PERFORMANCE WORK STATEMENT (PWS)

FOR

SYNERGY CUSTODIAL SERVICES

26 OCT 2022

1.0 DESCRIPTION OF SERVICES

The Contractor shall provide all personnel, management, tools, equipment, labor and travel necessary to ensure that custodial services are performed at Synergy, located at 100 Park Place Drive, Warner Robins, Georgia 31088 for WR-ALC Software Engineering Group (SWEG) in a manner that will maintain a satisfactory facility condition and present a clean, neat and professional appearance.

Services shall include cleaning, trash removal, restocking restroom and cantina supplies, etc, in order to meet the requirements of this Performance Work Statement (PWS). Maximum cleaning frequencies are established in PWS Appendix A, Air Force Custodial Common Levels of Service Standards. Facility cleaning levels and estimated square footages are established in PWS Appendix B, Estimated Square Footages and Cleaning Level. Facility drawings of areas cleaned are established in Appendix D, Floor Plans. All work performed by the Contractor shall be performed in accordance with applicable laws, regulations, Headquarters Air Force (HAF) standards, instructions, and commercial practices.

1.1 FACILITY CLEANING SERVICES

1.1.1 Non-carpeted Floors (Hard Floors: Vinyl Tile, Ceramic Tile, Raised Floors, Wood, &

Concrete) Hard floors shall be maintained free of grit, soil, dust, scuff and heel marks, stains, spills, debris, litter and other foreign matter by effective routine cleaning. Cleaning shall be accomplished by the most appropriate method (vacuum, sweep, dust mop, damp mop, and spot clean) and with cleaning solutions if applicable, for the specific floor type. After cleaning, floor surfaces to include grout shall have a uniform, clean appearance without streaks, swirl marks, detergent residue, or any evidence of soil, stain, film, or standing water. Baseboards, corners, and wall/floor edges shall also be clean. Chairs, trash receptacles, and other easily moveable items shall be moved to maintain floors underneath these items and returned to their original and proper position after cleaning.

1.1.2 Carpeted Floors

Carpets shall be maintained free of soil, dirt, debris, litter and other foreign matter by effective routine vacuuming. The Contractor shall use efficient vacuum cleaners that offer high airflow, high efficiency filtration, and an adjustable rotating brush agitation for more effective soil removal. Filter(s) shall be cleaned/dumped/or replaced prior to vacuuming. Carpet and rugs need to be spot vacuumed Monday through Friday, deep vacuum all carpets and rugs, which includes all offices, every two weeks. Any spots and/or spills shall be removed by the carpet manufacturer’s approved methods or other commercially accepted practices as soon as noticed.

All tears, burns, and raveling shall be brought to the attention of the facility manager. Area and throw rugs shall also receive this service. Chairs, trash receptacles, and other easily moveable items shall be moved to maintain floors underneath these items and returned to their original and proper position after cleaning.

1.1.3 Floor (Walk-Off) Mats

The Contractor shall vacuum and/or clean interior and exterior floor mats both above and underneath mats. Afterwards, mats shall be returned to their normal location.

1.1.4 Trash Collection/Removal (Centralized Locations)

The Contractor shall collect and dispose of all trash from larger collection sites such as break areas, cantina, restrooms, coffee bars, etc. The Contractor is responsible for replacement of large plastic trash bags, that support the above larger collection sites and supply small office trash bags for individual offices and all workspaces (165 each - size 3 ½ gallon).

1.1.5 Touch Surfaces

Wipe all touch surface areas throughout the building on a weekly basis which includes the glass doors, lobby, window ledges and blinds, all conference room tables and chairs will also need to be clean.

1.2 Restrooms/Cantina Cleaning Services

1.2.1 Clean and Disinfect

All surfaces of sinks, toilets, urinals, lavatories, dispensers, plumbing fixtures, partitions, dispensers, doors, walls, stalls, stall doors, entry doors (including handle, kick plates, ventilation grates, and metal guards), walls, and other such surfaces shall be cleaned and disinfected using a germicidal detergent. Restrooms shall have a clean scent or no odor at all. Toilets, and urinals will be free of spots, water spots, scale buildup, soap scum, odors, and any other deposits.

Mirrors shall be clean and have no streaks or other removable matter. Partitions shall be smudge and stain free. Vents (within 72 inches of the floor) shall be clean. Restroom sinks, countertops, and fixtures shall be free from water and scale deposits, soil, streaks, and other removable matter. Walls and grout shall be free of all film, spots, and detergent buildup. A cleaning schedule tracking sheet shall be posted on the inside of each door and updated with each cleaning.

