PERFORMANCE WORK STATEMENT
Pest & Rodent Control Services
1. The contractor shall provide pest control services monthly for the control of mosquitoes, cockroaches, gnats, ants, fleas, flies, bees, mice, rodents, and termites.
2. The contractor shall be licensed by the Guam Environmental Protection Agency (GEPA) to provide pest control services in the categories specified in this contract.
3. All pesticides used by the contractor shall be registered with the GEPA for the use intended. In order for any individual to purchase or use "Restricted Use" pesticides in any manner, he or she is required to obtain certification for each pesticide. Labels and Material Safety Data Sheets (MSDS) shall be for each pesticide intended to be used. Any proposed changes in approved pesticide usage shall be submitted for the Contracting Officer's approval at least five (5) working days in advance of the anticipated need.
4. Structural Pest Control
a. Structural Pest Control Services. The contractor shall perform an IN-DEPTH inspection of Building #50, U.S. Naval Hospital, Guam as specified below:
(1) Dry Wood Termite Control. Provide Dry Wood Termite Control in Accordance with GEPA Guidelines.
(a) All INTERIOR baiting will be above ground - NO DRILLING.
(b) The contractor shall take extreme caution when installing and/or performing dry wood termite control to avoid exposed baiting and unsafe locations of baiting.
(2) Subterranean Termite Control. Provide IN-GROUND control of subterranean termites in accordance with GEPA guidelines.
a) Installation of in-ground stations - NO DRILLING IN OR ON STRUCTURE.
b) All baiting above and in-ground stations - INTERIOR AND EXTERIOR.
c) Inspections and monitoring of stations - PROVIDE DETAILED LAYOUT OF STATIONS.
(3) Quarterly reports of baiting process and on-going monitoring.
Warranty. The Contractor shall provide the Contracting Officer with a written warranty in acceptable form, for each building treated for subterranean and dry wood termites good for a period of no less than one calendar year, guaranteeing re-treatment of any subsequent infestation and that any structural damage due to an infestation after the initial treatment shall be repaired at no additional cost to the Government.
5. Location of Work. Monthly service for mosquitoes, cockroaches, gnats, ants, fleas, flies, bees, mice, rodents, termites and fleas shall be provided to Building #50 - Naval Hospital.
