Performance Work Statement For

Temporary Trailer for Fire Station 2

1.0. DESCRIPTION OF SERVICES

1.1. Scope of Work. The contractor shall provide all equipment, tools, parts, transportation, personnel, labor, supervision, and management to provide and install one temporary modular sleeping, living and hygiene building. This modular building will be located south of building 1713, Fire Station 2 for FBNV210006, Construct Temporary Trailer for Fire Station 2, on Davis-Monthan AFB (DMAFB), AZ.

1.2. Tasks. The contractor shall perform standard set-up, lease, and removal of temporary modular sleeping/living quarter facility and all ancillary equipment at the end of the contract period. The contractor shall be responsible for all maintenance and repair of the modular facility for the duration of the contract period, which includes cleaning and servicing the restroom. All labor and materials will be installed in accordance with applicable codes.

1.2.1. Size. Minimum building size is 600 square feet for temporary sleeping, kitchen, office/computer area/living and hygiene areas.

1.2.1.1 Rooms. Required sleeping space is approximately 100 square feet per person.

Must be two separate sleeping rooms. Must accommodate two personnel for 24/7 operations. Must include one shower, one toilet and one restroom sink. The maximum area the building can occupy is roughly 54’ X 24’ feet. Must include kitchen area with kitchen sink, range, oven and refrigerator. Must include a common living/office/computer area that is separate from the sleeping area.

1.2.2. Installation. Building installation shall consist of finishing interior and exterior seams to include block, level, and seam building. Building will be placed on an unprepared surface and preparations may be necessary.

1.2.2.1. Floors. Floors shall be vinyl tile suitable for a sleeping/living/kitchen area.

Tile will be professionally seamed and installed across mate line as appropriate to fully conceals seams.

1.2.2.2. Ceiling Seam. The ceiling seams shall be acoustic T-Grid ceiling tile site installed totally conceal mate line.

1.2.2.3. Wall Seams. The wall seams shall be vinyl covered battens

1.2.2.4. Exterior Seam Trim. The exterior seam trim shall be as appropriate for siding.

1.2.2.5. Roof Seam. The roof seam trim shall be material recommend by roofing supplier.

1.2.2.6. Matching Skirting. Contractor shall install skirting matching the color of the building.

1.2.2.7. Electrical Service. NEMA 3-R disconnect as required, mounted on proposed modular building and connected to subpanel located in modular unit.

1.2.2.7.1 Connection from transformer to main electrical distribution panel/disconnect switch will all be done by contractor. Contractor is responsible for providing connection between modular sleeping trailer and distribution panel/disconnect switch.

1.2.2.8. Lighting. Lighting shall consist of standard overhead fluorescent lights.

1.2.2.9. Windows. Each exterior room shall have at least one window.

1.2.2.10. Entrance/Exit Doors. Building shall consist of at least two entry/exit doors and must meet building and fire code. There shall be a light attached to the building outside each entry/exit door.

1.2.2.11. Entry Stairs/Ramp. Building shall have stairs or ramp to the entrance.

1.2.2.12. Heating/Cooling. The building shall include a heating, ventilation, & air conditioning (HVAC) unit to handle heating and cooling needs of the building.

1.2.2.13. Communications. The building shall include receptacles that meet building code. The trailer will need 4 NIPR ports and 14 receptacle outlets.

1.2.2.14. Color and Design. The color and design of the modular building shall be consistent with the architectural character, form, and colors of adjacent structures. The building color shall be the equivalent, to match the base color scheme.

1.2.3. Restrooms. The contractor shall be responsible for all maintenance and repair of the restroom for the duration of the contract period, which includes cleaning and servicing the restrooms. All labor and materials will be installed in accordance with applicable commercial codes, to include a waste tank. The contractor shall service the restroom bi weekly, which includes emptying the waste tanks, and supply paper towels and toilet paper.

1.2.4. Contractor Submittals. The contractor shall submit a delivery schedule, floor plans, exterior elevations, exterior color samples, and site plan to Contracting Officer for approval.

1.2.5. Tear Down and Removal. The contractor shall perform removal of the temporary modular building within 30 days of contract expiration if the option is not exercised.

1.2.6. Maintenance and Repairs. The contractor shall be responsible for all maintenance and repairs that is a result of age of material or contractor workmanship. Government will be responsible for maintenance and repairs that are a result of Government negligence.

1.2.7. Deliverables. The contractor shall produce and deliver all documents relating to the proper care and maintenance of the modular building and all systems that accompany the modular building. Additional deliverables may be requested as deemed necessary, but will not exceed manuals for systems in or on modular building.

