About this file
This document is an Addendum to the Request for Proposals (RFP) issued by the Minnesota Department of Veterans Affairs (MDVA) for Occupational Therapy and backup Physical Therapy services at the Minnesota Veterans Home - Fergus Falls (MVH-FF), a 106-bed skilled nursing facility located in Fergus Falls, Minnesota. The Addendum responds to questions from potential responders regarding the qualifications and experience requirements for the therapists who will provide the services. Specifically, the MDVA clarifies that while responders may not have assigned therapists at the time of proposal submission, they must provide resumes and details on the qualifications of the therapists who will ultimately be assigned to the contract, and any changes in personnel will require the MDVA's prior written approval.
The RFP also requires responders to provide a detailed written work plan describing the medical services to be provided, scheduling, management, and billing processes. The contract will be awarded to a qualified responder to provide the requested Occupational Therapy and backup Physical Therapy services to the MVH-FF facility located in Otter Tail County, Minnesota.

Question and Answer Addendum

Questions from potential responders and answers from the MDVA
1. Question: I have a question on Section 2. Qualifications and Experience – Regarding the below requirement. If awarded the contract, we will hire for the Therapist positions.
Therefore, we do not have an assigned therapist at this point. Upon contract notification, we can provide the new hires information required – so the actual therapist performing the work is TBD based on our pool of qualified candidates. We have Regional Managers and VPs that are credentialed therapists that will oversee their hire, and I can provide their resumes, licenses, work experience, etc. but the actual therapist will be a new hire. Will that suffice for this requirement?
“Resumes of staff who will be assigned to the Contract and a matrix table listing staff, classification, and years of services with the company including a summary of educational experience, work experience, and relevant licenses and certifications held for each individual who will be assigned to the Contract. Responder should provide the location of those individuals (i.e., local, home or regional office). No change in personnel assigned to the project will be permitted without the prior written approval of the State's Authorized Representative.”
Answer: Permanent staff who possess the requisite licensure and are able to perform the services requested under this contract can be augmented by future hires, provided, however, that potential new hires will not be allowed to perform work under this contract until their resume, including a summary of educational experience, work experience, and relevant licenses and certifications held, is provided to the State's Authorized Representative who must approve the new personnel assigned to the contract in writing. Please indicate in the response the method for which you plan to have in place any future therapist positions to cover the services for the hours listed in
2.1.1 and 2.2.1.
2. Question: Besides the description of our services, scheduling, management and billing, are you looking for a specific written work plan?
“Qualifications and Experience. Responder should provide the following information:
A description of the medical services to be provided by the responder along with a detailed work plan that can be used as a scheduling and managing tool, as well as the basis for invoicing.”
Answer: Yes. We are also requesting a specific written work plan.
