PERFORMANCE WORK STATEMENT

FOR

INTEGRATED PEST MANAGEMENT SERVICES AT MACDILL AFB FLORIDA

1. DESCRIPTION OF SERVICES. The contractor shall provide all management, licenses, laboratory testing, tools, supplies, equipment, transportation and labor necessary to execute the Integrated Pest Management Plan (IPMP) plan at MacDill AFB, FL in a manner that will ensure the health and general well being of installation personnel. The following types of pests are common on this installation, and their abatement should be specifically addressed in the IPMP and used to determine workload: Health Related Pest (mosquito control), Structural pest, Stored Product Pest, General Household and Nuisance Pest, Miscellaneous Pest, Rodent Control, Nuisance Wildlife Control, Road Kill as detailed in the current MacDill IPMP.

1.1. BASIC SERVICES. The contractor shall review and update the existing IPMP yearly. The IPMP will also require yearly approval of the AFCEC Pest Management Consultant (PMC). The contractor’s IPMP shall establish the strategy and methods for conducting a safe, effective, and environmentally sound pest management program. Contractors may review the current IPMP to implement and improve yearly updates of the current IPMP. IPMP update is separately priced and is indicated in the bidding schedule of this contract. Current Fiscal Year 2019 IPMP and associated maps are indicated in Appendix F of this PWS.

The IPMP shall comply with Department of Defense Instruction (DoDI) 4150.07, Department of Defense (DoD) Pest Management Program, 26 Dec 2019; and AFI 32-1053, Integrated Pest Management Program, 6 August 2019.

The IPMP emphasizes the use of an integrated pest management program to prevent and control pests and disease vectors. This factor is part of the IPMP approach which emphasizes monitoring, pest prevention, the use of specific control measures adapted to the target pest, and a minimal use of chemicals.

1.1.1. FOOD SERVICE AND FOOD STORAGE AREAS. The contractor shall perform integrated pest management services at food service and food storage areas, in coordination with the base Public Health (PH) office and the COR. Pest control methods of treatment shall be coordinated with PH and the COR. Food service and food storage area locations are indicated in Appendix D of this PWS.

1.1.2. MILITARY FAMILY HOUSING AND TINKER ELEMENTARY SCHOOL. The contractor is not responsible for integrated pest management services at any MFH or Elementary School buildings. These buildings are indicated in Appendix B, Maps, of this PWS. The contractor is responsible for day time mosquito fogging and larval surveillance service at MFH. The contractor is responsible for mosquito larval treatment at MFH.

1.1.3. CHILD DEVELOPMENT CENTERS. The contractor shall perform integrated pest management services at the child development centers in coordination with PH and the COR. Pest control methods of treatment shall be coordinated with PH and the COR.

1.1.4. MOSQUITO CONTROL MEASURES. The contractor shall perform mosquito control measures, such as adult mosquito spraying (ultra-low volume application), larvae treatment, and habitat control, elimination, and/or sanitation, in coordination with PH and the COR. Recommendations for the need, frequency, and timing of mosquito control measures are based on contractor’s mosquito surveillance program. Coordinate with PH and the COR concerning mosquito control measures. Contractor is only responsible for ground mosquito control. Hillsborough County Mosquito Control (HCMC) assists MacDill AFB with all mosquito control aerial flights. HCMC provides pilot personnel and plane service at no cost to MacDill AFB. Contractor shall coordinate HCMC aerial flights in accordance with Annex 6 of IPMP. The contractor shall provide day time adult mosquito spraying fogging service (ultra-low volume application) to MFH and the Base. The contractor shall provide mosquito larval treatment at the Base and MFH. HCMC follows the December 2016 MacDill AFB Environmental Assessment for Aerial Application of Pesticides and Pesticides.

