Introduction The Industrial Computed Tomography 1500 (ICT 1500) and the Industrial Computed Tomography 2500 (ICT 2500) and associated subcomponents, located in Buildings 2113 and 984 respectively, Hill Air Force Base (HAFB), UT, are sophisticated and proprietary Computed Tomography (CT) inspection systems. These systems use computed tomography to inspect solid rocket motors internally for anomalies in a non-destructive manner and are vital to the inspection and sustainment of the Minuteman III (MMIII) Intercontinental Ballistic Missile (ICBM) weapon system.
SCOPE
The 709th Maintenance Support Squadron has a requirement to maintain and repair the ICT 1500 and 2500 CT systems through 2030 to support sustainment of OO-ALC supported weapon systems. Under this effort, the contractor shall provide engineering, troubleshooting, calibration, repair, maintenance, and parts for the ICT 1500 and 2500.
The requirement consists of over-the-phone technical support, on-site preventative maintenance, and unscheduled on-site repair for the ICT 1500 and 2500 CT systems. The contractor shall provide trained personnel, furnish all labor, tools, parts, materials, transportation, and lodging necessary to perform full-service maintenance, repair, and calibration of the ICT 1500 and 2500 CT systems.
The contractor shall provide training as needed for government personnel on the ICT 1500 and 2500 systems in accordance with (IAW) the (CDRL A001-Training-DI-PSSS-81523D).
The contractor shall provide familiarization as needed to government personnel as system changes and/or updates dictate.
All maintenance of the ICT 1500 and 2500 CT systems shall be performed in accordance with OEM COTS manuals and applicable Airforce guidance or standard industry best practices. The government is in possession of the existing operation and maintenance manuals for the various systems and sub-system components and will make these available to the Contractor upon contract award.
The contractor shall be competent in working with non-standard equipment configurations where original equipment manufacturer (OEM) parts and equipment are unavailable.
The contractor shall provide for management and control on all matters related to this PWS from conception to final completion.
Seven (7) days after contract award a single point of contact shall be designated by the contractor and shall provide oversight and direction to contract personnel.
Contract support shall cover all necessary operational support and ancillary functions that the contractor shall provide in connection with the servicing, repair, engineering, and technical support for overall assistance to the user.
Basic SERVICES The Contractor shall provide scheduled preventative maintenance to include configuration management, on-call technical support, and on-site un-scheduled repairs. The contractor shall correct all defects, provide all parts and labor to do so, perform system calibration, and provide training and familiarization to government personnel.
The contractor shall coordinate all site visits, communications or any other actions performed under the execution of this PWS with the 709 MXSS/MXDTAD.
PREVENTIVE MAINTENANCE
The contractor shall provide on-site preventative maintenance as required in the subsection below. The Contractor shall provide an Annual Maintenance Service Report (CDRL A002-DI-MGMT-80995A) within thirty (30) calendar days of contract award. This schedule shall accommodate the required maintenance intervals of each subsystem in the following subsection. This schedule shall be provided to and approved by the 709 MXSS/MXDTAD shop supervisor and provided to the Contracting Officer Representative (COR).
Preventive maintenance shall include any software updates.
Preventive Maintenance Preventative maintenance shall be accomplished at the specified intervals by the contractor/subcontractor on the following equipment and components:
Linatron (including Linatron chill and pump stand)-shall be serviced and inspected twice per year.
Gantry shall be serviced and inspected once per year.
CT Acquisition System (including backup hard drives) shall be inspected and maintained three times per year.
Configuration Management The contractor shall maintain configuration control of the ICT 1500 and 2500 CT systems and provide backups of the technical data package whenever software updates are accomplished, and specifications as required in the Configuration Management Data (CDRL-A003-DI-SESS-80766B).
The contractor shall be required to provide management of maintenance services, labor, equipment repairs, engineering support, spares, and replacement equipment for obsolete instruments with applicable hardware and software upgrades.
The contractor shall monitor and track ICT 1500 and 2500 CT system equipment obsolescence and provide, at a minimum, annual recommendations to the government for CT equipment and associated hardware and software upgrades to mitigate equipment obsolescence and negative performance trends. These recommendations shall be documented as part of the Annual Maintenance Service Report (CDRL A002- DI-MGMT-80995A) to be delivered IAW paragraph 3.1.
