: The offeror’s past performance for the contract demonstrated performance that met or exceeded contractual requirements over the past five years. The contractual performance of the element may have contained some minor problems for which proposed corrective actions taken by the contractor appear satisfactory, or completed corrective actions were satisfactory. There was an acceptable amount of risk to successful contract performance.
UNACCEPTABLE: The offeror’s past performance for the contract demonstrated performance that did not meet contractual requirements over the past five years; recovery for those projects is not likely in a timely or cost effective manner. The contractual performance of the element contained serious problem(s) for which the contractor's corrective actions appear or were ineffective. There was an unacceptably high risk that the contractor will not successfully perform.
NEUTRAL: The absence of relevant present and past performance information over the past five years or where the offeror has not provided a sufficient number of references that can be reached who can verify contract performance will result in the assignment of a neutral rating.
The offeror has little or no past performance for similar work upon which to base a meaningful performance risk prediction; therefore the offeror will not be evaluated favorably or unfavorably on past performance.