1.1.5. MACDILL AFB INTEGRATED PEST MANAGEMENT SELF HELP MANAGEMENT PROGRAM. The contractor shall provide and maintain an Integrated Pest Management Self Help Management Program for MacDill AFB building managers in accordance with Air Force Engineering Technical Letter (ETL) 10-5: Self Help Pest Management Program. The following pests are included in this self help pest management program: cockroaches, ants ( except for carpenter ants), centipedes, crickets, earwigs, firebrats, flies, silverfish, sow bugs ( pill bugs), mice, and miscellaneous flying and crawling insects that do not destroy Air Force property. All of these pests can be prevented and controlled through IPMP measures such as good housekeeping, proper food storage, pest exclusion, and minimally use of commercially available general-use pesticides or self – help insecticides or supplies. Contractor shall provide building managers with detailed instructions for application/use of approved IPMP and self-help pesticides in accordance with Air Force ETL 10-5. Contractor shall provide, stock, store, and dispense self help pest control materials to building managers. Contractor shall keep records of self help pesticide dispensed using the Integrated Pest Management Information System (IPMIS) computer system database. Contractor shall issue no pest control materials other than those specified in AF ETL 10-5 and the MacDill IPMP. Other self help pest control materials not specified in AF ETL 10-5 or the IPMP, may be used with prior approval from AFCEC Pest Management Consultant. Contractor shall deliver self help materials to each building manager as required/needed for each pest control call. If self help pest control methods as applicable are found to be non-conducive to treatment after 30 days or longer, the contractor shall use other methods of pest control treatment in accordance with IPMP.

1.1.6. NEW INTEGRATED PEST MANAGEMENT INFORMATION SYSTEM (IPMIS) SECURED GOVERNMENT NETWORK INTERNET PEST MANAGEMENT RECORD KEEPING. The AFCEC Command Entomologist is currently working on obtaining a new improved Integrated Pest Management Information system (IPMIS) secured computer network system. The contractor must be able to obtain a CAC card upon government request to be able to enter pesticide usage data information as often as required into this computer network system. The computer terminal to be used to enter this data information will be located at the service contracts office, Bldg. 30, CE Building. The contractor shall comply with new and future DOD IPMIS software system updates and pesticide data request updates as required by the AFCEC Command during the life of the contract.

The contractor shall summarize daily recorded pesticide applications for the previous month on a secured government network Integrated Pest Management Information System (IPMIS) computer generated data report as required by AFCEC pesticide reporting requirements. This report shall be submitted to the COR electronically by email by the 2nd working day of the following month. The contractor shall submit daily pesticide applications thru the IPMIS secured government network internet system. The contractor shall provide AFCEC Command Entomologist yearly updates of detailed pesticide usage, and licensing certification in accordance with Measure of Merits Report. Failure to comply with these requirements will result in a corrective action report.

1.1.6.1 CURRENT INTERIM MONTHLY PESTICDE DATA REPORT REQUESTS

Prior to implementation of new IPMIS secured government network data reporting, the Contractor shall submit a report to the COR within 2 working days following each month listing all pesticide applications made. Include pesticide name, use concentration, amount applied, date, and detailed location of use. A DD Form 1532 will be provided on request to facilitate record keeping. Failure to comply with these requirements will result in a corrective action report.

1.1.7. PESTICIDE SAFETY DATA SHEETS (SDS). All pesticide hazardous materials contemplated for use or brought on base for the purpose of this contract, shall have an SDS. The contractor shall submit to the Pest Management Contract COR, Hospital (Bioenvironmental Engineering), Fire Department, and the AFCEC Command Entomologist, an SDS and a specimen label of all pesticides used in this contract. Prior to start of contract, and whenever a new pesticide is used during the contract, the contractor shall submit an S.D.S and an specimen label for each pesticide used for this contract to the AFCEC Command Entomologist in coordination with the Pest Management Contract COR. Whenever a pesticide specimen label changes for the current list of pesticides, the contractor shall resubmit a new label. Pesticides that are not approved/ listed in the AFCEC Command list of approved pesticides shall be reviewed and approved using the Request for Approval of Non-Standard Pesticide form 20160916. Failure to comply with these requirements will result in a corrective action report.