Defects The Contractor shall report all defects found during preventative maintenance to the 709 MXSS/MXDTAD shop supervisor. In the event that defects are found, the Contractor shall coordinate all findings and discrepancies with the 709 MXSS/MXDTAD shop supervisor and include the findings in a Condition Found Report (CDRL A004-DI-MGMT-81648A-Condition Found Report) IAW paragraph 3.6.1.
On-Call Technical Support The contractor shall provide telephone support to government personnel to assist with troubleshooting and maintenance functions.
During the hours 0600 to 1530 Mountain Standard Time (MST) the contractor shall respond within one (1) business day to the 709 MXSS/MXDTAD shop supervisor, or COR.
The contractor shall provide the government with a phone number and an e-mail address for service calls.
The contractor shall respond only to service calls and emails from the 709 MXSS/MXDTAD shop supervisor, or COR.
Unscheduled On-Site Repair Contractor shall provide on-site repair of the ICT 2500 and 1500 systems and their associated components.
Requests for repairs that must be accomplished immediately will be deemed as critical by the Government to keep the ICT 2500 or 1500 CT system from imminent failure, or in order to restore the system to operational status in the event of a failure.
The Contractor shall respond to requests for critical repairs by telephone within one (1) business day of request and be on site to commence repairs within five (5) business days of notification. These repairs may include mechanical systems, computer software or hardware issues, repairing, replacing, or rebuilding parts, assemblies, subassemblies, and components plus testing.
Request for repairs that are needed to keep the system functioning properly and may include system disassembly, repairing, replacing, or rebuilding parts, assemblies, subassemblies, components, and end items, but do not result in failure or imminent failure of the ICT 2500 or 1500 will be deemed non-critical.
The Contractor shall respond to requests for non-critical repairs by telephone within one (1) business day of request and be on site and commence repairs within ten (10) business days of request. These types of repairs may also include periodic verification of system software and replacement of corrupted files.
Determination of a critical or non-critical repair will be at the discretion of the 709 MXSS/MXDTAD shop supervisor.
The Contractor shall continue repairs to completion as approved by the 709 MXSS/MXDTAD shop supervisor.
The Contractor shall coordinate all findings and discrepancies with the 709MXSS NDI shop supervisor and provide a Condition Found Report (CDRL A004-DI-MGMT-81648A-Conditon Found Report) IAW paragraph 3.6.1.
For maintenance actions with long lead times or where parts availability is an issue, the contractor shall provide a repair schedule (CDRL A005-DI-PSSS-82201-Long Lead Times Material Report) within five (5) business day of the determination of the lead time to be agreed to by the 709 MXSS/MXDTAD shop along with the Condition Found Report.
On-Site Reporting For any scheduled maintenance, calibration, alignment, or repair, the Contractor shall provide an On-site Reporting Log (CDRL A006 DI-MGMT-81605A-Briefing Data) to the 709 MXSS/MXDTAD supervisor(s).
Completion Upon completion of any preventative scheduled or un-scheduled maintenance, the contractor shall conduct a technical evaluation and a system calibration along with a representative from the 709 MXSS/MXDTAD shop.
Technical Evaluation Report Upon completion of any site-visit for preventative or un-scheduled maintenance (either critical or non-critical), the contractor shall provide a Maintenance Service Report (CDRL A002- DI-MGMT-80995A-Service Report) to the 709 MXSS/MXDTAD shop supervisor, and the COR within five (5) business days of the contractor’s departure from the installation.
The maintenance service report shall include a detailed description of any maintenance actions performed during the scheduled or un-scheduled site visit and, if defects were discovered, documentation of the problem, type of repair required, and an estimate of the time and cost required. It shall also include a list of anticipated repairs for the foreseeable future and a suggested course of action.
No defect shall be repaired without prior written approval by the 709 MXSS/MXDTAD shop supervisor and contracting.
