PART 1

GENERAL INFORMATION:

1.1. GENERAL: This is a non-personnel services contract to provide commercial internet services and cable television services (CATV) to Wright Patterson Air Force Base (WPAFB), Ohio. The Government shall not exercise any supervision or control over the contract service providers performing the services herein. Such contract service providers shall be accountable solely to the contractor who, in turn is responsible to the Government. Contractor and the Government recognize that, over the term of the Performance Work Statement (PWS), commercial internet and CATV technology and the regulation of these services will undergo continual change. The technology, equipment, and services required by this PWS may become obsolete, uneconomical, or otherwise, disadvantageous. Contractor and the Government, therefore, agree to consider in good faith proposals submitted by either party for modification of the PWS to permit the introduction of new equipment and technology or to otherwise improve commercial internet and CATV services to the base

1.2. SCOPE OF WORK: Contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services (except those identified as government furnished) necessary to perform commercial internet and CATV services as defined in this PWS. The work to be performed under this contract will be performed at WPAFB in all buildings, facilities, and structures occupied by WPAFB personnel set forth in this PWS to include any yet to be established locations occupied by WPAFB personnel.

1.3. PERIOD OF PERFORMANCE: The period of performance shall be for one (1) Base Year of twelve (12) months and four (4) 12-month option years.

1.4. TYPE OF CONTRACT: The Government will award a Firm Fixed Price (FFP) type contract.

1.5. HOURS OF OPERATION: Contractor is responsible for conducting business between the hours of 7:00 AM and 7:00 PM Monday thru Friday except Federal holidays. Contractor shall respond to service outages when reported, regardless of whether on a weekend or holiday. Contractor must at all times maintain an adequate workforce for the uninterrupted performance of all tasks defined within this PWS when the government facility is not closed for the above reasons. When hiring personnel, the contractor shall keep in mind that the stability and continuity of the workforce are essential.

1.6. RECOGNIZED HOLIDAYS:

1.7. CONTINUATION OF CONTRACTOR ESSENTIAL SERVICES: In order to ensure continuation of essential contractor services under all circumstances, Base Operations support function as depicted within Attachment XX is hereby designated as Mission-Essential. There may be times when real-world conditions dictate some, or all, contractor services elevated to designation as Mission-Essential;

examples (snow removal, hurricane evacuation support, elevated FPCONs, etc.). Required support for Wing Plan(s) execution are designated as essential contractor service as defined in Defense Acquisition Regulation Systems (DFARS) 252.237- 7023, Continuation of Essential Contractor Services. Contractor shall be responsible for performing those services identified as essential contractor services during crisis situations as directed by the Contracting Officer per DFARS regulation.

1.8. PHYSICAL SECURITY AND NETWORK SECURITY: Contractor shall be responsible for safeguarding all government property provided for contractor use. At the close of each work period, government facilities, equipment, and materials shall be secured. Contractor shall respond to all requests from the COR or Contracting Office to verify network security settings that are required for commercial internet services and CATV services.

1.9. CONTRACTING OFFICER REPRESENTATIVE (COR): The (COR) will be identified by a separate letter. The COR monitors all technical aspects of the contract and assists in contract administration. The COR is authorized to perform the following functions: assure that the contractor performs the technical requirements of the contract: perform inspections necessary in connection with contract performance: maintain written and oral communications with the contractor concerning technical aspects of the contract:- including government drawings, designs, specifications: monitor contractor's performance and notifies both the Contracting Officer and Contractor of any deficiencies; coordinate availability of government furnished property, and provide site entry of contractor personnel. A letter of designation issued to the COR, a copy of which is sent to the contractor, states the responsibilities and limitations of the COR, especially with regard to changes in cost or price, estimates or changes in delivery dates. The COR is not authorized to change any of the terms and conditions of the resulting order.

1.10. CONTRACTOR PERSONNEL: Contractor shall provide a primary and alternate representative to manage and oversee performance of work. Names of primary and alternate (who shall act for primary representative in his/her absence) shall be designated in writing to the Contracting Office. Contractor shall provide telephone numbers for primary and alternate representatives where these persons may be contacted before, during, or after normal work hours. Representatives shall be able to read, write, speak, and understand English fluently. Representatives shall have full authority to act for contractor on the installation and possess full contractual signatory authority. Contractor shall meet with Government contracting personnel when requested. Contractor may request a meeting with the Contracting Officer as necessary.