1.1.8. RECORD KEEPING ACCESS TO WORK ORDER COMPUTER GENERATED INFORMATION SYSTEM.

At contractor’s expense, the contractor shall provide a work order computer generated information system that will maintain the records of all current and archived work orders. The contractor will provide an “enterprise work order system” that is available to COR, contractor, and customer. The “enterprise work order system” will not be on USAF network unless it is COR approved. One hundred percent of all work accomplished as part of this contract must be entered in the work order management system. This system will provide effective utilization of tracking contractor’s customer support. The work order computer generated information system will provide a “self Help” support system that will provide the government with the following information: Customers are able to create their own work order, COR is able to do reporting and metrics, and contractor is able to edit and maintain work order data in the system.

During periods of system unavailability or downtime, regardless of the reason, contractor shall immediately implement procedures to capture all work load data via spreadsheet until system is available. Contractor will load all records from spreadsheet to system within 24 hours of system availability. During periods of system downtime greater than one week, contractor will provide a weekly copy of spreadsheet via email or CD/DVD. Currently, there is no network connectivity between contractor’s building and the rest of MacDill AFB.

Regardless of system availability, the contractor will load all work order information generated by email, phone, or verbal requests. Contractor is responsible to report system availability issues immediately to COR when system is on line or offline.

The contractor will provide an export of each fiscal year work order documents. The government will have full access to download a complete copy of the data at any time as the work order data is the property of the government.

This work order data and report and query information will be provided at the beginning of each month or upon COR request during the duration of the contract. The contractor will provide the following data and report and query information to the government:

1.1.8.1. DATA ENTERING REQUEST INFORMATION. At a minimum the contractor will provide the following required and available fields. Required fields: Date of Request, Requestor’s Name, Facility Manager’s Name , Facility Number, Requestor’s Phone Number, Facility Manager’s Phone Number, , Description of Pest Problem: Indicate in this field whether work order is a newly customer generated work order , contractor’s generated work order to Inspect a previous pest problem ; work order assigned to a facility, work order assigned to a same facility or a group of facilities ( example: mosquito fogging and food service areas ) until the same pest problem is resolved ; action taken each time the contractor visits a facility or a group of facilities, action taken each time the contractor visits the same facility or a group of facilities until the same pest problem is resolved, etc. ; detailed type of pest serviced ( ants, Bees/Wasps, carcass removal, miscellaneous, mosquito, raccoons and wildlife, roaches, rodents , termites, etc. ) ; contractor’s pesticide applicator name; general categories of pest Control service treatment ( carcass removal, fog/ULV/or space treatment, manual pesticide application, pest surveillance, power pesticide application, self- help operations, termite inspections, and trapping ). Available Fields: Facility Name, Facility Address. Please also see Appendix I, Suggested IPM Services Record Keeping Access to Work Order Generated System, Data Entering Request Information for more information.

1.1.8.1.1. DATA REPORT AND QUERY INFORMATION BASED ON DATA REQUEST INFORMATION PROVIDED IN PARAGRAPH 1.1.8.1. Access upon COR request to current, all open, close, and archived work orders during the duration of the contract. COR will have access to all fields described in paragraph 1.1.8.1. At a minimum, the contractor will provide the following predefined queries/reports with their associated fields:

Monthly Reports:

Open Work Orders: Open Date, days open, requestor’s name.

Closed Work orders: Open Date, Closed Date, requestor’s name.

Number of Work Orders.

Number of Work Orders by Detailed Type of Pest Serviced

Number of Work Orders by Work Order Assignment Operational Hour Percentages of General Categories of Pest Control service Treatment: Percentage of hours spent in carcass removal, fog/ULV/ or space treatment, manual pesticide application, pest surveillance, power pesticide application, self-help operations, termite inspections, and trapping.

Customer Surveys: Customer Input of Contractor’s Quality of Work Performed. Customer surveys are voluntary at customer convenience. All completed work orders with no customer survey input will be automatically closed after 30 days.

Please also see Appendix J, Suggested Pest SC Record Keeping Access to Work Order Generated System, Data Report and Query Information for more information.