Upon completion of the demonstration, personnel from 709 MXSS/MXDTAD will perform an operational test on the ICT 1500 and/or 2500 CT system to determine if all deficiencies have been corrected.
The final inspection of the ICT 1500 and/or 2500 will be accomplished by the 709 MXSS/MXDTAD supervisor, the ICBM Propulsion Engineering CT System engineer or designated Government representative prior to contractor departure.
System Calibration Upon completion of any maintenance or repair actions, system calibration shall be performed and demonstrated to ensure that original ICT 1500 or 2500 system performance specifications have been met.
Calibration shall be accomplished by the appropriate contractor/subcontractor by ensuring performance requirements for image quality are met as described in ASTM E1695, modular transfer function (MTF) and contrast discrimination function (CDF) measurements.
The contractor shall provide the 709 MXSS/MXDTAD supervisor and the COR a Certification of Calibration (CDRL A007-DI-QCIC-80798C-Calibration Certificate/Report) to document that all system and subsystem components are within operational parameters prior to leaving the installation.
Parts and Parts Availability When a component repair is not possible, the contractor shall provide the sufficient replacement component(s). Equipment repairs that cannot be made on-site will be performed at the contractor’s facility or repair facility they designate.
The Contractor shall use OEM parts if available or parts of equal quality. Due to the diminished availability of OEM or equivalent parts, rebuilt or refurbished parts will be deemed acceptable. The replacement of worn or defective parts shall be consistent with the OEM’s design of the equipment if parameters are available or IAW industry standards.
The equipment shall meet all form, fit, and function requirements as necessary to meet the defined operational requirements of the user manuals for operation, repair, and for calibration.
If spare parts are readily available, the contractor shall replace them within five (5) business days. Spare parts shall consist of available items in government furnished inventory, which are required to perform maintenance actions.
Upon government approval the contractor shall purchase spare assets and store the spares in government furnished equipment inventory located at HAFB.
Replacement equipment for obsolete instruments shall consist of new configuration items with the applicable software replacing ICT 1500 or 2500 CT system components no longer procurable or supportable.
For long lead time items where parts availability is an issue, the contractor shall also provide a repair schedule (CDRL A005-DI-PSSS-82201-Long Lead Times Material Report), within five (5) business day of the determination of the lead time. The contractor shall procure items on a schedule agreed to by the 709 MXSS/MXDTAD shop supervisor as applicable.
All products developed under this contract shall be considered Government property and shall have no license encumbrances. All data shall be developed and provided in accordance with the data rights clauses in the contract and as identified herein. Unless the contractor has expressed a claim to providing data to the Government with less than unlimited data rights, the Government will assume unlimited data rights.
The contractor shall not disclose or disseminate any data created for, or provided from, the Government to any entities without approval from the 709 MXSS/MXDTAD shop supervisor.
Training The contractor shall provide one (1) annual training session to keep government personnel current on the ICT 1500 and 2500 CT systems and associated equipment. This training session shall be exercised as an option at the discretion of the government. The contractor shall provide training plan to the government within thirty (30) days of option award. A certification of training completion shall be provided upon completion prior to contractor’s departure from the installation IAW the (CDRL A001- Training-DI-PSSS-81523D).
If the option is exercised by the government, the contractor shall provide one (1) training class per year which shall accommodate up to ten (10) individuals for a duration of up to eight (8) hours. Training classes will be scheduled, in advance, by the 709 MXSS/MXDTAD.
Annual training shall include hands-on instruction, and training materials IAW the training plan (CDRL A001-Training-DI-PSSS-81523D). Course content shall cover the following topics:
1. Introduction to computed tomography
1. Radiation safety – general radiation safety as well as ICT 1500 and 2500 CT system specific safety training
1. CT image quality – how to read images, and how to identify acceptable/unacceptable images.
ICT 1500 and 2500 CT system operation – overview of how the systems work, how to use the software, and troubleshoot within the scope of the user.
Familiarization In addition to annual training, the contractor shall provide familiarization to up to four (4) required individuals as directed by the 709 MXSS/MXDTAD.