1.11. CONTRACTOR EMPLOYEES: Contractor personnel shall present a clean, neat, and professional appearance and be easily recognized as contractor personnel. This may be accomplished by wearing distinctive clothing showing the name of the company or by wearing appropriate badges that contain the company name and employee name. No item of military clothing shall be worn by a contractor as an outer garment.

1.11.1. The Government reserves the right to restrict the employment, under this contract, of any contractor employee, or prospective contractor employee who is identified as a potential threat to health, safety, security, general wellbeing, or the operational mission of the installation and its population.

Contractor shall perform criminal background checks, at contractor’s expense, to ensure that said employee has not been convicted of any felony offense at the time of hire date.

1.12. QUALITY ASSURANCE: The Government shall evaluate the contractor’s performance under this contract in accordance with the Quality Assurance Surveillance Plan. This plan is primarily focused on what the government must do to ensure that the contractor has performed in accordance with the performance standards. It defines how the performance standards will be applied, the frequency of surveillance, and the minimum acceptable defect rate(s).

1.13. VOLUNTEER PROTECTION PROGRAM: Contractor shall participate in the Voluntary Protection Program (VPP). Wright-Patterson AFB is in the process of pursuing VPP recognition or has already been recognized under the OSHA Voluntary Protection Program (VPP). VPP impacts all “applicable contractors” operating on Air Force Installations. Contractor shall be responsible for ensuring their employees and managers have a comprehensive understanding of VPP as well as full compliance with OSHA requirements. Contractor shall follow the safety and health rules of the installation or VPP site.

1.13.1. Contractor shall submit a Safety and Health Plan and corresponding site safety checklist to the CO not later than (10) calendar days after contract award. Contractor’s plan shall include appropriate measures to ensure the contractor reacts promptly to investigate, correct, and track alleged safety and health violations and/or uncontrolled hazards in contractor work areas. The plan shall:

1.13.1.1. Identify the roles and responsibilities of the following individuals with respect to safety and VPP:

Management, Supervisors, Employees, and Safety Coordinator;

1.13.1.2. Identify applicable safety rules and regulations;

1.13.1.3. Include a worksite hazard analysis to include base-line hazard identification and required control measures;

1.13.1.4. Include a job site analysis to include hazards of tasks required to control measures;

1.13.1.5. Identify employee safety and health training requirements and the documentation process;

1.13.1.6. Include a workplace inspection frequency, to include identifying the individual conducting the inspections;

1.13.1.7. Include employee hazard reporting procedures;

1.13.1.8. Identify individual(s) responsible for corrective action of hazards;

1.13.1.9. Identify first aid/injury procedures;

1.13.1.10. Identify procedures for accident investigation and reporting;

1.13.1.11. Identify emergency response procedures; and

1.13.1.12. Identify the process for tracking controlled hazards in contractor work areas.

1.13.2. Contractor shall submit their Total Case Incidence Rate (TCIR) and Days Away, Restricted, and or Transfer Case Incident Rate (DART) rates and OSHA Form 300A annually to the CO for consolidation and submission as part of the installation’s annual VPP Safety and Health Management report. TCIR and DART rates are due by January 15th of each year (A003).

1.13.3. Contractor’s Quality Control Plan shall identify the processes and procedures the contractor will use to track compliance with the Safety and Health Plan, and the process and procedures that will be used to correct violations.

1.13.4. Contractor shall establish these VPP requirements for all subcontractors who qualify as applicable contractors under the resulting contract.

1.14. BUSINESS OFFICE: Contractor shall staff a business office (BO) to handle all complaints and requests for repairs, during regular business hours (8:30 a.m. to 5:00 p.m. Monday through Friday except government holidays). After Normal Duty Hours, the contractor shall be available to receive complaints and requests for repairs. A 24-Hour contact number shall be provided to the Contracting Officer Representative (COR) and Contracting Office.

1.15. ADMINISTRATIVE RECORDS: All records, documents, and associated papers provided by the government and generated during the period of this contract become government property and will remain in place upon contract termination or completion. During the period of the contract, records, documents, and associated papers shall be available for review by the government.