1.2.2 Restroom Floor Care

All floor surfaces shall be maintained in accordance with paragraph 1.1.1 by effective routine cleaning. Moveable items shall be tilted or moved to sweep and damp mop underneath. Grout on the wall and floor tiles shall be free of dirt, scum, mildew, and residue.

1.2.3 Stock Restroom Supplies

Contractor shall ensure restrooms are stocked sufficiently so any Contractor provided supplies including toilet tissue, paper towels, and hand soap do not run out. Supplies shall be stored in designated areas. If supplies run out prior to the next service date, the Contractor shall refill within two (2) hours of notification during normal business hours. Contractor to supply Government representative with key to paper dispensers (towels and toilet paper).

1.2.4 Cantina, Drinking Fountains & Coffee Bars

Contractor shall clean the cantina, the seven drinking fountains, two coffee bars typically located near or in the vicinity of restrooms, the six microwaves (inside and out), and the five refrigerators (inside and out). All porcelain and polished metal surfaces, including the orifices and drain, as well as exterior surfaces of fountains shall be cleaned and disinfected. Drinking fountains shall be free of streaks, stains, spots, smudges, scale, and other obvious soil.

1.2.5 Restroom Trash Removal

All trash containers in restrooms shall be emptied, trash disposed of, and containers returned to their original locations. The Contractor shall provide and replace any obviously soiled, worn or torn plastic trash can liners. Trash receptacles shall be left clean, free of foreign matter and free of odors.

1.2.6 High Dusting

High Dusting will be performed so that all dust, lint, cobwebs, litter and dry soils will be removed from all surfaces of 8’10” or more above floor surfaces area. Open ductwork not included in high dusting requirement.

1.3 Periodic Cleaning Services

1.3.1 Periodic Hard Floor Maintenance

Hard floors shall be cleaned, scrubbed, sealed, polished, waxed, and stripped as required for the appropriate surface to maintain, protect and ease normal routine floor cleanings. After periodic maintenance, floor surfaces to include grout shall have a uniform, clean appearance without streaks, swirl marks, detergent residue, or any evidence of soil, stain, film, or standing water.

1.3.2 Periodic Carpet Restorative Cleaning

Carpets shall be deep cleaned by an Institute of Inspection, Cleaning and Restoration Certification (IICRC) company that uses a hot water extraction method when specifically requested and approved by the Contracting Officer. No bonnet cleaning will be allowed. Carpets shall be cleaned in order to remove embedded soil from carpet fibers. Deep cleaning shall employ an effective technique and/or method to address soil suspension, soil extraction and drying. After deep cleaning, the carpeted area shall show an improvement in visible appearance. Cleaners shall not adversely affect the appearance or durability of the carpet and shall not cause shrinking. Cleaners used shall leave no residue to attract soil. A ll cleaning solutions shall be removed from baseboards, furniture, trash receptacles, chairs, and other similar items. Chairs, trash receptacles, and other items shall be moved to clean carpets underneath and returned to their original location after the carpet has dried or placed on Styrofoam blocks to allow circulation of air underneath until dry.

1.4 Contractor Provided Equipment and Supplies

1.4.1 Contractor-Furnished Equipment/Vehicles

The Contractor shall provide and maintain all equipment and vehicles necessary to perform the requirements of this contract. The Contractor may perform cleaning, minor repair and other normal maintenance requirements of equipment on site. Maintenance and repair of Contractor vehicles or their privately owned vehicles shall not be accomplished on site. Contractor vehicles shall have the company name prominently displayed on both sides of the vehicle and be maintained to present a neat, professional appearance. Contractor vehicles shall be used solely for the intended purpose of performing services in this performance work statement during normal hours of operation.

All equipment used in the performance of this contract shall be in good operable condition and carry a U/L (Underwriters Laboratory) listing. Equipment found to be unsafe and unable to function as designed, shall not be used in performance of this contract. The Contracting Officer’s Representative (COR) or WR-ALC/SE Safety Office may inspect the Contractor’s equipment and vehicles at any time and direct the removal of any unsafe or unusable equipment or vehicle from the installation.

