PERFORMANCE WORK STATEMENT FOR
INDUSTRIAL AND RESIDENTIAL INTEGRATED
SOLID WASTE MANAGEMENT

Voluntary Protection Program (VPP) Addendum
1.0. DESCRIPTION OF SERVICES. The Contractor shall provide all personnel, equipment, tools, materials, vehicles, supervision, and other items and services necessary to perform integrated solid waste management, to include residential refuse/recycling collection for the Military Family Housing (MFH) units at Wright Patterson AFB, Ohio. The Contractor shall perform to the standards in the contract as well as all local, state, and federal regulations, international agreements, Status of Forces Agreements (SOFA) and appropriate Final Governing Standards (FGS) or, in their absence, the Overseas Environmental Baseline Guidance Document (OEBGD). Estimated workload factors are in Appendix A, and collection areas are in Appendix A, Tables: A-1.1 and A-2.1.
1.1. COLLECTION REQUIREMENTS.
1.1.1. Main Base
1.1.1.1 Municipal Solid Waste. The Contractor shall empty refuse containers based on historical data for the first 30 days of performance or at least weekly. The Contractor shall establish initial vehicle routes and collection schedules upon contract award. Details on size, routes, and frequencies shall be submitted to the Contracting Officer (CO) 5 calendar days prior to start of contract performance. The Contractor may propose adjustments to the schedule at any time but as a minimum the Contracting Officer’s designated Representative (COR) and contractor shall monitor fill capacities during the first 30 days of performance and during one other period to be agreed upon during the same performance period, in order to determine if adjustments to container size, routes, or frequencies should be made to provide the minimum number of pickups necessary to maintain a healthy, vector free environment and prevent waste container overflow.
No changes are allowed to the schedule or haul route without CO or COR coordination. Historical frequency tables are located at Appendix A, Table A-1. When the Contractor completes service at a location there shall be no litter, waste, or recyclable material on the ground within a 10 foot radius of the container.
1.1.1.2. Food Waste. Empty containers used to dispose of food waste as necessary to maintain a healthy, vector free environment (on days when the serviced facility is in operation). Food Waste containers and pickup days are identified in Appendix A, Table A-1.
1.1.2. Military Family Housing (MFH) Non-Privatized
1.1.2.1. Municipal Solid Waste. MSW containers shall be placed at curbside by MFH occupants on scheduled pickup days. Pickup shall be accomplished once per week. The contractor is responsible for cleaning up any spills, debris, etc. while collecting MSW at each location.
1.1.2.2. Recycling (Military/Family Housing). Contractor provided recycling containers will be placed at curbside by MFH occupants on scheduled pickup days. Pickup shall be accomplished once per week. See paragraph 1.2.1 for a list of recyclable materials. The contractor shall place the containers in an upright position after emptying. At no time shall the recyclables be mixed with other types of waste.
1.1.2.3. Compostable Organics (Military/Family Housing). Yard and wood waste less than 50 pounds and six feet in length will be placed at curbside by MFH occupants on scheduled pickup days. Pickup shall be accomplished once per week year-round. During the first two weeks of January, contractor shall pickup and dispose of Christmas trees (not subject to length/weight requirements) placed at curbside by MFH residents. At no time will this yard waste be mixed with other types of waste.
1.1.2.4. Bulk Item Pickup. The contractor shall pick up bulk items placed at the curbside by MFH occupants throughout the year during the contract period. Contractor shall submit a proposed bulk item pickup schedule as part of the proposal package. Bulk items may consist of (but are not limited to) furniture, consumer electronics and appliances that do not contain Ozone Depleting Substances, cartons, and bundles. The government reserves the right to reduce the Bulk Item Pickup frequency at any time. The contractor shall divert recyclable items to the base recycling center.
1.1.2.5. On-Call Dumpster (Military/Family Housing). The Contractor shall furnish on call collection services for Military Family Housing. These services shall include providing 8 cubic yard containers as required. The COR will provide instruction to the Contractor: 1.) when to deliver and where to place the containers; 2.) when to empty containers; and 3.) when to permanently remove the containers. The Contractor shall within one (1) business day of notification. All containers provided by the contractor shall be clean and odor-free upon arrival at the designated site.
1.1.3.1. Unscheduled Pickup and Special Events (MSW). In addition to the scheduled solid waste collections, the CO or COR may request the contractor make unscheduled collections and disposals.
For unscheduled events, the contractor shall respond within one business day of notification from the CO or COR. For special events, the contractor shall also provide (or reposition as directed) solid waste containers. These containers shall be placed in locations for a length of time designated by the CO or COR. The CO shall notify the contractor in writing t least 7 days in advance of the special event date. All containers provided by the Contractor will be clean and odor- free upon arrival of designated site. See Appendix A, Table A-1.1 for workload data.
1.1.3.2. Construction and Demolition Debris (C&D). When requested by the CO or COR, the contractor shall use government furnished roll-off containers for collection of C&D.
The contractor shall place containers at the designated site within one (1) business day of notification by the CO or COR. The contractor is responsible for disposal being in accordance with existing local, state, and federal regulations. The contractor shall be responsible for any permit or fees associated with the use of off-base disposal location and invoice for reimbursement.
The contractor shall recycle or dispose of all C&D at a recycling facility or permitted landfill.
The contractor must provide weight tickets of all roll-off containers to the COR.
1.1.3.3. Public/Common Use MSW Receptacles (National Museum of the United States Air Force). Contractor shall empty, collect, and dispose of all waste, and replace Contractor provided liners in government furnished containers at the locations designated in Appendix A Contractor shall empty, collect, and dispose of all waste located in public/common areas during their regular weekly collection.