1.16. FRANCHISE AGREEMENT: Contractor shall be required to obtain a franchise agreement in accordance with U.S.C. Section 541, unless an exception applies, or the cable services provided are exclusively for direct mission support and to the exclusive benefit of the Air Force.

PART 2

COMMERCIAL INTERNET SERVICES:

2.1. DESCRIPTION OF SERVICES: Contractor shall provide commercial internet installation and service to all of WPAFB users. Contractor shall provide broadband service at specified speeds with 99% uptime per month, within two categories: modem broadband and dedicated fiber link broadband, as outlined below:

• Modem Economy – minimum 7 mbps down 768 kbps up

• Modem Standard – minimum 12 mbps down 1 mbps up

• Modem Premium – minimum 12 mbps down 1.5 mbps up

• Dedicated Fiber Link Broadband - Economy – minimum 15 mbps down 3 mbps up

• Dedicated Fiber Link Broadband - Standard – minimum 25 mbps down 5 mbps up

• Dedicated Fiber Link Broadband - Premium – minimum 40 mbps down 6 mbps up

• Dedicated Fiber Link Broadband - Ultra Economy - minimum 500 mbps down 500 mbps up

• Dedicated Fiber Link Broadband - Ultra Standard - minimum 1 gigabyte down 1 gigabyte up

• Dedicated Fiber Link Broadband - Ultra Premium - minimum 4 gigabyte down 4 gigabyte up.

2.2. BROAD BAND MODEM SERVICE TIERS: Contractor shall provide broad band modem service tiers to WPAFB users in the categories listed below:

2.2.1. BROAD BAND MODEM – ECONOMY: Installation shall include service of minimum 7 mbps down 768 kbps up, 1 Status IP Address, 15 E-Mailboxes, Webmail, Access for all Mailboxes, 50 Mb Web Space, DNS Hosting, Access to Member Services Area.

2.2.2.. BROAD BAND MODEM - STANDARD: Installation shall include service of minimum 12 mbps down 1 mbps, 1 Status IP Address, 15 E-Mailboxes, Webmail, Access for all Mailboxes, 50 Mb Web Space, DNS Hosting, Access to Member Services Area.

2.2.3.. BROAD BAND MODEM – PREMIUM: Installation shall include service of minimum 12 mbps down 1.5 mbps up, 1 Status IP Address, 15 E-Mailboxes, Webmail, Access for all Mailboxes, 50 Mb Web Space, DNS Hosting, Access to Member Services Area.

2.3. DEDICATED FIBER LINK SERVICE TIERS: Contractor shall provide dedicated fiber link service tiers to WPAFB users in the categories listed below:

2.3.1. DEDICATED FIBER LINK BROADBAND – ECONOMY: Installation shall include service of minimum 15 mbps down 3 mbps up, 1 Status IP Address, 15 E- Mailboxes, Webmail, Access for all Mailboxes, 50 Mb Web Space, DNS Hosting, Access to Member Services Area.

2.3.2. DEDICATED FIBER LINK BROADBAND – STANDARD: Installation shall include service of minimum 25 mbps down 5 mbps up, 1 Status IP Address, 15 E- Mailboxes, Webmail, Access for all Mailboxes, 50 Mb Web Space, DNS Hosting, Access to Member Services Area.

2.3.3. DEDICATED FIBER LINK BROADBAND – PREMIUM: Installation shall include service of 40 mbps down 6 mbps up, 1 Status IP Address, 15 E- Mailboxes, Webmail, Access for all Mailboxes, 50 Mb Web Space, DNS Hosting, Access to Member Services Area.

2.3.4. DEDICATED FIBER LINK BROADBAND – ULTRA ECONOMY: Installation shall include service of minimum 500 mbps down 500 mbps up, 1 Static IP address, 15 E- Mailboxes, Webmail, Access for all mailboxes, 50 Mb Web Space, DNS Hosting, Access to Member Services Area.

2.3.5. DEDICATED FIBER LINK BROADBAND – ULTRA STANDARD: Installation shall include service of minimum 1 gigabyte down 1 gigabyte up, 1 Static IP address, 15 E- Mailboxes, Webmail, Access for all mailboxes, 50 Mb Web Space, DNS Hosting, Access to Member Services Area.