The Contractor shall provide adequate numbers of equipment items and vehicles to effectively fulfill the scope of this contract. In addition, the Contractor shall maintain or have backup capability to provide continued service in the event primary equipment or vehicles are down for extended maintenance or repairs. Equipment failure shall not alleviate the Contractor from performing any requirement contained in this contract.

1.4.2 Contractor Furnished Restroom Supplies

Plastic Trash Can Liners/Bags: Suitable for the type of trash receptacle. Toilet Tissue: Two-ply.

Paper Towels: Bi-Fold, Tri-Fold, Dispenser type. Hand Sanitizer Soap: ESD-325 Certified Foam Soap for unique installed liquid hand sanitizer dispensers located at restroom sinks. Recycled Products: 100 percent of all paper products procured (toilet tissue and paper towels) shall contain recycled materials.

1.4.3 Contractor Furnished Cantina Supplies

Plastic Trash Can Liners/Bags: Suitable for the type of trash receptacle. Paper Towels: Roll type for wall mount dispenser. Recycled Products: 100 percent of all paper products procured (paper towels) shall contain recycled materials.

1.4.4 Contractor Furnished Hand Sanitizer

Maintain proper levels of hand sanitizer in five (5) Government furnished hand sanitizer dispensers.

1.5 Miscellaneous Requirements

• Turn off unnecessary lights.

• Report fire hazards, conditions, and items in need of repair to the Contracting Officer’s

Representative (COR) or facility manager.

• Turn in lost and found articles to the facility manager and/or security manager.

• Notify facility manager and/or security manager when unauthorized or suspicious persons are seen on the premises.

• Employees entering any facility or area after it has been secured shall enter through one door and lock the door behind them. These facilities or areas shall not be left unsecured at any time, including the period when work is being performed. Upon completing work in a facility or area, Contractor employees shall ensure all doors and windows that were secured upon their arrival are closed and secured upon their departure.

• The Contractor shall display the appropriate caution signs when cleaning floors or any other surface that might pose a risk to injury for any personnel that are present.

1.6 Over and Above Work

Work discovered during the course of performance that is:

1.6.1 Within the general scope of the contract, not covered by the line item(s) for the basic work under the contract, and necessary in order to satisfactorily complete the contract.

1.6.2 Upon discovery of the need for over and above work, the Contractor shall prepare and furnish to the Government a work request. Upon request the Contractor shall promptly submit to the Contracting Officer, a proposal for the over and above work. The Government and Contractor will then negotiate a settlement for the over and above work. Contract modifications will be executed to definitize all over and above work.

2.0 SERVICES SUMMARY (SS)

Contractor shall refill restroom supplies within two (2) hours of notification should supplies run out prior to next service date.

3.0 GOVERNMENT FURNISHED PROPERTY AND SERVICES

3.1 GOVERNMENT FURNISHED FACILITIES

The Government shall not provide office and storage space other than Janitorial closets in the buildings.

3.1.1 Janitorial Closets

Janitorial closets will be available for use by the Contractor, without cost, for the purpose of storing materials and equipment, excluding flammable materials. The Contractor will be required to maintain odorless and clean closets. Closets will be free of clutter and debris. Cleaning supplies and tools will be stored and organized in such a manner as to allow easy access and movement in closet. Closets provided for janitorial service use will be locked, if locks are available. The Contractor shall provide the Government POC a spare set of keys to janitorial closets. The Government will not be responsible for Contractor’s stored supplies or equipment kept in the building or janitor’s closets or for the Contractor’s employees’ personal belongings. A small excess supply of toilet paper and paper towels shall be kept in the janitorial closet. Contractor shall provide a set of keys to paper towel and toilet paper dispensers.

3.2 GOVERNMENT FURNISHED SUPPLIES AND EQUIPMENT. None.

3.3 Government Furnished Utilities

The Government will furnish electricity, water and sewage service as necessary for accomplishment of work in accordance with this contract.

3.3.1 Utility Conservation

The Contractor shall adhere to all base level utility conservation practices or requirements. The Contractor shall be responsible for operating under conditions that prevent waste of utilities.

3.4 Solid Waste Collection and Disposal

The Contractor shall use existing bulk containers to dispose of trash or solid waste generated from accomplishment of services detailed in this PWS. The Contractor shall adhere to all base level recycling programs.

4.0 GENERAL INFORMATION

4.1 Security Requirements

4.1.1 Government Security Regulations

The Contractor shall ensure personnel, information, system, facility, and international security requirements are met in accordance with 32 CFR Part 117, National Industrial Security Program Operating Manual (NISPOM), Air Force Instruction (AFI) 31-101, Integrated Defense.