1.2.7. Warranty Replacement. The contractor shall be responsible for honoring manufacturers’ and contract warranties.

2.0. GENERAL INFORMATION

2.1. Contractor Personnel Qualifications. Contractor personnel must be fully trained and qualified to perform modular building installation services.

2.2. Contract Manager. The contractor shall designate a contract manager who shall be responsible for contract performance and can act for the contractor on all matters relating to daily operation of the contract. The Contract Manager may be a contractor employee performing services. Upon contract award, the contractor shall provide the name and phone number of the contract manager, in writing, to the Contracting Officer. The contract manager shall be available by telephone during regular business hours. The contract manager must be able to read, write, speak, and understand English fluently.

2.3. Contractor Duty Hours. The contractor shall perform installation services between the hours of 7:00 a.m. to 4:00 p.m., Monday through Friday, excluding Federal Holidays.

Duty hours may be adjusted, upon mutual agreement between the Government and the contractor.

2.4. Federal Holidays. The following Federal Holidays are observed at DMAFB.

The contractor is not required to perform services on federal holidays, or the actual day of observance.

2.5. Security Requirements

2.6.1 Listing of Employees. The contractor shall maintain a current listing of employees performing services on DMAFB. The list shall include the employee's name, driver’s license number and state of issue, and social security number. Upon contract award, the list shall be provided to the Contracting Officer. An updated listing shall be provided when an employee's status or information changes.

2.6.1. Pass and Identification Items. The contractor shall ensure the pass and identification items required for contract performance are obtained for employees and non-Government owned vehicles.

2.6.2. Retrieving Identification Media. The contractor shall retrieve all identification media, including vehicle passes from employees who depart for any reason before the contract expires;

e.g. terminated for cause, retirement, etc., and turn them in to the issuing office.

2.6.3. Traffic Laws. Contractor employees shall comply with all base traffic regulations.

Contractor employees are subject to random vehicle speed control checks. Contractor personnel cited for speeding on the installation may suffer loss of base driving privileges, debarment from the base, or other administrative action.

2.6.4. Random Personnel and Vehicle Searches. Contractor personnel are subject to random personnel and vehicle searches. If contractor personnel refuse to be searched, they will be denied entry to the base, and may result in loss of base driving privileges, debarment from the base, or other administrative action.

2.6.5. Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor owned vehicle or privately owned vehicle while on Davis-Monthan AFB.

2.6.6. For Official Use Only (FOUO). The contractor shall comply with DoD 5400.7-R, Chapter 4, DoD Freedom of Information Act (FOIA) Program requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding FOUO material.

2.6.7. Reporting Requirements. Contractor personnel shall immediately report to an appropriate Government authority any information or circumstances of which they are aware may pose a threat to the security of Department of Defense personnel, contractor personnel, resources, and classified or unclassified defense information.

2.6.8 Physical Security. Contractor employees shall be responsible for safeguarding all Government property and controlled forms provided for contractor use. At the end of each work period, all Government facilities, equipment, and materials shall be secured.

2.7 Safety. The contractor shall comply with all applicable laws and safety regulations. Compliance with OSHA and other applicable laws and regulations for the protection of employees is exclusively the obligation of the contractor, and the Government will assume no liability or responsibility for the contractor’s compliance or noncompliance with such responsibilities.

2.7.1. Fire Prevention and Safety Standards. Contractor employees shall comply with the Occupational Safety and Health Act (OSHA), Davis-Monthan AFB Fire Protection Program, Air Force Occupational Safety, and the Fire Prevention and Health Program.

2.7.2. Smoking in AF Facilities. Contractors are advised that the Air Force has placed restrictions on the smoking of tobacco products in Air Force facilities. AFI 40-102, Tobacco Use in the Air Force, outlines the procedures used by the commander to control smoking in our facilities. Contractor employees and visitors are subject to the same restrictions as QAP. Smoking is permitted only in designated smoking areas.

2.7.3. Work Area. The contractor shall, at all times, keep work areas free from accumulations of waste material, rubbish, tools, scaffolding, equipment, and materials. Upon completion of the job or when away from the work site, the contractor shall leave the work area and premises in a clean, neat, safe and workmanlike condition. The contractor is responsible for removal and proper disposal of all job related waste material at a permitted site.

