1. DESCRIPTION OF SERVICES: The Contractor shall provide all management, labor, tools, supplies, materials, equipment, and lubricants necessary to perform automatic door inspections, testing, preventive maintenance and repairs at Patrick Air Force Base, Florida. Maintenance will be in compliance with manufacturer’s specifications.

1.1 INSPECTIONS, PREVENTATIVE MAINTENANCE AND TESTING: The Contractor shall perform these services quarterly (every three months) beginning the first full month following the award of the contract. NOTE: Contractor SHALL NOT perform any maintenance on doors currently under a manufacturer’s warranty without prior written approval of the Contracting Officer

Representative (COR). Performance shall be in compliance with:

a.

ANSI A 156.10 Standard for Power Operated Pedestrian Doors, encompassing automatic sliding, swinging and folding doors.

b.

ANSI A156.19 Standard for Power Assist and Low Energy Power Operated Doors, encompassing doors that operates at slow speed with low power and can be remotely activated by a push switch/plate or other remote activation device.

c. ANSI A156.27 Standard for Revolving Doors

d. ANSI A117.1 or ADA Assure compliance for accessibility

e. UL or ETL UL or ETL Listing, or the equivalent

1.1.1 INSPECTIONS: If required, the Contractor shall perform diagnoses of electrical and mechanical problems to determine the extent of any needed repairs or replacements required to restore the unit to a fully operating reliable status. Doors requiring substantial repair may be deferred for “Scheduled Repairs”. When Doors are deferred the Contractor must submit a “Work

Request” or “Cost Proposal” prior to repair. The Contractor shall notify the Contracting Officer

(CO) in writing, within one work day of discovery, of any automatic door found:

a. to be unrepairable

b. to need repair to the operating systems, doors, frames and/or glass

c. to require repairs beyond those already specified

d. to be a safety hazard

1.1.1.1 STANDARDS: Acceptable standards include proper wind and weather seals, proper alignment for smooth operation and effective closure; rated speed; responsive and reliable door operation; traffic handling capabilities; satisfactory resistance to the effects of highly variable, exterior wind load.

1.1.1.2 DOWNTIME: Downtime must not exceed more than 72 hours per year per automatic door and a maximum of three emergency service calls per year for all automatic doors. Increased frequency of service calls is not considered acceptable performance.

1.1.2 PREVENTATIVE MAINTENANCE AND TESTING: Preventative maintenance and testing shall minimize wear and tear on all parts of the doors listed in Appendix B. Quarterly preventative maintenance shall include at a minimum: cleaning mechanisms, lubricating, adjusting door alignments, adjusting mechanical elements, adjusting electronic elements and replacement small parts. Testing shall be performed IAW paragraph 1.1

1.1.2.1 MAINTENANCE CONSUMABLES AND SMALL PARTS: During the performance of this contract, without additional cost to the Government, the Contractor shall provide all of the following items: all lubricants, rollers, springs, program chips, sealant, bumper pads, sweeps, astragals, and limit switches. In addition, the Contractor shall bear the cost of small parts costing

$12.00 and below.

1.2 SCHEDULE: The Contractor shall submit a quarterly maintenance schedule that includes services at each automatic door location. The Contractor shall submit the schedule at the pre-performance meeting. The CO will approve or disapprove the schedule. The Contractor shall modify the schedule if necessary.

1.2.1 ALTERNATE SCHEDULE: If, for any reason the Contractor cannot comply with the approved schedule, the Contractor shall notify the Contracting Officer and arrange for an alternate schedule that will be approved or disapproved by the contacting officer. Adjustments to the schedule will be made at no additional cost to the Government.

1.3 SCHEDULED REPAIRS: If parts are available, repairs $2,000.00 or less shall be repaired during regularly scheduled quarterly visits. If the Contractor is unable to complete “Scheduled

Repairs” during the regularly scheduled visits the Contractor shall notify the CO and COR, to discuss and determine whether repairs can wait until the next scheduled quarterly visit.

1.3.1 QUOTES: The Contractor shall obtain quotes from three competitive sources for any repair cost exceeding $2,000.00. The Contractor shall document and provide a copy of this record to CO.

The Contractor shall obtain prior CO approval for repair cost exceeding $2,000.00. Discounts and rebates on parts purchased by the Contractor to complete necessary and authorized repairs shall be credited to the Government.

1.3.2 WARRANTY: The Contractor shall be responsible for submitting claims for manufacture warranty repairs or replacement. The Contractor shall register all newly purchased parts or materials for any applicable warranty, maintain warranty documentation, and receive manufacturer recall information for potential fire and safety hazards.

1.4 OFF-SITE REPAIR OF GOVERNMENT PROPERTY: When repairs cannot be accomplished on-site, the contractor shall be responsible for any damages or loss from the time an item is removed until it is returned and reinstalled. The CO shall be notified prior to an item being removed. The CO and Contractor shall conduct an inventory of items removed and returned.