4.1.2 Security Clearance

This custodial contract is an unclassified effort; therefore, access to classified information is prohibited. Contractor personnel shall not be authorized access to classified and/or controlled unclassified material.

4.1.3 Access to Government Facilities

The 402 SWEG Security Manager assigned to Robins South (Synergy) will ensure Contractor personnel comply with the security procedures of the facility. The contractor must be accompanied by a government employee at all times when access to Rm 171 (Lab) is required.

A detailed, reoccurring schedule for this endeavor must be communicated and followed by all parties.

4.1.4 Operations Security (OPSEC)

The Contractor shall ensure compliance with OPSEC requirements including procedures to protect sensitive, but unclassified, Government projects and/or programs. The Contractor shall ensure Contractor personnel who perform work at Robins South (Synergy) facility comply with the OPSEC procedures of the facility.

4.1.5 Security Incident or Violation

The Contractor shall immediately notify the on-site SWEG Security Manager of any potential or actual security incidents or violations including potential or actual unauthorized disclosure or compromise of classified and/or sensitive, but unclassified, information.

4.2 Government Points of Contact (POCs)/Facility Managers. The facility managers shall be notified of any issues with the day-to-day activities on the contract.

4.3 Place of Performance

Services shall be performed at 100 Park Place Drive, Warner Robins, Georgia 31088.

4.4 Schedule

Contractor personnel shall adhere to Government holiday and family day schedules. The Contractor shall ensure Contractor personnel are available for custodial support during the hours of 0630 through 1515 hours Monday through Friday. The Contractor may find it necessary to deviate from the normal base hours of operation, to ensure timely completion of work under this PWS at no additional cost to the Government. The Contractor shall perform facility cleaning, restroom cleaning services, and periodic cleaning services under this contract during the hours 1000- 1500 Monday through Friday.

4.5 Recognized Holidays. Unless otherwise required by the Government, the Contractor shall not have access to Government facilities to work or telework on all federal holidays or during facility closings as authorized by the Government. If the holiday falls on Saturday, it is observed on Friday. If the holiday falls on a Sunday, it is observed on Monday.

4.6 Facility Closures

Work scheduled but not accomplished because of facility closure due to weather, exercises, or actual alert, will be accomplished as soon as possible after reopening the facility.

4.7 Quality Management System (QMS)

Initial copy shall be submitted within fifteen (15) calendar days of contract award. Subsequent submittal shall be within thirty (30) calendar days of any updates. Copies (contractor format, Microsoft Office compatible) shall be submitted electronically to the Government POCs listed in

4.2 and the Contracting Officer. The Contractor shall ensure the quality of services through a quality and/or inspection system. The Contractor shall submit a Quality Control Plan (QCP) addressing detection of quality program problems and defects, identification of root causes for quality related problems/defects, correction of root causes related to detected problems/defects, and follow-up to ensure quality related problems/defects do not recur. The plan shall include the schedule of quality assurance/control inspections planned by the Contractor. 100% compliance with the QMS is required. (CDRL A001, Quality Management System (QMS), DI-MGMT- 82184)

4.7.1 The contractor will maintain an internal Self-Assessment Program (SAP) to evaluate the effectiveness of existing procedures and internal controls, to determine organizational compliance with regulatory requirements and to complement external inspections and assessments. Additionally, the contractor shall provide support, as required, for contingencies, exercises, and surge operations. The Contracting Officer, facility manager or security manager will notify the contractor as soon as notification of upcoming event becomes known.

4.7.2 The contractor shall support Air Force initiatives and participate in other applicable Quality Control programs and Continuous Process Improvement (CPI) initiatives designed to indicate the appropriate end-state of managed technical orders. The contractor shall comply with resulting changes to existing processes/procedures from these programs/initiatives. (AFMCI 90-104).

4.8 Affirmative Procurement Programs (APP)

4.8.1 Agency Affirmative Procurement Programs

4.8.1.1 In accordance with FAR 23.404(b)(1), 100% of purchases of Environmental Protection Agency (EPA)-designated products included in the Comprehensive Procurement Guidelines (CPG) list [https://www.epa.gov/smm/comprehensive-procurement-guideline-cpg-program] and/or United States Department of Agriculture (USDA)-designated products included in the biobased product listings [http://www.biopreferred.gov] shall, at a minimum, meet the EPA or USDA standards for recovered materials or biobased content, respectively, unless an item cannot be acquired competitively within a realistic timeframe, meet appropriate performance standards, and/or be acquired at a reasonable price.

