PERFORMANCE WORK STATEMENT

FOR

FURNITURE MOVES, REPAIRS, AND INSTALLATIONS

I. DESCRIPTION OF SERVICES

The contractor shall provide laborers and foreman as defined in this Performance Work Statement (PWS), except as specified herein as government furnished property or services, to provide rearrangement, repair and installation of office furniture from various manufacturers to include mobile type systems, located at the 60th Medical Group, David Grant Medical Center, Travis Air Force Base, and California. To perform the following tasks the contractor must have installation, rearrangement, and repair experience with various brands of furniture currently owned by the 60th Medical Group. We ask for that because we reuse a lot of furniture which has no instructions for someone to follow to disassemble and or reassemble. Some brands currently in use are Indiana Desk, Work stream Hamilton Sorter, Great Openings, Russ Bassett, Herman Miller, Affordable Interior Systems, and TAB Products. To complete furniture installation, rearrangement, and repair contractor laborers and foreman tasks include:

The rearrangement, and repair of various brands of furniture currently owned by the 60th Medical Group.

Inventory all furniture parts provided by the government per rearrangement, repair and installation requirement to determine if the government needs to purchase additional parts.

Installing furniture safely in accordance with the manufacturer installation guide as to not void the warranty.

Contractor shall arrange the date and time of furniture rearrangement, repair, and installation with the Medical Equipment Management Office (MEMO) within 48 hours upon request from MEMO staff. The contractor will be required to bring all necessary tools, equipment, and transportation to complete the above tasks. When placing Blanker Purchase Agreement (BPA) calls the MEMO staff will tell the vendor which of the above tasks needs to be completed. The vendor will then provide a quote based on the tasks and the hours that it will take to be completed. All routine furniture rearrangement, repair and installation services shall be accomplished during normal duty hours 7:30 AM to 4:30 PM, Monday through Friday, (excluding Federal Holidays).

II. GENERAL INFORMATION

1. Initial Inspection. All furniture rearrangement, repair, and installation of equipment will be inspected and approved by MEMO prior to be installed and after installation is complete to ensure use compliance with safety guidelines in the Medical Treatment Facility. Contact a MEMO Representative to schedule inspection.

2. Removal of Furniture. Contractor shall deliver any furniture that is removed or not utilized to the Medical Logistics warehouse in coordination with the MEMO staff.

III. PERIOD OF PERFORMANCE

1. BPA will be required for 5 years.