1.5 UN-SCHEDULED AND EMERGENCY REPAIRS: The Contractor shall be readily available for unscheduled emergency repair services. These services may occur any time between regularly scheduled quarterly inspection and maintenance visits, on a 24/7 basis. Contractor emergency responders shall call the authorized caller back upon arrival on base. The Contractor must be onsite and ready to begin the required work within 6 hours of notification. The Contractor shall arrive at

Patrick AFB, FL no later than 1000 hours of the next calendar day.

2. SERVICES SUMMARY: The Contractor service delivery requirements are summarized into performance objectives that relate directly to standards of performance required to meet mission essential needs. For the Performance Objective to be met, service delivery must be in substantial compliance with applicable performance standards. The Performance Threshold describes the minimum overall levels of service delivery required for acceptable quality control. Failure to meet these Performance Thresholds means that Contractor Quality Control is unacceptable. The following is a list of the key performance objectives that will be verified as contractually compliant by government personnel, however, inspection of any contract requirement is authorized.

3. GOVERNMENT FURNISHED PROPERTY AND SERVICES: Water and electricity are available at or near the automatic doors listed in Appendix B. These services are available for use by the Contractor only in quantities necessary to perform the work as required by this PWS. The

Contractor shall be responsible for all temporary connections necessary for operations and for their removal prior to completion of work.

4. GENERAL INFORMATION:

4.1 QUALITY ASSURANCE: The Contractor must perform all services IAW the prescribed performance objectives stated in the Service Summary. The Government has the right to inspect all services called for by the contract, to the extent practicable at all times and places during the term of the contract.

4.1.1 The Government will inspect and evaluate the Contractor’s performance to ensure services are received in accordance with requirements set forth in this contract. The COR will use the

Contractor’s work schedule or modified version thereof, to record validation results. Results of the validation then become the official Air Force record of the Contractor’s performance. When a performance threshold has not been met or Contractor performance has not been accomplished, the

COR will initiate and provide a report to the CO.

4.1.2 The CO along with the Contracting Officer Representative are the authorized Government representative(s) who will perform assessments of the Contractor’s performance.

4.2 SECURITY REQUIREMENTS: Security and base access requirements are contained in clause 5352.242-9000 “Contractor Access to Air Force Installations”. Contractor employees will be required to obtain and display identification passes. Anticipate delays in getting commercial vehicles on base and allow time for commercial vehicles to reach their destination by driving designated routes at posted speed limits throughout the base. Procedures for commercial vehicle access to the base are subject to change without prior notice.

4.2.1 ID Passes: The Contractor is responsible to obtain such clearances and/or passes as required for his/her employees to enter military installations on Patrick AFB prior to start to work. The

Contractor shall be responsible for all passes issued to his/her employees under this contract.

Applications shall be coordinated through the CO. At the time of application, the 45th Security

Forces Squadron may conduct a background check on employees. Upon termination of employment, the Contractor shall be responsible for returning identification cards and/or base passes to Security Forces within 24 hours.

4.2.2 Work in Controlled Areas:

4.2.2.1 Part of the work of this contract will be performed in the AFTAC areas of buildings 10989, 10991 and 10992, which is rigidly security controlled. All Contractor personnel will be allowed access to the building through only one entrance and escorted at all time while in the building.

4.2.2.2 The building and facilities will continue to be operated by Government personnel during the work of this contract. The Contractor shall keep all noise, dust, traffic, and interruption of

Government activities to an absolute minimum.

4.2.2.3 The Contractor shall vacate the building upon alert conditions, normally not exceeding 4 hours duration.

4.2.2.4 All Contractors will provide an access list to the AFTAC’s Security section at least one day before work is to start. The access list will indicate the names of all personnel who will require entry into the controlled areas. Each individual shall show identification and be signed in daily.

Departure and reentry on the same day will not require signing in a second time.

4.2.2.5 The rear entrance door shall be designated for Contractor personnel entry and departure.

Special arrangements will be made in advance with the AFTAC Security section and the AFTAC

Civil Engineering section for other entrances that need to be used for movement of equipment and supplies in or out of the controlled areas.

4.2.2.6 Since assigning escorts is a matter requiring prior coordination, a set time for meals and breaks shall be established by the Contractor as identified to AFTAC Security Police and the assigned escort. Workers who return from meals and breaks before the end of the established period shall be required to wait until the escort returns. Lunch and breaks shall not be taken in the main lobby.

4.3 HOURS OF OPERATION:

4.3.1 Normal Hours of Operation: The Contractor shall perform all routine services during normal work hours. Normal work hours are 7:00 AM to 4:00 PM (EST), Monday thru Friday.

The Contractor may be required to work during weekends in case of Unscheduled (Emergency)

Repairs. In addition, the Contractor may be authorized and permitted to work outside normal work hours, including the extension of daily work hours and/or weekend work, by first obtaining approvals from the affected facility manager(s) and CO.

4.3.2 Recognized Holidays: The Contractor is not required to provide service on the following days: New Year’s Day, Martin Luther King Day, Presidents Day, Memorial Day, Independence

Day, Labor Day, Columbus Day, Veterans Days, Thanksgiving Day and Christmas Day. If the holiday falls on Saturday, it is observed on Friday. If the holiday falls on a Sunday, it is observed on Monday.