4.8.1.2 In accordance with FAR 23.404(a)(iii), the Contractor shall provide estimates and verification of recovered material for EPA-designated CPG products and certification for both EPA-designated CPG products and USDA-designated biobased products. In accordance with the Robins Air Force Base, Georgia / Green Procurement Program (GPP) Plan, the Contractor shall provide estimates and certifications to the Government for completion of the applicable attachment(s) to be included in the Purchase Request (PR) package.

4.9 Cleaning and Restroom Supplies

The Contractor shall furnish a listing of all cleaning and restroom supplies utilized during the contract period to the Contracting Officer and Government POC within five (5) calendar days of contract award. After award, any new cleaning supplies added to the contract will need acceptance before being used on the installation. The cleaning inventory supply list shall contain the following information:

All cleaning supplies shall be non-flammable. NOTE: Products containing chlorofluorocarbons (CFC’s) or ozone depleting substances shall not be allowed or approved for use. The Contractor shall be responsible for the proper disposal of all their cleaning supplies.

4.10 HAZMAT

Any products that meet the criteria of “Hazardous Waste” must be bar coded and tracked until permanently removed from the property. Criteria for identifying hazardous waste, is contained in Subpart C of 40 CFR, Part 261.

4.10.1. Notification of Environmental Spills

If the Contractor spills or releases any substance listed in 40 CFR 302 into the environment, the Contractor shall immediately report the incident to the Environmental Coordinator IAW local procedures and notify the Quality Assurance Personnel (Contracting Officer or Contracting Officer’s Representative) QAP. (Reference most recent version (including revisions adopted during the term of the contract) of Environmental Requirements for Robins Air Force Base, RAFB-01560.

4.10.2 Hazardous Material/ Waste Management

The Contractor shall dispose of any Hazardous Waste IAW Federal, state and local regulations.

The Contractor shall not use products that are or contain Toxic 17 chemicals, Extremely Hazardous Substances (EHS), Ozone Depleting Substances (ODS), and/or Persistent Bio accumulative and Toxic (PBT) chemicals. Any Hazardous Material (HM) containing one of these banned substances will not be allowed on base.

4.11 Environmental Requirements for Robins AFB Base

The Contractor shall comply with most recent version (including revisions adopted during the term of the contract) of Environmental Requirements for Robins Air Force Base, RAFB-01560.

4.12 Weapons, Firearms, and Ammunition

Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their Contractor-owned or privately owned vehicle while at the (Synergy) Complex.

4.13 Contractor Manager

The Contractor shall establish and maintain an off-site office or utilize existing office location.

The contract manager or alternate shall be available during normal duty hours to meet with Government personnel at the installation within 1 hour of notification by the Contracting Officer’s Representative (COR) to discuss problem areas. The Contractor shall provide the Contracting Officer’s Representative (COR) telephone number(s) where validation results and complaints can be reported. This information will be kept updated by the Contractor whenever personnel changes occur. The manager or alternate shall have full authority to act for the Contractor on all contract matters relating to the daily operation of this contract.

4.14 Personnel

Contractor personnel shall present a neat appearance. Contractor personnel shall be easily recognizable while in the facility in conjunction with this contract. This shall be accomplished through the wear of distinctive clothing, overcoats, or hats, bearing the company name or logo.

The coloring or design of the items selected should be such that identifies personnel easily and quickly for reasons of safety and personal protection. The Government is authorized to restrict the employment under the contract of any Contractor employee or prospective Contractor employee, who is identified as a potential threat to the health, safety, security, general well-being, or operational mission of the installation and its population.

4.15 Key Control

The Contractor shall establish and implement methods of ensuring keys/combinations issued to the Contractor by the Government are not lost or misplaced and are not used by unauthorized persons. The Contractor shall not duplicate any keys issued by the Government. The Contractor shall immediately report to the Contracting Officer, facility manager or security manager any occurrences of lost or duplicated keys. In the event keys, other than master keys, are lost or duplicated, the Contractor may be required, upon written direction of the Contracting Officer, to re-key or replace the affected lock or locks with no cost to the Government. The Government may, however, at its option, replace the affected lock or locks or perform re-keying and deduct the cost of such from the monthly payment due the Contractor. In the event a master key is lost or duplicated, the Government shall replace all locks and keys for that system, and the total cost will be deducted from the monthly payment due the Contractor. Badges and keys will be issue out each service day, contractor employees will need to return them back to the security window after the completion of work.