Familiarization shall fulfill the purpose of keeping government personnel current on the operation of the ICT 1500 and/or 2500 CT systems as system operational parameters change due to hardware or software updates or any other changes that affect the operation of the ICT or associated equipment.
Familiarization shall take place during preventative maintenance visits and/or un-scheduled repair visits as needed and coordinated by the 709 MXSS/MXDTAD.

CDRL SUMMARY
Contract information data shall be delivered electronically in accordance to the corresponding requirement for each Data Item listed in Table 5-1 (below). The contractor shall have no more than one error per quarter for all CDRL submittals. The contractor shall submit CDRLs according to the due date indicated and have no more than one CDRL per quarter submitted after the identified due date. Grammatical errors do not count as an error. Substantive mistakes shall be corrected within two working days and corrective action taken to prevent recurrence of inaccurate late CDRLs.

ADDITIONAL REQUIREMENTS
Government Provided Support The Government will provide necessary working space, including heat, light, ventilation, and electrical powered outlets for the use of contractors’ maintenance personnel. The Government shall provide access to the necessary facilities where the equipment resides. The place of performance shall be Hill Air Force Base (AFB), UT.
Government telephones shall be used for official business only.
Quality Control Plan The contractor shall establish a complete Quality Control Plan (QCP) to ensure the requirements of this contract are provided as specified. The contractor's quality management program shall comply with the minimum quality management systems elements specified in the American National Standards Institute (ANSI), American Standards Organization (ASO), and International Standard Organization (ISO) family of standards. The report shall be submitted to the 309 Maintenance Acquisition Support Office (MASO), NLT 30 days post contract award. (CDRL A008-DI-QCIC-8179A-QAPP).
Quality Assurance The 309 MASO COR will perform assessments of the contractor’s performance. Subsequent to contract award, specific name and contact information of the 309 MASO COR with a letter defining duties and authority will be furnished to the contractor.
The 309 MASO COR will inform the task manager in person when discrepancies occur and will request corrective action. The 309 MASO COR will make a notation of the discrepancy on the assessment checklist with the date and time the discrepancy was noted and will request the task manager (or authorized representative) to initial the entry on the checklist.
The government may inspect each task as completed or increase the number of quality control inspections if deemed appropriate because of repeated failures discovered during quality control inspections or because of repeated customer complaints.
Any matter concerning a change to the scope, prices, terms or conditions of this contract shall be referred to the Contracting Officer (CO).
SECURITY:
Security Review Date: 20240423 Reviewed By: Stiefken Security Review Control Number: 20240423CS Conflicts of Interest The contractor shall not assign to contract or task order performance any person, including any employee of the U.S. Government, if employing that person would create a conflict of interest under any law, regulation or policy of the United States Government. Additionally, the contractor shall not assign to contract or task order performance, any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives approval according to DoD Directive 5500.7-R, Standards of Conduct, and any other applicable regulation or policy of the Department of the Air Force or its subordinate commands, as applicable, including but not limited to Air Force Materiel Command.
Contractors will not use any other form of identification to access any government installation or facility while working on a government contract except the issued credentials associated with the awarded contract or task order. Example: using a military retiree card to access an installation to work on a contract.
Security Requirements Foreign nationals (FN) will not be escorted, or sponsored, onto the installation without being properly vetted. Any request must follow 309 MXSG OI 16-102, Security Requirements section 7. FNs must be approved for access via the Foreign Disclosure Office and Air Force Office of Special Investigations (AFOSI).
Contractor will schedule all visits and coordinate Base entry by contacting 709 MXSS/NDI shop supervisor. The contractor shall be escorted at all times.
The contractor shall comply with all security, force protection, operation security and antiterrorism requirements at each installation.
The contractor shall comply with 309 MXSG in and out-processing procedures for all contractor personnel to ensure required security-related briefings and debriefings are accomplished. Specific in and out-processing checklists may be included at the task order level.
The contractor will use the COR. as the primary means of reviewing and approving forms and for communicating with necessary government representatives.
Communications Security All communications with DoD organizations are subject to Communications Security (COMSEC) review.
The DoD authorizes the military departments to conduct COMSEC monitoring and recording of telephone calls originating from, or terminating at, DoD organizations.