2.3.6. DEDICATED FIBER LINK BROADBAND – ULTRA PREMIUM: Installation shall include service of minimum 4 gigabyte down 4 gigabyte up, 1 Static IP address, 15 E- Mailboxes, Webmail, Access for all mailboxes, 50 Mb Web Space, DNS Hosting, Access to Member Services Area.

2.4. SERVICE REQUESTS: Customers shall submit all requests for new installation to the Contracting Officer Representative (COR) and 88th Communications Squadron. The request must be approved for installation by the Authority to Operate (ATO) at Air Force Life Cycle Management Center (AFLCMC). Once ATO approval is granted, the COR shall notify the contractor of installation request.

Contractor shall have seven (7) working days to provide cost estimate and propose any technical solutions to the Customer. Installation of the commercial internet services shall be completed within 60 days of the Customer approved cost estimate.

2.5. REPAIR REQUESTS: All user requests for repairs shall be submitted to the COR who in turn shall notify the contractor of repairs needed. Contractor shall respond within (1) hour during normal duty hours and within two (2) hours during non-duty hours (if request is deferred by the user, the contractor shall respond by 8:30 a.m. on the next duty day). Contractor shall notify the COR within twenty-four (24) hours if a request for service or repair is not completed or not otherwise resolved to the user’s satisfaction.

2.6. INTERRUPTION OF SERVICE: Contractor shall at all times maintain the system in good condition and repair to provide uninterrupted service in conformance with all standards and requirements.

Contractor shall limit to the shortest time possible any interruption of service for the purpose of maintaining, repairing, or upgrading the system. Preplanned interruptions shall be preceded by written notice to users and shall occur during periods of lower usage times (after normal duty hours if allowable).

The COR shall be notified five (5) working days in advance of expected interruption of service. Said period shall begin when the contractor receives notice from a user that service has been interrupted or when the contractor has actual or constructive notice of said interruption, whichever is earlier, and shall cease when the service has been restored to the user.

2.7. COMPLAINTS: Contractor shall respond to user complaints within five (5) working days. The contractor shall maintain written records of all complaints received during the performance of the contract.

Written records shall include the date of complaint, identity of the user, the nature of the complaint, and the contractor’s response.

2.8. PAYMENTS: Payment for installation and/or service of commercial internet will be done with Government Purchase Card (GPC) between customer and contractor for all purchases under the allowable GPC amount. Contractor shall provide a monthly funds expenditure report (CDRL A002) to the COR and CO that shall include a list of account numbers, organization ID’s, an account POC (with email address), subscribed service, and payment amount.

2.8.1. All installation or commercial internet service amounts over the GPC allowable amount shall be estimated by the contractor and completed by a separate contract action. These contract actions shall also be included in monthly funds expenditure report (CDRL A002) submitted to the COR and CO.

2.9. COMMERCIAL INTERNET SERVICE INSTALLATION: Contractor shall provide installation of commercial internet in broad band modem service and/or dedicated fiber link service at various locations at Wright Patterson AFB, OH. Contractor shall include all labor, tools, parts, transportation, materials, and documentation necessary to provide installation of these services.

2.9.1. INTERIOR INSTALLATION: Cable shall be concealed behind ceiling and walls and not be run along interior walls or baseboards, unless approved by the COR. The Internet outlets shall be installed in accordance with commercial practices and be set flush with finished walls.

2.9.2. EXTERIOR INSTALLATION:

2.9.2.1. BUILDING SERVICE: The diameter of any holes passing through walls shall not exceed the diameter of the cable or conduit by more than one-eighth inch. All exterior holes shall be sealed to prevent entry of moisture or insects into the facility.

2.9.2.2. EXTERIOR MOUNTING: Cables shall be installed parallel or at right angles to walls or structural members. Mounting shall follow building lines, where possible (i.e., brick, mortar, siding), in straight horizontal and vertical lines, with bends tied to building corners.

2.9.2.3. SERVICE DROPS: Contractor shall mount all aerial service drops parallel to utilities and or phone lines. If dual aerial service drops are used, the contractor shall use dual type cable to present a clean appearance. Contractor is prohibited from using government-furnished conduit supplied for CATV service drops, aerial service drops, and alternate underground routes.