4.16 Schedules

The Contractor shall submit an annual schedule for services described in PWS paragraphs 1.1, 1.2, 1.3, and Appendices A and B to the Contracting Officer and Government POC for acceptance within five (5) calendar days of contract award, identifying by room number/name in numerical order, and approximate time cleaning will occur. The Contractor shall be allowed 30 days to adjust work schedules based on inspections, use, and traffic, minimum frequencies listed in Appendix A and with the Contracting Officer and Government POC as appropriate. This second work schedule shall be submitted to the Contracting Officer and Government POC for acceptance within 10 calendar days after the first contract month period and shall be known as the Cleaning Frequency Schedule (CFS). Any permanent changes to the CFS must be submitted 10 calendar days before implementation and receive Contracting Officer acceptance before the Government will allow the proposed changes. The CFS may be submitted electronically using a file format compatible with Government software programs such as "Microsoft Office" software. One time (non-permanent) schedule changes due to customer requests or base exercises shall be submitted to the Government POC listed in 4.2 and do not require Contracting Officer approval. (CDRL A002 Work Management Plan, DI-MGMT-81911/NOT 1).

4.17 Contract Modification

4.17.1 Existing Facility Floor Plan Change

Modifications for a price increase or decrease to the contract shall only be made when the overall identified “cleaning area” square footage, as identified in Appendix B, of a facility increases or decreases or a frequency of cleaning service changes.

4.17.2 New Facility and/or New Facility Addition

Modifications for a price increase to the contract shall only be made when a new facility or addition is officially added to the contract and identified in Appendix A and B, with all information agreed upon.

4.18 Environmental Management System (EMS). Environmental Management Systems (EMS) Awareness Training: All contractor personnel working on Robins AFB who perform activities on the installation are required to complete Air Force-provided initial EMS Awareness Training. It is the responsibility of the Prime Contractor to ensure that all sub-contractors, vendors, and employees complete this training prior to beginning work on Robins AFB.

Notification of training completion shall be sent to the Robins AFB Contracting Officer (CO) to be maintained in the contract file for tracking purposes. Failure to provide documentation of EMS Training may result in termination of the contract.

4.13.1 EMS training is provided by Environmental Management. Request for Robins EMS Awareness training through email at 78ceg.cev.FrontOfc@us.af.mil. Contractors have two options to satisfy the mandatory EMS Awareness Training requirement. Contractors that do not have a Common Access Card must exercise Option 2.

4.13.2 Option 1 (Preferred Option): The Air Force version of EMS Awareness Training is available through the myLearning website: https://lms-jets.cce.af.mil/. EMS Awareness Training is located under the Air Force Civil Engineer Center (AFCEC) /Air Force Environmental Courses tab. This option is only available to contractors who have a CAC and requires self-enrollment.

4.13.3 Option 2: Contractor requests a copy of the Robins EMS Awareness Training Power Point Presentation from Environmental Management. The request should be sent to the Environment Management Workflow Box at the following email address, 78ceg.cev.FrontOfc@us.af.mil. Once the presentation is received, the contractor is responsible for ensuring that all his/her employees view the training.

4.14 ISO 50001 – Energy Management System. Due to the WR-ALC being ISO 500001 certified, all contractors are subject to the Energy Management System (EnMS) requirements and shall abide by certification guidelines whenever applicable.

4.14.1. EnMS Training. All contractor personnel working on Robins AFB who perform activities on the installation are required to complete Air Force provided initial EnMS Training. 4.14.1.1. EnMS training is provided by the Energy Assurance Office. The Contractor shall request the training by sending an email to 402MXSG.MXDEA.EnergyAssurance@us.af.mil.

4.14.2. Notification of training completion shall be sent to the Energy Assurance Office (402MXSG.MXDEA.EnergyAssurance@us.af.mil) and the CSMO POC to be maintained for tracking purposes. 4.20 Contractor Safety Program. (See Appendix C, Part II, Para 7) The Contractor shall establish and maintain an acceptable safety program in accordance with their accepted Safety and Health Plan. The Air Force is part of the OSHA Voluntary Protection Program (VPP). All shall familiarize themselves with the requirements of VPP. Information on the VPP can be accessed at http://www.osha.gov/dcsp/vpp/index.html. Additionally, the Contractor is responsible for the safety and health of all subcontractor employees. No more than two Class C mishaps per contract year and Zero Class A or B mishaps will be tolerated. All changes to the plan must also be submitted in writing and approved by the Government.