2.8. Contractor Vehicle Requirements. All vehicles used in the performance of the contract shall be properly registered and meet local, state, and federal safety and emissions requirements. Each contractor vehicle shall have the contractor logo neatly exhibited on each side of the cab. Vehicles shall be in satisfactory mechanical condition and present a neat and clean appearance. Any vehicle(s) not so maintained may be ordered removed from the base by and at the sole discretion of the CO, until satisfactory repairs are made. The contractor shall operate vehicles to comply with base traffic regulations. Vehicle-mounted communication equipment shall conform to all applicable base and federal frequency regulations and avoid conflict with existing frequencies.

3.0. GOVERNMENT FURNISHED PROPERTY AND SERVICES

3.1. Utilities. The Government will provide the contractor with a source of electricity for use in performance of this contract. The contractor shall ensure employees practice utility conservation, and operate under conditions that prevent the waste of utilities.

3.2. Emergency Services. The Government will provide security forces services (call 228- 4444, for emergencies only), and fire protection services (call 228-3333). There are no emergency medical services available on Davis-Monthan AFB, therefore the contractor must dial 911 for emergency medical services.

4.0. Environmental Protection Compliance

4.1.1. Adherence to the DMAFB Contractor Environmental Guide (CEG). Adherence to environmental guide is mandatory and will help increase the contractor’s awareness of the installation’s Environmental Management System (EMS) and environmental regulatory requirements that impact DMAFB. The guide covers the significant environmental aspects and impacts that contractors often influence at DMAFB. A list of these aspects can be found in Appendix F, Environmental Management System (EMS) Most Significant Environmental Aspects Awareness Form, of the guide. Upon contract award, the contractor shall complete Appendix F and submit it to the CO. The CEG will be provided by the CO. Questions concerning the CEG should be directed to the Program Manager (355 CES/CEIE) at 228-4840.

4.1.2. Environmental Management System (EMS) Awareness Training. The contractor shall comply with the DMAFB CEG in completing the required EMS Awareness Training prior to start of work. The training is a requirement for project managers, on-site managers/supervisors, and contractor personnel who have a direct influence on the significant aspects identified in appendix F of the guide.

Training registration and login instructions can be found in appendix E, EMS Awareness Level Training Login and Registration Instructions, of the CEG. The contractor shall submit a completed copy of the training certificate along with Appendix F to the Contracting Officer upon contract award to document completion of training.

4.1.3. Environmental Laws and Regulations. The contractor shall comply with all applicable Federal, State, and local laws, regulations, and requirements regarding environmental protection. In the event environmental laws/regulations/requirements change during the term of the contract, the contractor shall comply with such law/regulation as changed. The contractor is responsible for any penalties, fines, etc.

resulting from violations. The contractor shall immediately notify the COR when violations occur, and cooperate fully with fire department personnel and Government investigators.

4.1.4. Environmental Permits. Contractors shall comply with all regulatory requirements to include permits if applicable. Pima County Department of Environmental Quality (PDEQ) and Arizona Department of Environmental Quality (ADEQ) require permits for asbestos removal or demolition as well as projects that require earth-moving or soil disturbance. These requirements may include storm protections. The contractor shall treat areas subject to dust producing activities with liquid palliatives which will not harm regrowth of vegetation, or other methods of dust control. The contractor shall also ensure methods to protect native plants, birds and wildlife are included in projects where required. All permit costs shall be considered as having been included as a factor in the contractor's bid price.

4.1.5. Hazardous Materials/Waste. The contractor shall be responsible for the proper management of both hazardous and non-hazardous waste. The contractor shall segregate hazardous or regulated materials and non-hazardous or non-regulated materials and handle, transport, store, and dispose of those materials, in accordance with the Resource Conservation and Recovery Act, all applicable federal, state, and local statutes and regulations, and in accordance with appendix C, Hazardous, PCB, and Universal Waste: Proper Management and Disposal, of the Contractor Environmental Guide. The contractor shall coordinate all hazardous shipments with the 355 Civil Engineer Squadron Hazardous Waste Program Manager (355 CES/CEIE) at 228-4840.

4.1.6. Hazardous Material (HAZMAT) Handling. The contractor shall have approval from Hazardous Waste Program Manager (355 CES/CEIE) at 228-4840, in accordance with the CEG, prior to handling, using, and storing any chemicals, solvents, lubricants, and other products that require Safety Data Sheet (SDS) or Material Safety Data Sheet (MSDS) on the installation. The contractor shall plan, fill out, and submit a HAZMAT worksheet to the COR prior to bringing HAZMAT on the base - see appendix A, Guidance For Submission Of Safety Data Sheets For Hazardous Material (HAZMAT) and Reporting of Hazmat Usage, and Attachment(s) A-1 to A-3 of the CEG for details. The contractor shall maintain one copy of the MSDS or SDS for each HAZMAT line item used within the work center.