2.9.2.4. CLEARANCES: Internet cabling shall not conflict with or cause any other cabling to violate ground or power line clearance criteria unless a waiver has been granted by the base civil engineer or a designated representative.

2.10. UNDERGROUND CONNECTIONS:

2.10.1. Under paved areas and roadways, the internet cables shall be installed in conduit not less than two

(2) inches in diameter. Conduit shall be zinc-coated rigid steel or scheduled 40 PVC. Conduit shall be extended not less than two (2) feet beyond pavements and roadways, when such roadway is used for vehicular traffic. Under heavily traveled roadways, conduit shall be installed by boring. Pavement may be cut and repaired in an approved manner on lesser traveled roads on approval of the base civil engineer.

2.10.2. Trenches in which direct burial cables are places shall have a minimum depth of eighteen (18) inches below grade, shall not be less than six (6) inches wide, and shall generally be in straight lines between cable connections. Bends in trenches shall have a radius of not less than thirty-six (36) inches. A marker tape six (6) inches below grade shall be used above all direct buried cable.

2.11. EXCAVATION AND BACKFILLING:

2.11.1. EXCAVATION PERMIT: Contractor shall obtain an excavation permit one (1) day prior to start of work from the base civil engineer or a designated base communications group representative before commencing any digging or excavation on the Base.

2.11.2. EXCAVATION METHODS: Hand digging is required within four (4) feet of existing utilities identified by excavation permit and in areas immediately adjacent to and within approximately one (1) foot of buildings.

2.11.3. BACKFILLING: Shall be accomplished with well-pulverized soil and each twelve (12) inch layer of soil shall be compacted to a CE 55 density if at least ninety-five (95) percent.

2.11.4. OPEN EXCAVATION: Shall be barricaded when contractor personnel are not present in the immediate vicinity of the work site.

2.12. PAVEMENT CUTS: Pavement cuts shall be made only after the approval of the location and circumstances by the base civil engineer. Traffic barricades and warning lights to mark the excavation shall be provided by the contractor. The restored pavement shall be equal to or better than the original pavement.

2.13. FINAL CLEANUP: After the work is completed, the work site shall be returned to its original state.

2.14. REFUSE COLLECTION: All waste generated by the contractor in performance of the services specified within this PWS shall be deposited in government dumpsters, excluding hazardous materials in which proper disposal instructions shall be complied with. Contractor shall comply with base recycling regulations.

2.15. SERVICE SUMMARY: Contractor service requirements are summarized into performance objectives that relate directly to mission essential items. The performance threshold briefly describes the minimum acceptable levels of service required for each requirement. These thresholds are critical to mission success. The performance objectives will be measured based on customer complaints, government surveillance, and Corrective Actions Reports (CAR).

CABLE TELEVISON (CATV) SERVICES

3.1. DESCRIPTION OF SERVICES: Contractor shall provide CATV installation and service to all of WPAFB users. Contractor shall provide tiered services that includes public access channels, education access channels, and government access channels. The service shall also include the retransmission of locally broadcast television signals. Contractor shall maintain, measure, and test the CATV service system in accordance with industry standards and as required by Federal Communications Commission (FCC) regulations set forth in 47 CFR Part 76. Contractor shall deliver CATV service and programming with unreported outages of no more than five (5) minutes per month.

3.1.1. CHANNEL CAPACITY: Contractor shall provide CATV at a minimum of sixty (60) channels capable of delivering high frequency (VHF) and frequency modulation (FM) spectrums and selected portions of the ultra-high frequency (UHF) spectrum.

3.1.2. EMERGENCY OVERRIDE: Contractor shall provide an emergency override system that permits the Government to override the CATV system and transmit messages on all channels at any time, with a live-recorded voice or other audio signal or text scroll message.