1.1.9. SUBTERRANEAN TERMITES TREATMENT FOR BUILIDNG FACILITIES; DRYWOOD TERMITES FUMIGATION FOR BUILDING FACILITIES; AND ANT TREATMENT OF BALLFIELDS, TRAINING AREAS, RUNNING AREAS, ETC. MacDill AFB may require the contractor during the duration of this contract to treat for subterranean termites at building facilities; dry wood termites fumigation at building facilities; and ant treatment at ball fields, training areas, running areas etc. This work is separately priced and is indicated on the bidding schedule of this contract. Possible ant treatment areas are indicated in Appendix B, Maps of this PWS.

1.1.10. PROTECTION OF ENDANGERED SPECIES. All pest control activities shall be accomplished in a manner that assures maximum protection of endangered species and environmental quality in accordance with IPMP.

1.1.11. REMOVAL AND DISPOSAL OF DEAD ANIMALS OUTSIDE OF BUILDING FACILITIES AFTER WORKING HOURS, WEEKENDS, AND ON HOLIDAYS. MacDill AFB may require the contractor during the duration of this contract to remove and dispose of dead animals outside of building facilities after working hours, on weekends, and on holidays. Working hours are 0730-1630 Monday-Friday. Weekends are Saturday-Sunday. Holidays are all Federal Holidays indicated on paragraph 4.1 of this PWS. This work is separately priced and is indicated on the bidding schedule of this contract.

1.1.12. MOSQUITO LARVAL SURVEILLANCE AND TREATMENT SERVICES. The contractor shall provide an Initial Larval Mapping and Survey Inspection and an Ongoing Larval Mosquito Surveillance as follows:

1.1.12.1 INITIAL LARVAL MAPPING SURVEY AND INSPECTION. The contractor shall provide a ground survey inspection identifying larval habitats. If required the contractor shall provide an aerial survey. The contractor shall provide this information via GIS Database using GPS coordinates. The contractor shall also coordinate potential treatment schedules with HCMC.

1.1.12.2 ONGOING LARVAL MOSQUITO SURVEILLANCE. The contractor shall provide a potential monthly larval habitat surveillance and inspection. The monthly surveillance and inspection shall last for a period of twelve months per year. At the beginning of each mosquito season, the contractor shall provide a schedule to the COR. All applicable larval mosquitoes shall be predicted using tidal and weather data. All breeding sites shall be mapped into GIS data base using GPS coordinates. All surveillance findings shall be reported weekly to the COR, CE CAD and IT Department, HCMC, and Public Health. The contractor shall coordinate all required larval treatment with HCMC.

1.1.12.2.1 ONGOING LARVAL MOSQUITO TREATMENT. The contractor shall provide a potential larval habitat treatment. The treatment shall last for a period of twelve months per year. At the beginning of each mosquito season, the contractor shall provide a schedule to the COR. All applicable larval mosquitoes shall be treated using information as indicated in paragraph 1.1.12.2 of this PWS. All treatment shall be mapped into GIS data base using GPS coordinates. All completed larval treatment shall be reported weekly to the COR, CE CAD and IT Department, HCMC, and Public Health. The contractor shall coordinate all required larval treatment with HCMC. All aerial larval treatment will be provided by HCMC.

1.1.12.3 LARVAL INSPECTION AND SURVEILLANCE LOCATIONS AT MACDILL AFB. The contractor shall inspect and surveil mosquito larva at MacDill AFB at locations such as permanent water, temporary fresh floodwater, tidal marsh, artificial containers and catch basins.

1.1.12.4 LARVAL INSPECTION REPORTING. The contractor shall provide monthly and annual reports to the COR, CE CAD and IT Department, HCMC, and Public Health. The contractor shall routinely coordinate and communicate with HCMC all MacDill AFB larval inspection surveillance and treatment.

In addition to the larvicide surveillance and weekly treatment reports, the contractor shall provide a cumulative integrated monthly report from day one to present that will indicate the following

Information: Contract Number, Application date, application time, site location, site conditions, larva activity, larva stage, predators present, biting, area treated in square feet, chemical name, amount in ounces, and inspector.

The contractor shall coordinate with our Civil Engineering Information Technology and CAD departments to provide the government on demand access to this data anytime it is required by the DOD Pest Management Program without the contractor's assistance.

All data generated for this contract will become property of the government.