4.1.7. Prohibited Substances/Chemicals. The contractor shall not use products that are, or contain Extremely Hazardous Substances (EHS), Ozone Depleting Substances (ODS), and/or Persistent Bio-accumulative and Toxic (PBT) chemicals. Any HAZMAT containing one of these banned substances will not be allowed on base. Contact the DMAFB Environmental Office (355 CES/CEIE) at 228-7201 if you have any questions on materials.

4.1.8. Hazardous Materials/Waste Spills. The contractor shall take every reasonable precaution to prevent spills of HAZMAT, and immediately report any HAZMAT spills to the designated 309 AMARG POC and base fire department by calling 228-3333, in accordance with Appendix J, HAZMAT Incident Response Procedures, of the DMAFB Spill Prevention, Control, and Countermeasure (SPCC) Plan. The contractor shall comply with this plan when handling and storing all oils and fuels. The Government Project Manager will provide the SPCC Plan to the contractor, upon contract award. The contractor shall clean and dispose of all HAZMAT spills in accordance with local, state, and federal laws and regulations and the Contractor Environmental Guide. The contractor shall bear full financial obligation incurred from cleaning and disposing of HAZMAT spills, or release of HAZMAT within the confines of DMAFB by contractor employees.

4.1.9. Spill Containment Kits. The contractor shall ensure that all vehicles used for transport of HAZMAT on DMAFB, carry a spill containment kit capable of containing a spill, until first responders arrive. The contractor shall make spill containment kits available for inspection by Government personnel, upon request.

4.1.10. Hazardous Materials Inventory. The contractor shall maintain an up-to-date hazardous material inventory with copies of Safety Data Sheets (SDS) for all materials used on the job site. The contractor shall maintain a complete copy of all approved SDS’s and AF Form 3952s.

4.2. Hazardous Materials Management. The contractor shall remove all hazardous materials from work areas by the end of each workday and store in approved flammable locker with inventory on cabinet door exterior. Contractor shall not conduct process treatments (e.g. paint thinner solvent recycling) on base.

4.2.1. Final Report. The contractor shall submit a final inventory, not later than ten working days following termination/completion of this contract, of all hazardous materials used since the last hazardous usage report to the COR. The final inventory will list all hazardous materials used, total amount of each used, and a description of the disposition of any remaining hazardous material.

4.3. Permits, Licenses and Certifications. Unless otherwise specified in this contract, obtain necessary permits, licenses, and certifications; give all required notices; and comply with applicable Federal, State, County, and local laws, codes, and regulations in performance of the requirements of this contract. Maintain records of such requirements and make the documentation available to the Government for review. All pesticide application will be made by State or Host Nation certified applicator as prescribed in DoDI 4150.07. Permits shall be included as a factor in the contractor’s bid price. Provide copy of approved license by the 5th day after award PoP.

4.4. Interfaces. Do not unduly interfere with regularly scheduled Government operational activities in the performance of contract requirements. In the event a Government employee requests to temporarily cease work in the area, report the instructions, including the name of the Government employee involved, to the CO or COR immediately by the most expedient means. Notify the COR verbally of disputes with customers or other base contractors and follow-up in writing as necessary.

4.5. Safety Requirements and Reports. The contractor shall perform work in a safe manner as required by Occupational Safety and Health Act (OSHA) Safety and Health Requirements. Provide a verbal report to the COR as soon as possible of each occurrence of damage to Government property or an accident resulting in death, injury, occupational disease, or adverse environmental impact. Provide a completed copy of required Accident Investigation Reports to the COR within five calendar days of each occurrence.

4.6. Burrowing Owls. There are several owl burrows on DMAFB, marked with signs. The contractor shall observe/adhere to the warnings on the sign to protect the native species.

4.7. Cacti Preservation. The contractor shall not move or harm any saguaro cacti. A permit is required to move or plant saguaro cacti. If directed by the COR to move barrel cacti, the contractor shall "salvage" the barrel cacti, by moving it to a Government designated location for storage or transplant it to another designated landscaped area.

4.8. Monsoon Season. During months of July – September, DMAFB experiences hurricane winds, lightning, and thunderstorms capable of dropping 4 inches of water per hour. The contractor should stay informed daily/hourly through local weather stations as conditions can change rapidly.