4.4 CONSERVATION OF UTILITIES: The Contractor shall instruct employees in utilities conservation practices. The Contractor shall be responsible for operating under conditions which prevent the waste of utilities.

4.5 ENVIRONMENTAL CONTROLS: The Contractor shall be responsible for the safe handling of the Contractor’s hazardous materials while on PAFB. The Contractor shall immediately report spills/leaks of hazardous materials to the PAFB Fire Department by dialing

“911”. The Contractor shall report all spills, leaks to 45 CES/CEIE and the CO. If the event occurs on weekends, holidays or after normal business hours, contact 45 CES/CEIE and the CO the next business day.

4.5.1 Material Storage and Use: The Contractor shall follow manufacturer’s guidelines and professional recommendations for control of humidity, temperature, cleanliness, and materials handling. This includes hazardous materials. The Contractor shall comply with, but not limited to:

AFI 32-7086, Hazardous Materials Management and the 45th SW HAZMAT Tracking Procedures for Construction and Service Contractors. No hazardous materials are allowed to be brought on to any 45 SW Installation without prior approval. The contractor shall contact the 45 CES/CEIE

Hazardous Materials Program Manager at 321-494-9268 to review hazardous materials authorization procedures prior to hazardous materials being brought on to 45 SW facilities.

4.6 GOVERNMENT OBSERVATIONS: Government personnel, other than CO’s, may from time to time, with CO coordination, observe Contractor operations. However, these personnel may not interfere with Contractor performance or make any changes to the contract.

4.7 SAFETY REQUIREMENTS: In performing work under this contract, the Contractor shall:

4.7.1 Perform work in a safe manner as required by OSHA, General Industry, Occupational Safety and Health Standards

4.7.2 Develop and provide a safety plan for the protection of Government facilities and property and to provide a safe work environment for Contractor personnel.

4.7.3 Provide protection to Government property to prevent damage during the period of time the property is under the control or in possession of the Contractor.

4.7.4 Include a clause in all subcontracts to require sub-contractors to comply with the safety provisions of this contract as applicable.

4.7.5 Record and report promptly (within one hour) to the CO, all available facts relating to each instance of damage to Government property or injury to either Contractor or Government personnel.

4.7.6 In the event of an accident/mishap, take reasonable and prudent action to establish control of the accident/mishap scene, prevent further damage to persons or property, and preserve evidence until released by the accident/mishap investigative authority through the CO.

4.7.7 If applicable, include a clause in each applicable subcontract requiring the subcontractor’s cooperation and assistance in accident reporting and investigation.

4.8 PUBLICATIONS:

4.8.1 Compliance with all publications, regulations and operating instructions provided by the

Government is required when:

4.8.2 They pertain to the procedures for materials expediting herein and where the Contractor is authorized by the Performance Work Statement to accomplish the work specified in the publication, regulation or operating instructions.

4.8.3 The publications prescribe USAF policies, use of materials, procedures and processes applicable to the work requirements.

4.8.4 The Contractor is required to acquire and work on the latest version of the publication.

4.9 CONTRACTOR PERSONNEL: The Contractor shall have at least two (2) years’ experience as a specialized automatic door installation, service and repair business.

4.9.1 Contract Manager: The Contractor shall provide a contract manager and alternate contract manager (responsible in the absence of the contract manager) who shall be responsible for the performance of the services. The names of the contract manager and alternate(s) shall be provided to the CO in writing prior to the beginning of the first performance period. The contract manager and alternate(s) must be able to read, write, speak, and understand English.

4.9.2 The contract manager and alternate shall have full authority to act for the Contractor on all contract matters relating to daily operations of this contract.

4.9.3 The contract manager or alternate shall be available during normal duty hours within two (2) hours to meet on the installation with Government personnel (designated by the CO) to discuss problems. After normal duty hours the contract manager or alternate shall be available within four

(4) hours.

4.9.4 Contractor Employees: The Contractor shall not employ persons for work on this contract if such employee is identified to the Contractor by the CO as a potential threat to the health, safety, security, general well-being or operational mission of the installation and its population.

4.9.5 Contractor personnel shall present a neat appearance and be easily recognized as Contractor employees. This may be accomplished by wearing distinctive clothing bearing the name of the company or by wearing appropriate badges, which contain the company name and employee name in English.

4.9.6 The Contractor shall not employ any person who is an employee of the U.S. Government if employing that person would create a conflict of interest. Additionally, the Contractor shall not employ any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives approval according to DOD 5500-7-R Joint Ethics

Regulation. The Contractor shall not employ any person who is an employee of the Department of the Air Force if such employment would be contrary to the policies in AFI 64-106, Air Force

Industrial Labor Relations Activities.

4.9.7 Unless notified by the CO, the Contractor shall report to the 45 th

Civil Engineer Squadron

Operations Engineering Service Contracts section after performing the services in this contract.

4.9.8 Contractor Personnel Qualifications and Appearance: The Contractor shall be, or shall assign only, a fully qualified person(s) to perform the work required by this Performance Work

Statement (PWS). The Contractor is required to submit a list of employees, certifying their qualifications to the CO, no later than 30 days after the award of the contract.