4.20.1 Safety and Health Plan: Within 10 calendar days after contract award, the contractor shall email a copy of their company Safety Plan that is required to ensure compliance with the Occupational Safety and Health Act (OSHA) (Public Law 91-596) to 402mxw.se.wf@us.af.mil.

The Safety Plan will be provided to the Safety Office for review. The Safety Office will provide acceptance or non-acceptance within 5 working days of receipt to the government point-of-contact who will notify the CO. Subsequent submittals shall be within five (5) days of any required updates (CDRL A003, DI-SAFT-82080/NOT 1, Contractor's Safety Plan).

4.20.2 The Contractor Safety Plan shall address in detail the items listed in the Contract Appendix C, Part II through Part IV as applicable. For task specific procedures identified in the Contract Appendix C, the contractor’s internal procedures for those tasks must be identifiable within the submitted Safety Plan or shall be provided in an amendment or addendum to the company’s Safety Plan. If task specific procedures identified in the Contract Appendix C, are not submitted with the contractor’s Safety Plan, the submitted Safety Plan will be returned as NOT ACCEPTED. The contractor will have 5 working days to resubmit the Safety Plan, amendment or addendum to address or correct issues identified for non-acceptance. The Safety Office will provide acceptance or non-acceptance of the resubmission within 5 workings days of receipt to the government POC who will notify the CO.

4.21 Mishap Notification. (See Appendix C, Part II, Para 5) The Contractor shall notify the Robins AFB Safety Office telephonically at (478)-222-0043 during duty hours and (478)-926- 3567 after duty hours, within four (4) hours after initial notification of all mishaps or incidents mailto:402MXSG.MXDEA.EnergyAssurance@us.af.mil mailto:402MXSG.MXDEA.EnergyAssurance@us.af.mil http://www.osha.gov/dcsp/vpp/index.html mailto:402mxw.se.wf@us.af.mil for damage to DoD property (material plus labor) entrusted by this contract and employee injuries as a result of the contract operation. A follow-up (written or e-mail) of this mishap/incident notification shall be sent within three (3) calendar days to the Government Representative, the Procurement Contracting Officer (PCO) or the Administrative Contracting Officer (ACO) (with a copy to program manager), who will forward it to the Safety Office. The information that is not available at the time of the initial notification shall be provided within 20 calendar days after the mishap. Mishap notification shall contain, as a minimum, the following information:

a. Contract, Contract Number, Name and Title of Person(s) Reporting

b. Date, Time and exact location of accident/incident

c. Brief Narrative of accident/incident (Events leading to accident/incident)

d. Cause of accident/incident, if known

e. Estimated cost of accident/incident (material and labor to repair/replace)

f. Impound Authority Name and Phone Number (as applicable)

g. Nomenclature of equipment and personnel involved in accident/incident

h. Corrective actions (taken or proposed)

i. Other pertinent information (CDRL A004, DI-SAFT-81563/NOT 1, Accident/Incident Report)

4.22 Mission Essential Functions. The Functional Service Manager (FSM) has determined, by placing in writing their decision, that this requirement is NOT Mission Essential (M-E) in accordance with DFARS 237.76, Continuation of Essential Contractor Services.

5.0 APPENDICES

The Government will make all publications, forms, references and report formats listed available. Publications can be accessed on-line at http://www.e-publishing.af.mil/. Supplements or amendments to listed publication from any organizational level may be issued during the life of the contract. The Contractor shall immediately implement those changes in publications, which result in a decrease, or no change in the contract price. Prior to implementing any such revision, supplement, or amendment that will result in an increase in contract price, the Contractor shall submit to the Contracting Officer a price proposal and obtain prior approval.

Price proposals shall be submitted within 15 calendar days from the date the Contractor receives notice of the revision, supplement, or amendment giving rise to the increase in cost of performance. Changes in the contract price due to supplements and amendments shall be considered under the FAR 52.212-4, Contract Terms and Conditions - Commercial Items clause.

Failure of the Contractor to submit a price proposal within 15 calendar days from the date of receipt of any change, shall entitle the Government to performance in accordance with such change, at no increase in contract price.