3.2. CATV SERVICE TIERS: Contractor shall provide the following service tiers to WPAFB users.

3.2.1. STANDARD TIER: Contractor shall provide a standard tier of CATV service, which shall include all services required by the rules of the FCC and any other federal law or regulation, public access channels, educational access channels, public affairs channels, and government access channels, the retransmission of locally broadcast television signals and other appropriate services. Contractor shall provide a broadcast program guide on the standard tier package, listing at a minimum, the time and channel for all programs available to the subscriber. The standard tier service shall include the following broad categories of programming:

3.2.2. ALTERNATE TIER: MEDICAL CENTER—RESERVED: Contractor shall provide an alternate tier of CATV service, which shall include all services required by the rules of the FCC and any other federal law or regulation, public access channels, educational access channels, public affairs channels and government access channels, the retransmission of locally broadcast television signals and other appropriate services. This alternate tier of service shall be provided to the WPAFB Medical Center for distribution at the Medical Center only. The alternate tier of service shall include the standard tier service and the following categories of programming:

3.2.3. BASIC TIER: Contractor shall provide a basic tier of CATV service, which shall include all services required by the rules of the FCC and any other federal law or regulation and the retransmission of locally broadcast television signals. This basic tier of service shall be available to various situational awareness organizations on the WPAFB facility. The basic tier of service shall include the following categories of programming:

3.3. GOVERNMENT ACCESS CHANNELS: Contractor shall provide the Government three (3) Government Access Channel(s) for noncommercial use. Contractor shall provide a CATV system designed and constructed to provide government access channel(s) programming from designated locations on base and ensure that the government access channel(s) cannot be viewed outside of WPAFB. Contractor shall not be obligated by this PWS to provide program origination equipment for use in operating the government access channel(s).

3.4. EDUCATIONAL ACCESS CHANNELS: Contractor shall provide seven (7) Educational Access Channel(s), for noncommercial use and without charge to users. Contractor shall provide a CATV system designed and constructed to provide the educational access channel(s) programming to designated locations on base and ensure that the educational access channel(s) cannot be viewed outside of WPAFB.

Contractor shall not be obligated by this PWS to provide program origination equipment for use in operating the educational access channel(s).

3.5. SERVICE REQUESTS: Customers shall submit new service requests for installations, and/or additions/changes/disconnections to existing services to the COR, and 88th Communications Squadron.

Contractor shall respond to new requests within one (1) working day of receipt from COR. Contractor shall have seven (7) working days to provide a cost estimate to Customer and propose any technical solution. The technical solution shall consist of the proposed service installation route including any necessary conduit installation, trenching and any equipment necessary to provide service. Contractor shall complete installation of CATV services within 14 days of the Customer approved cost estimate.

3.6. REPAIR REQUESTS: All user requests for repairs shall be submitted to the COR who in turn shall notify the contractor of repairs needed. Contractor shall respond within (1) hour during normal duty hours and within two (2) hours during non-duty hours (if request is deferred by the user, the contractor shall respond by 8:30 a.m. on the next duty day). Contractor shall notify the COR within twenty-four (24) hours if a request for service or repair is not completed or not otherwise resolved to the user’s satisfaction.

3.7. INTERRUPTION OF SERVICE: Contractor shall at all times maintain the system in good condition and repair to provide uninterrupted service in conformance with all standards and requirements.

Contractor shall limit to the shortest time possible any interruption of service for the purpose of maintaining, repairing, or upgrading the system. Preplanned service interruptions shall be preceded by written notice to users and shall occur during periods of lower usage times (after normal duty hours if allowable). The COR shall be notified five (5) working days in advance of expected interruption of service. Said period shall begin when the contractor receives notice from a user that service has been interrupted or when the contractor has actual or constructive notice of said interruption, whichever is earlier, and shall cease when the service has been restored to the user.

3.8. COMPLAINTS: Contractor shall respond to user complaints within five (5) working days.

Contractor shall maintain written records of all complaints received during the performance of the contract.

Written records shall include the date of complaint, identity of the user, the nature of the complaint, and the contractor’s response.

3.9. PAYMENTS: Payment for CATV installation and/or service will be done with Government Purchase Card (GPC) between customer and contractor for all purchases under the allowable GPC amount.

Contractor shall provide a monthly funds expenditure report (CDRL A002) to the COR and CO that shall include a list of account numbers, organization ID’s, an account POC (with email address), subscribed service, and payment amount.

3.9.1. All CATV installation and service over the GPC allowable amount shall be estimated by the contractor and completed by a separate contract action. These contract actions shall also be included in monthly funds expenditure report (CDRL A002) submitted to the COR and CO.