1.1.12.5 MOSQUITO MANAGEMENT PLAN. The contractor shall provide an up to date mosquito management plan indicating all information and coordination required for this effort. This plan will be included in the IPM Plan.

1.1.13. COMPREHENSIVE REVISION OF FY 24 IPM PLAN. The contractor shall review, update, revise, and add new information as required to match new format section E5 Enclosure 5 of DODI 4150.07 to the current FY IPM Plan. The IPM Plan revision shall be provided in accordance with Appendix K, Performance Work Statement, Preparation of Integrated Pest Management Plan, MacDill AFB Florida of this PWS.

1.1.14. GOVERNMENT FURNISHED PROPERTY PELLET GUN. The government will provide the contractor a pellet gun and associated equipment in accordance with AFI 32-1053, Integrated Pest Management Program, 20 November 2014, paragraphs 3.7.13.2.1 and 3.7.13.2.2. Please also refer to revised Appendix E, Government Furnished Property for a detailed pellet gun property list. Whenever it becomes necessary to carry the pellet gun on base outside of his pest management building compound, the contractor shall inform security forces and wear a blaze orange vest labeled “ Pest Management “.

2. SERVICES SUMMARY.

2. 1. SERVICES DELIVERY SUMMARY. The Services Delivery Summary identifies the performance objectives and performance thresholds for all critical tasks associated with providing the services outlined in this PWS. This section contains only those items considered most important for mission accomplishment. The performance objectives are those services/tasks required in the PWS. The performance threshold briefly describes the minimum acceptable levels of service required for each requirement. The thresholds are critical to mission success. The Government expects the Contractor to perform at the acceptable levels throughout the life of the contract.

4. GENERAL INFORMATION.

4.1. Hours of operation. The contractor shall perform the services required under this contract during the following hours: 0730-1630 normal hours Monday-Friday.

4.2. Contractor Personnel. The contractor shall provide an on-site point of contact to the Contracting Officer for all contractual matters. This point of contact shall be available within 60 minutes during normal hours of operation to meet with government personnel to discuss problem areas. Contractor personnel shall present a neat appearance and be easily recognized. This shall be accomplished by wearing uniforms bearing the name of the company. Contractor personnel shall not smoke in facilities, block fire lanes at any time, or prop open exit doors while performing pest management services. Contractor personnel shall park their vehicles in the area designated by the facility manager. Contractor personnel will interrupt their work at any time to allow for the passage of patrons, personnel, and/or visiting dignitaries. Contractor personnel will comply with all safety and security regulations in accordance with base policy and/or specific site or building requirements as outlined in the MacDill IPMP.

4.2.1. Customer Complaint Service Call. Contractor shall provide a customer complaint service call point of contact during normal duty hours to dispatch personnel in response to customer complaints.

4.3. Physical Security. The contractor shall make prior arrangements to enter designated secure areas at MacDill AFB. The contractor personnel will be escorted at all times in secured areas.

4.4. PERFORMANCE OF SERVICES DURING CRISIS DECLARED BY THE PRESIDENT OR SECRETARY OF DEFENSE. IAW DoDI 3020.37 (Continuation of Essential DoD Contractor Services During a Crisis), and aforementioned Air Force implementation, unless otherwise directed by an authorized Government representative, it is determined that services may be performed on an extended basis up to 24 hours per day, as required. Emergency services will be as directed by contracting officer. All basic services will continue as directed by the Contracting Officer. During such periods, some or all of the personnel employed under this contract may be identified as emergency essential and may be granted access to the base during increased security measures. During performance of specific services the contractor may be required to work any of the 24 hours of a day due to operational requirements. Any extended special or emergency (e.g. disaster relief operations) after-hours service, which increases the cost of performance above and beyond the negotiated contract price, shall be addressed by the contracting officer before requested services are performed (or at the earliest practicable time, when an immediate response to the situation is required to prevent injury or loss).