The Government advises the contractor that at any time they place a call to, or receive a call from, a DoD organization, they are subject to COMSEC procedures.
The contractor assumes the responsibility for ensuring wide and frequent dissemination of the above information to all employees dealing with official DoD information.
Operations Security (OPSEC) Requirements During the course of this contract, in addition to those restrictions, instructions and guidelines delineated in PWS and/or other references provided, the contractor will adhere to the following minimum requirements.
Introduction of personnel electronic devices into government spaces, laptops, tablet Personal Computers (PC)s, cellular phones, cameras, recording devices, and data recording/storage devices is strictly controlled and forbidden in most cases. Company issued equipment required for the performance of work must be approved by the 309 MXSG security manager (SM). Photography and recording is not allowed in restricted areas and controlled areas except for official use and by approved photography letter only. NOTE: Unless otherwise stipulated in the contract, contact the Installation Public Affairs or SM for approval.
Contractor personnel shall not discuss government operations in public or over unprotected or unencrypted communications. Official Business controlled unclassified information (CUI) may only be transmitted as directed in the PWS as it adheres to CUI Guidelines.
The Contractor shall not post to company websites, publications, newsletters, or other media any images, data or information that reveal sensitive government operations, personnel, equipment, and/or classified or CUI. When in doubt, company press releases, related to this contract, will be coordinated through the Public Affairs Office prior to release.
Contractor personnel shall not disclose to unauthorized third parties, post to unofficial sites (including all Social Networking sites) any images, data or information that reveals sensitive government operations, personnel, equipment, including, but not limited to: Tactics, techniques and procedures, production or work schedules, any visible or concealed modifications, upgrades, additions to Air Force installations, property or weapons or equipment; increases, change, or decreases in work/deployment frequency or government personnel, project schedules, logistics or material delivery schedules; specialized equipment orders, deliveries, shipments, etc. (Unauthorized disclosure and transfer of National Security Information is punishable under 18 USC § 793.); Unauthorized disclosures and attempts to solicit this type of information by unauthorized third parties or others not affiliated with this contract shall be reported to the SM and/or installation SFS, COR, and, if applicable, the company FSO and/or the Defense Security Service.
Practice OPSEC and implement countermeasures to protect Critical Information (CI) and other sensitive unclassified information and activities vigilance or degrade the planning and execution of military operations performed or supported by the contractor in support of the mission. Protection of CI will include the adherence to and execution of countermeasures which the contractor initiates or as provided by OPSEC manager or installation OPSEC Program Manager, for CI on or related to the PWS.
The contractor must mark and protect any information IAW CUI, to include E-mail, and adhere to all CUI standards and policies.
Contractors who have access to the Ogden Air Logistics Complex (OO-ALC) and Government Servicing Units (GSU)s facilities or critical information shall have OPSEC training IAW AFI 10-701, Operations Security (OPSEC). Initial training will be approximately one (1) hour and follow-on recurring annual training will be approximately 1/2 (0.5) hour. OPSEC training will be conducted within 30 days of assignment.
Contractor Identification & Badges All contractor management, staff and contractor personnel shall be clearly identified as such at ALL times. Contractor identification shall include visible badges, conversations, mail, email, teleconferences, video teleconferences, faxes, and/or other electronic communication whether with government personnel, other contractor personnel, or with the public when supporting this contract where their status as contractor employees might not otherwise be apparent or where they might be mistaken for civil service employees. At a minimum, contractor management staff or contractor personnel shall clearly identify themselves using the label “contractor” in email addresses in accordance with Federal Acquisition Regulation (FAR) 37.114(c), Special Acquisition Requirements; opening ALL phone conversations with a statement materially the same as, “[office name], I am [employee name], a [prime contractor’s company name] employee,” including the employing contractor’s name in the letterhead and/or signature block of any written correspondence; and any other means necessary.
In/Out-Processing All contractors are required to in/out-process from each respective organization regardless of if they are issued a DBIDS or CAC.