3.10. CATV SERVICE INSTALLATION: Installation of new CATV requirements shall not be performed until approved by the COR and the 88th Communications Squadron. Contractor shall follow applicable industry standards, commercial practices, and Base Civil Engineer (BCE) procedures during all CATV installations. BCE procedures take precedence over installation methods unless the contractor can prove benefit of alternative methods which shall be approved by the Government before use.

3.10.1. INTERIOR INSTALLATION: Cable shall be concealed behind ceiling and walls and not be run along interior walls or baseboards, unless approved by the Government. Cable outlet boxes shall be installed in accordance with commercial practices and be set flush with finished walls.

3.10.2. EXTERIOR INSTALLATION:

3.10.2.1 BUILDING SERVICE: The diameter of any holes passing through walls shall not exceed the diameter of the cable or conduit by more than one-eighth inch. All exterior holes shall be sealed to prevent entry of moisture or insects into the facility.

3.10.2.2. EXTERIOR MOUNTING: Contractor shall install at CATV cable parallel or at right angles to walls or structural members. The mounting of cable shall follow building lines, where possible (i.e., brick, mortar, siding), in straight horizontal and vertical lines, with bends tied to building corners. Exterior cable mounting shall be prohibited where government-furnished conduit is supplied.

3.10.2.3. SERVICE DROPS: Contractor shall mount all aerial service drops parallel to utilities and or phone lines. If dual aerial service drops are used, the contractor shall be dual type cable to present a clean appearance. Contractor is prohibited from using government-furnished conduit supplied for CATV service drops, aerial service drops, and alternate underground routes.

3.10.2.4. CLEARANCES: CATV cabling shall not conflict with or cause any other cabling to violate ground or power line clearance criteria unless a waiver has been granted by the base civil engineer or a designated representative.

3.10.3. CABLE DAMAGE: Contractor shall respond to all cable cuts and/or damages within one (1) hour of notice during normal duty hours and within two (2) hours of notice during non-duty hours.

Contractor shall notify the COR and contracting office if service cannot be restored within twenty-four (24) hours of response. The responsible party that cut and/or damaged the cable shall be liable for all repair costs.

3.10.3.1. CABLE DAMAGE BY FAULT OF GOVERNEMT: Contractor shall notify the COR and contracting office of any cable cuts and/or damages by fault of the Government. After approval, the contractor shall repair cable cuts and/or damages and negotiate compensation from the Government.

3.11. UNDERGROUND CONNECTIONS:

3.11.1. Under paved areas and roadways, the CATV cables shall be installed in conduit not less than two

(2) inches in diameter. Conduit shall be zinc-coated rigid steel or scheduled 40 PVC. Conduit shall be extended not less than two (2) feet beyond pavements and roadways, when such roadway is used for vehicular traffic. Under heavily traveled roadways, conduit shall be installed by boring. Pavement may be cut and repaired in an approved manner on lesser traveled roads on approval of the base civil engineer.

3.11.2. Trenches in which direct burial cables are places shall have a minimum depth of eighteen (18) inches below grade, shall not be less than six (6) inches wide, and shall generally be in straight lines between cable connections. Bends in trenches shall have a radius of not less than thirty-six (36) inches. A marker tape six (6) inches below grade shall be used above all direct buried cable. Tape shall bear the warning “CABLE TV.”

3.12. EXCAVATION AND BACKFILLING:

3.12.1. EXCAVATION PERMIT: Contractor shall obtain an excavation permit one (1) day prior to start of work from the base civil engineer or a designated base communications group representative before commencing any digging or excavation on the Base.

3.12.2. EXCAVATION METHODS: Hand digging is required within four (4) feet of existing utilities identified by excavation permit and in areas immediately adjacent to and within approximately one (1) foot of buildings.

3.12.3. BACKFILLING: Shall be accomplished with well-pulverized soil and each twelve (12) inch layer of soil shall be compacted to a CE 55 density if at least ninety-five (95) percent.

3.12.4. OPEN EXCAVATION: Shall be barricaded when contractor personnel are not present in the immediate vicinity of the work site.

3.13. PAVEMENT CUTS: Pavement cuts shall be made only after the approval of the location and circumstances by the base civil engineer. Traffic barricades and warning lights to mark the excavation shall be provided by the contractor. The restored pavement shall be equal to or better than the original pavement.