4.5. QUALITY CONTROL The contractor shall develop and maintain a quality control program to ensure pest management services are performed in accordance with commonly accepted commercial practices and standards. Quality Control Plan shall be submitted to the Contracting Officer NLT COB of the first day of contract start (otherwise no contract start). The contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. The contractor shall provide a quality control manager who will be independently responsible for planning, organizing, directing, and controlling the Quality Control Plan. The authority delegated to him will be comparable to that of the project manager on all matters pertaining to quality. The Quality Control Manager will also designate the inspection method and frequency for each function and performance standard. The contractor will provide a monthly quality control report indicating all quality control work performed during the month. This monthly report will explain in detail all steps taken from the contractor to improve the quality of the work received from customer complaints during the month. This report will also indicate newer continuous improvement processes the contractor may have developed to better serve MacDill AFB customers.

4.6. QUALITY ASSURANCE. The Government will periodically evaluate the contractor’s performance by appointing a representative(s) (i.e. - COR) to monitor performance to ensure services are received. The Government representative will evaluate the contractor’s performance through intermittent inspections of Government facilities and receipt of complaints from base personnel. The Government may request a quality control inspection at any time based on criteria such as, but not limited to repeated failures during quality control inspections and/or customer complaints. The Government may investigate, at any time, complaints from customers located on the installation. The contractor shall be responsible for initially validating customer complaints and providing a written response to the Government representative within 1 business day. However, the Government representative shall make final determination of the validity of customer complaint(s) in cases involving disputes with customer(s). The contractor will be given four hours after verbal notification to correct the unacceptable performance.

4.7. SECURITY REQUIREMENTS. The MacDill AFB security requirements are listed in Appendix A of this PWS.

4.8. CHANGE REQUIREMENT. Integrated pest management services are subject to change during this contract. The change may be initiated by Civil Engineering (CE) or a designated representative who shall determine increased or decreased pest management service requirements; however, no change shall be made to services without prior approval from the Contracting Officer. Any variation in the PWS shall be furnished by the COR personnel to the Contracting Officer for contract modification.

4.9. UNSCHEDULED INTERRUPTIONS. Pest management operations are subject to unscheduled interruptions and pest management personnel may be required to immediately leave the designated work area or be denied access due to security operations. Mission requirements may necessitate suspension or cancellation of pest management operations for that period. All incomplete work shall be rescheduled for completion within one working day or a time agreed upon with the Pest Control Manager and authorized by COR personnel.

4.10. SPECIAL QUALIFICATIONS. Per DoDI 4150.07 and AFI 32-1053, the contractor will provide contract pest management employees who are certified through the State of Florida in the appropriate pest management categories prior to the start of the contract. The contractor shall not permit contract employees that are not state certified in pest management applications to use or apply pesticides on MacDill AFB. No contract pest management employee will apply pesticides under another person’s or the company’s certification. Evidence of state certification in the appropriate pest management categories for each contract pest management employee shall be submitted to the contracting officer, or his/her designated representative, and shall be maintained current thereafter throughout the period of this contract. All pesticide applicators, including the Certified Pest Control Operator in charge shall provide current documentation of Chapter 482, Florida Statues as a Certified Pest Control Operator in each of the following categories: Termite and Wood Destroying Organisms, Fumigation, Lawn and Ornamental, and General Household and Rodent Control. Additionally, all pesticide applicators, including the Certified Pest Control Operator in charge, shall provide current documentation of Chapter 487 licensing under Public Health/Mosquito Control. Certifications/licenses must be held prior to beginning of employment and must remain in good standing throughout the period of the contract. In addition, all mixing and storage of pesticides shall be performed on the installation. The government will provide facilities for storing and mixing of chemicals. If the contractor spills or releases any hazardous substances (for example, substances listed in the Code of Federal Regulations, Title 40, Part 302), the contractor shall immediately notify the government representative and the base environmental coordinator. The contractor shall provide proof of current State of Florida pest management business license/certification for providing integrated pest management services at MacDill AFB FL, as required in Paragraph 1 of this SOW. All proof of current State of Florida business license/certification shall be made available to the government before the contractor starts work.