Security Incident Should a security incident occur, the contractor shall: immediately report to the site location 309 MXSG Security Manager (SM) all available facts related to each instance, take such precautions as the SM may reasonably require for security purposes, take reasonable and prudent action to establish control of the scene; prevent further violation, to preserve evidence until released by proper authority; and cooperate fully and assist Government personnel as the Government conducts an investigation of the violation. NOTE: Participation shall last until the investigation is complete.
Training Requirements The 309 MXSG (SM) and/or COR will administer and maintain all individual training and records, for compliance purposes, associated with this PWS. Training records must be made available during self-inspections and other oversight activities.
GENERAL INFORMATION
Hours of Operation Normal Hours of Operation: The contractor shall perform the services required under this contract during the following hours: Monday through Friday 0530-1600 – EXCEPT FEDERAL HOLIDAYS and other base closures.
Federal Holidays: Unless otherwise required by the Government, the contractor shall not have access to the Government facilities on the following federal holidays: New Year’s Day, Martin Luther King’s Birthday, Washington’s Birthday, Memorial Day, Independence Day, Juneteenth, Labor Day, Columbus Day, Veterans’ Day, Thanksgiving Day, and Christmas Day. If a holiday falls on Saturday, it is observed on Friday and working hours are Monday through Wednesday for that week. If the holiday falls on a Sunday, it is observed on Monday. Any other circumstances that may prompt base closure, such as Days of National Mourning, may also restrict opportunity to perform the exchanges. In the event of a base closure, exchanges shall be performed on the next business day. There may also be up to six AFMC Goal Days and four Wingman Days per year awarded to Government civilian employees, as well as possible weather related days and Government directed holidays. On these days, the contractor shall not generally be required to work and the Government shall not be billed for work not performed.
NOTE: Good Friday is regarded as a holiday by the Utah Wage Determination; however, Good Friday is not a Federal Holiday so the Government facilities will be open for work that day. The contractor shall plan on employees working business as usual on that day, unless the Government supervisor or section chief determines that such services are either unnecessary or will be less than a full normal workday.
Down Days/Weather Days: The Government will notify the contractor when official notice has been given for Energy/Weather Down days. The contractor is not expected to work on any Energy/Weather Down days.
Rescheduling: Down Days/Weather days may be rescheduled if conditions/emergency operations require.
Conservation of Utilities The contractor shall instruct their employees in utilities conservation practices. The contractor shall be responsible for operating under conditions which prevent the waste of utilities.
ENVIRONMENTAL CONTROLS
Hazardous Work Environment The Contractor shall ensure their employees are made aware that the performance of these services will occur in industrial areas. The industrial complex at Hill AFB has the potential to expose workers to hazardous materials which may include, but is not limited to: hexavalent, chrome, cadmium, beryllium, lead, and others. While the Contractor shall not perform duties in a regulated area (where exposure are expected to exceed the permissible exposure limits set by 29 CFR1910.10xx), there may be contact hazards with these materials. The contractor shall ensure proper protective measures and training is taken to ensure contracted employees are protected from hazards. The 75th Air Base Wing (ABW) Bio-Environmental Engineering Flight produces BIO Survey of these areas which will be made available upon award.
Compliance with Laws and Regulations. The contractor shall be knowledgeable of and comply with all applicable Interstate, Federal, State, and Local laws, regulations, and requirements regarding environmental protection. In the event environmental laws/regulations change during the term of this contract, the contractor is required to comply as such laws come into effect.
Notification of Environmental Spills If the contractor spills or releases any substance contained in 40 CFR 302 into the environment, the contractor or its agent shall immediately report the incident to the 709 MXSS/NDI shop supervisor. The liability for the spill or release of such substances rests solely with the contractor and its agent.
Safety Program The contractor shall implement a safety program that ensures protection of Government personnel and property. (See Appendix B) In the event of an accident/mishap, (CDRL A010-DI-SAFT-81563-Accident/Incident Report) the contractor shall take reasonable and prudent action to establish control of the accident/mishap scene, prevent further damage to persons or property, and preserve evidence. An email copy of the mishap / incident notification shall be sent within three (3) calendar days to the 709 MXSS/NDI shop supervisor, who will forward it to OO-ALC safety.