3.14. FINAL CLEANUP: After the work is completed, the work site shall be returned to its original state.

3.15. REFUSE COLLECTION: All waste generated by the contractor in performance of the services specified within this PWS shall be deposited in government dumpsters, excluding hazardous materials in which proper disposal instructions shall be complied with. The contractor shall comply with base recycling regulations.

3.16. SERVICE SUMMARY: Contractor service requirements are summarized into performance objectives that relate directly to mission essential items. The performance threshold briefly describes the minimum acceptable levels of service required for each requirement. These thresholds are critical to mission success. The performance objectives will be measured based on customer complaints, Government Surveillance, and Corrective Actions Reports (CAR).

GOVERNMENT FURNISHED PROPERTY (GFP):

4.1. GOVERNMENT FURNISHED PROPERTY: The Government will provide power, air conditioning and adequate space for amplifiers, termination, and distribution equipment, and other necessary equipment not to exceed 90 square feet in any given building where service may be required.

Prior coordination with building custodian and 88 Communications Squadron (if for joint use of a communications closet) and approval of COR is required. Additionally, the Government will provide a fenced open area of land for the placement of satellite dishes and various equipment. In accordance with AFI 32-9003, para.3.2.4., rental payment is not required for services to support the direct mission of the contract and fulfill the objectives listed within this PWS. The use of this property for any commercial internet and cable services outside of this contract will require a separate franchise agreement, real property agreement, and rent payment. This area is approximately 0.7327 acres (31,917 SF) – please see PART 8, ATTACHMENT 1.

4.2. UNDERGROUND DUCT: Government will provide access to underground ducts only when no alternate route is available or feasible and then only on an as available basis. Prior coordination with Civil Engineering and 88 Communications Squadron and approval of COR is required.

4.3. FIBER AGREEMENT: Contractor and the Government shall, on a case-by-case basis, use each other’s distribution systems and fiber optic cable at no cost to either party by mutual agreement.

4.4. FACILITIES: Any facilities the contractor may request for use during the performance of this PWS shall be requested through 88 ABW/CEIAP (Real Property Office). This PWS makes no provision for facilities located on WPAFB.

PART 5

DEFINITIONS & ACRONYMS:

5.1. DEFINITIONS:

5.1.1. CONTRACTOR. A supplier or vendor awarded a contract to provide specific supplies or service to the government. The term used in this contract refers to the prime.

5.1.2. CONTRACTING OFFICER. A person with authority to enter into, administer, and or terminate contracts, and make related determinations and findings on behalf of the government. Note: The only individual who can legally bind the government.

5.1.3. CONTRACTING OFFICER'S REPRESENTATIVE (COR). An employee of the U.S. Government appointed by the contracting officer to administer the contract. Such appointment shall be in writing and shall state the scope of authority and limitations. This individual has authority to provide technical direction to the contractor as long as that direction is within the scope of the contract, does not constitute a change, and has no funding implications. This individual does NOT have authority to change the terms and conditions of the contract.

5.1.4. KEY PERSONNEL. Contractor personnel that are evaluated in a source selection process and that may be required to be used in the performance of a contract by the Key Personnel listed in the PWS. When key personnel are used as an evaluation factor in best value procurement, an offer can be rejected if it does not have a firm commitment from the persons that are listed in the proposal.

5.1.5. PHYSICAL SECURITY. Actions that prevent the loss or damage of Government property.

5.1.6. QUALITY ASSURANCE. The government procedures to verify that services being performed by the contractor are performed according to acceptable standards.

5.1.7. QUALITY ASSURANCE Surveillance Plan (QASP). An organized written document specifying the surveillance methodology to be used for surveillance of contractor performance.

5.1.8. QUALITY CONTROL. All necessary measures taken by the contractor to assure that the quality of an end product or service shall meet contract requirements.

5.1.9. SUBCONTRACTOR. One that enters into a contract with a prime contractor. The Government does not have privity of contract with the subcontractor.

5.1.10. UPTIME. Uptime is a measure of an application or website availability to its end users.

5.1.11. WORK DAY. The number of hours per day the contractor provides services in accordance with the contract.

5.1.12. WORK WEEK. Monday through Friday, unless specified otherwise.