4.11. AIR FORCE DOCUMENTS & RECORDS MAINTENANCE. Government contractors who create, receive, and maintain Air Force records, electronic or paper, will meet the requirements of Air Force Manual (AFMAN) 33-363. The contract must include requirements for the delivery of all pertinent documentation of contractor program execution. Training is available for contractor personnel who manage Air Force records. 4.12. WIDE AREA WORK FLOW (WAWF) MONTHLY INVOICE. The contractor shall submit a WAWF monthly invoice by the second working day of the following month.

4.13. GOVERNMENT-FURNISHED FACILITIES: The government will furnish or make available facilities described in Appendix E. Government facilities have been inspected for compliance with the Occupational Safety and Health Act (OSHA). No hazards have been identified for which work-arounds have been established. Should a hazard be subsequently identified, the government will correct OSHA hazards according to government developed and approved plans of abatement taking into account safety and health priorities. A higher priority for correction will not be assigned to the facilities provided hereunder merely because of this contracting initiative. The fact that no such conditions have been identified does not warrant or guarantee that no possible hazard exists, or that work-around procedures will not be necessary or that the facilities as furnished will be adequate to meet the responsibilities of the contractor. Compliance with the OSHA and other applicable laws and regulations for the protection of employees is exclusively the obligation of the contractor. Further, the government will assume no liability or responsibility for the contractor’s compliance or noncompliance with such requirements, with the exception of the aforementioned requirement to make corrections according to approved plans of abatement subject to base-wide priorities. Before any modification of the facilities performed by the contractor at his or her expense, the contractor must furnish the contracting officer documentation describing, in detail, the modification requested. No alterations to the facilities must be made without specific written permission from the contracting officer. In the case of alterations necessary for compliance with OSHA, such permission must not be unreasonably withheld. The contractor must return the facilities to the government in the same condition as received, fair wear and tear and approved modifications excepted. These facilities must only be used in performance of this contract.

4.14. CONSERVATION OF UTILITIES: The contractor must make sure employees practice utilities conservation. The contractor must be responsible for operating under conditions that prevent the waste of utilities. (a) Lights must be used only in areas where work is actually being performed. (b) Employees must not adjust mechanical equipment controls for heating, ventilation, and air conditioning systems. (c) Water faucets or valves must be turned off when not in use.

4.15. KEY CONTROL

The contractor must establish and implement methods of making sure all keys issued to the contractor by the government are not lost or misplaced and are not used by unauthorized persons. The contractor must not duplicate any keys issued by the government.

The contractor must immediately report to the COR or contracting officer any occurrences of lost or duplicated keys.

In the event keys, other than master keys, are lost or duplicated, the contractor may be required, upon written direction of the contracting officer, to rekey or replace the affected lock or locks without cost to the government. The government may, however, at its option, replace the affected lock or locks or perform rekeying and deduct the cost of such from the monthly payment due the contractor. If a master key is lost or duplicated, all locks and keys for that system must be replaced by the government and the total cost deducted from the monthly payment due the contractor.

The contractor must prohibit the use of keys issued by the government by any persons other than the contractor’s employees. Opening of locked areas by contractor employees to permit entrance of persons other than contractor employees engaged in performance of contract work requirements in those areas is prohibited.

4.16. CONTRACTOR REPORTING OF LABOR HOURS

The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for MacDill AFB via a secure data collection site. The contractor is required to completely fill in all required data fields at http://www.ecmra.mil.

Reporting inputs will be for the labor executed during the period of performance for each Government fiscal year (FY), which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October of each calendar year. Contractors may direct questions to the CMRA help desk.

APPENDIX A

MacDill AFB CONTRACTOR SECURITY APPENDIX

ANTI-TERRORISM

The contractor must complete an Installation Records Check without any disqualifying incidents for installation access. All contractor employees will complete Level I AT awareness training via the Contractor Awareness Pamphlet provided by the 6th Contracting Squadron, and annually thereafter if applicable. Contractor may duplicate the pamphlet for dissemination to contractor employees. Lead contractor will maintain a listing of all trained employees for inspection by 6th Contracting Squadron and or 6th Air Refueling Wing Antiterrorism Officer. Contractors will immediately notify base security forces of any suspicious activities. Contractor will initiate actions directed by the installation commander during changes to force protection conditions, increased surveillance, and protection of infrastructure.