GOVERNMENT OBSERVATIONS
During the period that the contractor is working on equipment, government personnel, other than CO, may, from time-to-time, observe contractor operations. However, these personnel may not interfere with contractor performance.
FOREIGN OBJECT DAMAGE (FOD) AND TOOL CONTROL
The contractor shall follow the applicable guidance in chapters/paragraphs/sections of AFI 21-101 and HQ AFMC, Hill AFB, and 309 MXW supplements to the AFI 21-101 concerning base tool (chapter 10 section B), and FOD control policies (chapter 14, and sub paragraphs designated as 41), daily. Cloth rags are considered tools and will follow the same regulations and procedures as tools.
If tools, and/or equipment are lost, the contractor will report the loss to the GM/COR no later than two hours after discovery. If tools are lost or not accounted for, then the contractor shall implement the lost tool procedures of AFMC supplement to AFI 21-101, chapter 10, section B.
Contractors performing functions in industrial areas on the center shall have a method for controlling and accounting for all tools used. All contractors working on the center shall inventory their tools at the beginning and end of each shift and at the end of each task to check for any lost or missing tools.
ENVIRONMENTAL:
Weather Hazards. Workers may encounter hazardous weather conditions including snow, heavy rain, lightning, extreme dust, and other conditions that warrant appropriate precautions.

A. Safety Program Requirements The contractor shall implement a safety program plan that ensures protection of Government personnel and property. (CDRL A009-DI-SAFT-82313). The program shall consist of, as a minimum:
1. Mishap reporting, as defined in paragraph B1 below.
2. A Safety Plan that addresses, as a minimum, the subjects listed in Section II –Specific Requirements, and shall be used during the performance of the work described in the contract. The Safety Plan shall be approved by OO-ALC/SE (Safety Office) prior to commencement of any work described in this contract.
3. Routine and recurring surveillance to ensure the safety requirements of this contract are enforced.
4. Competent personnel to provide surveillance of the Safety Plan.
5. Identification of segregated work site locations for operations that cannot be co-mingled with general industrial operations and the process for ACO approval of operations and changes at these specific sites.
6. All contractor personnel shall be trained and qualified to perform their duties safely.
7. The contractor shall include a clause in all subcontracts requiring the subcontractors to comply with the safety provisions of this contract, as applicable.
B. Mishap Notification
1. The contractor shall notify OO-ALC/SE Workflow (OO-ALC.SE.Workflow@us.af.mil) or the Hill AFB Command Post (801-777-3007) after normal duty hours, and the designated Government Representative (GR), i.e., the ACO, PCO, or DCMA QAR (Quality Assurance Representative) within one (1) hour of all mishaps or incidents at or exceeding $2,000 (material + labor) in damage to DOD property entrusted by this contract, even if the government is wholly or partially reimbursed. This notification requirement shall also include physiological mishaps/incidents. A written or email copy of ALL the mishap/incident notification shall be sent within three (3) calendar days (CDRL A010-DI-SAFT-81563) to the GR, who will forward it to OO-ALC/SE. For information not available at the time of initial notification, the contractor shall provide the remaining information no later than 20 calendar days after the mishap, unless extended by the ACO.
Mishap notifications shall contain, as a minimum, the following information:
(a) Contract, Contract Number, Name and Title of Person(s) Reporting
(b) Date, Time and exact location of accident/incident
(c) Brief Narrative of accident/incident (Events leading to accident/incident)
(d) Cause of accident/incident, if known
(e) Estimated cost of accident/incident (material and labor to repair/replace)
(f) Nomenclature of equipment and personnel involved in accident/incident
(g) Corrective actions (taken or proposed)
(h) Other pertinent information
2. The contractor shall cooperate with any and all government mishap investigations. Additionally if requested by government personnel or designated government representative (GR), i.e., the ACO, PCO, or DCMA QAR (Quality Assurance Representative), the contractor shall immediately secure the mishap scene/damaged property and impound pertinent maintenance and training records, until released by safety investigators.
3. The contractor shall provide copies of contractor data related to mishaps, such as contractor analyses, test reports, summaries of investigations, etc. as necessary to support the government investigation.
4. The contractor shall support and comply with the safety investigation and reporting requirements of DAFI 91-204, Chapters 1 – 5.
C. General Safety Requirements:
| If the safety plan is modified, the contractor shall submit the proposed modification, in writing, to the Contract Administration Office safety representative. |
| The contractor is solely responsible for compliance with all federal, state and local laws, the Occupational, Safety and Health Act (OSHA) (Public Law 91-596) and the resulting standards, OSHA Standards 29 CFR 1910 and 1926, as applicable, Air Force Occupational Safety and Health Standards (AFOSH STD), to include DAFMAN 91-203, Air Force Occupational Safety, Fire, and Health Standards, DAFI 91-204, Safety Investigations and Reports, Contract Safety, and the protection of their employees. Additionally, the contractor is responsible for the safety and health of all subcontractor employees. |
| The contractor shall ensure assigned personnel are adequately trained and qualified for the task being performed. Brief all personnel on the hazards involved with operations and applicable precautions to be taken. Should unidentified hazards arise, cease operations until actions are taken to eliminate or mitigate hazards to safe levels. |
| Compliance with OSHA |
and other applicable laws and regulations for the protection of contractor employees is exclusively the obligation of the contractor. Note: AFOSH STDs are annotated because many of the Air Force Standards exceed the OSHA standard criteria. If a conflict is noted, the most stringent requirement takes precedence. The government shall assume no liability or responsibility for the contractor's compliance or non-compliance with such requirements. The contractor shall furnish to each of his/her employees a place of employment, which is free from recognized hazards. The contractor shall brief his/her employees on the safety requirements of this contract and on hazards associated with prescribed tasks. The contractor is responsible for compliance with OSHA Public Law and the resultant standards identified within. In addition, the contractor is required to flow down the safety requirements/specification to all subcontractors. This applies to Federal Acquisition Regulation (FAR) 12 commercial acquisitions as well. This contract shall in no way require persons to work in surroundings or under working conditions which h are unsafe or dangerous to their health. The contractor must coordinate and perform work so as not to impact the safety of government employees or cause damage to government property. This requires providing personnel with protective equipment and associated safety equipment as may be necessary. The contractor must also protect personnel from hazards generated by the work. If the contractor employs BILINGUAL speaking employees, they must post bilingual signs and have written procedures for specific tasks in applicable languages.
SECTION II – SPECIFIC REQUIREMENTS
The contractor’s prepared Safety Plan shall:
· Provide contact information for on-site personnel
· Include a work hazard analysis of the worksite and operations to be performed to include baseline hazard identification and required control measures
· Identify employee safety and health training requirements and the documentation process
The contractor shall ensure that each element identified below is adequately addressed in detail in the safety and health plan:
PEDESTRIAN CROSSWALKS: All contractor personnel are required to use the closest crosswalk, or traffic controlled intersection when crossing the road. Pedestrians must look both ways to ensure the coast is clear before stepping out into the crosswalk. Pedestrians DO NOT have the right of way unless they are already in the crosswalk. Contractor vehicle operators have the same responsibilities as pedestrians, to share the road and mutually observe and yield to pedestrians.
MOTOR VEHICLES: Contractor shall comply with the standards in: DoD Directive 5525.4, Enforcement of State Traffic Laws on DoD Installations", Nov 2, 1981, Para 3-4; DODI 6055.4, DoD Traffic Safety Program, 20 Jul 99; AFI 91-207, USAF Traffic Safety Program, 22 May 07; Each applies to all persons at any time on an Air Force Installation and includes all leased, owned, or privatized property including housing areas. In addition: AFI 13-213, Airfield Management, 29 Jan 08, Paragraphs 1.3.6 and 4.4.2.1 applies to all contractors, sub-contractors, vendors, commercial delivery companies, and all other private business vehicles who operate anywhere on Hill Air Force Base, including the airfield (to include the industrial areas and any buildings or hangars located upon the airfield) in support of their mission.
HOUSEKEEPING: Housekeeping shall be conducted according to the requirements in OSHA Standard 29 CFR 1910.141. CLEAN AS YOU GO will be enforced.