INSTALLATION ENTRY CONTROL PROCEDURES FOR ALL CONTRACTORS

The following appendix provides information from Air Force Instruction 31-101 and local supplements on requirements for entering and conducting business while on MacDill Air Force Base (MAFB), Florida.

1. Contract Award.

1.1. Upon award of a contract, all contractors (including Small Purchase contractors), will have background checks conducted on all employees (including subcontractors or temporary employees) requiring access to MAFB.

1.2. The base Point of Contact (POC) is the unit or the base contracting office administratively who services the contract. The authorized format used for submitting employee personal information for background checks is located at Attachment 1, Request for Background Check.

1.3. The contractor is responsible for providing their employees’ personal information to their base POC. Failure to provide all personal information required or providing fraudulent information will result in the employee’s base access being denied.

· Full name to include middle names or any known alias.

· Date of Birth.

· Social Security number.

· Driver’s license number and state of issue.

· Project name and contract number.

· Requestor’s name.

. Each base POC is appointed by their commander or management director on DD Form 577, Appointment/Termination Record – Authorized Signature, and forwards a signed copy to the Visitor’s Reception Facility (VRF) annually.

1.5. The base POC sends a request, via email, to the 6 SFS/Background Check email address, which can be located on the Global Listing (MacDill). Personal employee information will be included on Attachment 1, Request for Background Check/Entry Access List (EAL).

1.6. The 6th Security Forces Squadron (6 SFS) will, upon receiving the EAL from the base POC, conduct a background check of contractor personnel using the approved local, state, and federal government web sites.

1.7. Any contractor employee found to have a criminal conviction listed below or have an outstanding warrant(s) will not be allowed entry to the base.

· US Citizenship, immigration status, or Social Security Account Number that cannot be verified.

· Barred from entry/access to any military installation or facility.

· Wanted by federal or civil law enforcement authorities, regardless of offense/violation.

· Name appears on any federal agency’s "watch list or “hit list” for criminal behavior or terrorist activity.

· Any conviction for firearms or explosive violations within the last three years.

· Incarcerated for 12 months or longer within the past three years, regardless of the offense.

· Any conviction of espionage, sabotage, treason, terrorism, murder.

· Conviction of a sexual assault, armed assault/robbery, rape, child molestation or kidnapping.

· Drug possession with intent to sell or drug distribution.

1.8. Upon completion of the background check, the results will be electronically mailed to the requesting base POC. The email response will state whether the applicant is approved, denied, or pending further review by 6 SFS/S5 and base legal. Contract employee(s) approved for base access will report to the VRF, Building 1089 to retrieve the AF Form 75, Visitor Pass.

1.9. Background checks are valid for two years unless the person is terminated from the job, or is involved in a serious offense as listed paragraph 1.7. Requests for base access exceeding the two year period will require another background check.

1.10. The base POC identifies and appoints responsible contractor supervisors to sponsor sub-contractors and contract workers on base to perform services for no more than five business days (short-term) without a completed background check. Subsequent sponsorship (long-term) will require the proper completed background check by the VRF.

1.11. The base POC is responsible for identifying and retrieving all AF Form 75’s from the contract employees once the contract has expired or the contract employee’s employment is terminated and returns the passes to the VRF.

2. Contractor Visitor Passes for Entry to MAFB

2.1. The base POC completes Attachment 2, Request for Issuance of AF Form 75, and electronically send the request to the VRF for issuance of the AF Form 75 for the duration of the contract, not to exceed one year.

2.2. Contractors must possess the proper photo identification media (driver’s license/state identification card, military identification card, or other authorized U.S. governmental photo media) to be issued a base pass. Non-U.S. citizens must provide original Immigrations & Naturalization Service photo media and Social Security card.

3. Contractor Vehicle Passes for Entry to MAFB.

3.1. Operators of vehicles must provide the following documentation to register vehicles.

· Valid driver’s license.

· Valid vehicle registration or rental agreement.

· Valid insurance (except fleet vehicles) or rental agreement.