1.1.3. Additional Pickups: In the event unusual conditions cause a container to require pickup before the scheduled time, the CO or COR may request in writing (email/ sufficient) pickup within 24 hours
1.1.3. . Container Relocations: The Government may request relocation of containers. The request will be made in writing and the container will be relocated within hours.
1.1.3. . On Call Collection (Asbestos). Asbestos shall be disposed of in accordance with local, state and federal regulation in force at the time of disposal. The government shall provide one 30 yard covered container for collection of asbestos waste material. This container shall be identified on all sides as containing asbestos material. The location of this container shall be as specified by the CO/COR. Relocation of this container shall be done as specified by the CO/COR at no additional cost to the government. Verification that the asbestos waste is double bagged and properly tagged is the responsibility of WPAFB Civil Engineering in accordance with EPA 40 CFR-61M, 5 Apr 1984 and Ohio EPA OAC 3745-20. The contractor is responsible for disposal being in accordance with existing local, state, and federal regulations. The contractor shall be responsible for any permit or fees associated with the use of off-base disposal locations and invoice for reimbursement.
1.1.3. . Surge Requirement Capabilities. Refuse collection services to support national commitments up to and including a declared war that would generate surge requirements shall be provided upon notification by the CO/COR. These services shall be provided 24 hours a day, 7 days a week as required for the duration of the emergency. Surge requirements beyond the capability of the contractor may be augmented by the government when mission accomplishment is endangered. The CO/COR shall advise the contractor of the required level of effort verbally and follow-up in writing.
1.1.3. . Composting. The contractor shall provide the following services for yard waste:
1.1.3. .1. Off-Base Disposal of Composting Materials Government furnished collection containers shall be emptied of compost materials. The compost shall be disposed of at an Environmental Protection Agency (EPA) authorized site. Compost collection shall be on an on- call basis, as requested by the COR. Contractor shall respond within 24 hours of notification. These compost collection station is located at building 10879 Area A. The contractor shall be responsible for any permit or fees associated with the use of off-base disposal locations and invoice for reimbursement.
1.1.3. . Saw Dust Collection. The contractor shall collect saw dust from designated locations.
These saw dust collection locations are identified in Appendix A. Table A-1.1. The contractor is responsible for disposal being in accordance with existing local, state, and federal regulations. The contractor shall be responsible for any permit or fees associated with the use of off-base disposal locations and invoice for reimbursement.
1.1.3.1 Fuzz Box Collection The contractor shall provide service to the “Fuzz Box” compactors located at buildings 10828 and 20306. Product in these containers is a finely shredded paper that is no longer recyclable. These containers shall be taken to a separate dumping site from the landfill which possesses a license to dump such product
1.1.4. Inclement Weather Schedule. The contractor shall collect MSW and recyclable material during periods of inclement weather. In cases of severe weather, the CO or COR may authorize exceptions. When exceptions are granted all missed collections shall be performed within 24 hours after the severe weather has terminated, unless the CO authorizes additional time.
1.1.5. Route Parameters. The contractor shall propose routes to the base through State Route 444, at the West Ramp/Commercial Delivery Gate (Gate 26A), Headquarters (Gate 12A), Skeel Ave. (Gate 15A), Main Gate (1A), National Road (Gate 19B), and Springfield Street (Gate 1B), to o the CO for acceptance. These base gates may close unpredictably due to Force Protection (security levels) conditions at any given time. The Contractor shall make adjustments to their schedule as the situation dictates. The Commercial Delivery Gate is Gate 26A, and the location is off State Route 444. Times required to complete the inspection process can be unpredictable, the contractor should be prepared for possible delays.
1.1.6. Points of Collection. Collection stations for MSW and recyclable materials are shown in Appendix A, Table A-1.1. The contractor shall position bulk containers for customer ease in depositing MSW. The contractor shall also position containers to minimize interference with adjacent parking areas, sidewalks, roadways, overhead utilities, trees, storm drains, and other potential obstructions.
1.1.6.1. MFH Collection Stations. (Appendix B, Table B-2) for refuse and recyclable materials at their permanent location with 89 brick quarter housing units, one farm house, and 10 housing units on Yount Drive shall be returned to their original location after emptying.
Also, collection of refuse at their permanent location at the public/common area shall be emptied.
Pickup shall be accomplished one per week.
1.1.6.2. On-Call Collection (Compactors and Front Load). Compactors listed as on-call in Appendix A, Table A-1 shall be picked up within twenty-four (24) hours of notification by the COR. Each compactor shall be cleaned inside and outside at an off-base location and return compactors within four (4) hours of the container being picked up.
1.1.7. Cubic Yard Capacity. Proposed capacity of containers is indicated in Appendix A, Table A-1. The contractor is encouraged to identify changes to the capacity of containers and/or pickup frequency to improve the overall efficiency of MSW and (recycling removal, MFH), and to submit these changes to the CO or COR. Proposed changes shall be coordinated with the CO/COR.
1.1.8. Government-Approved Containers. Collection of MSW and recyclable materials in all areas except residential areas shall be from Government-provided, CO or COR accepted containers, recycling bins or plastic bags.
1.1.8.1. MFH Refuse Containers. On an as needed basis, the contractor shall provide all curbside containers, consisting of refuse containers: 96 gallon capacity wheeled containers with lid, meeting the base architectural/appearance standards, and recycling containers that shall be 96 gallon containers, with wheels and lids, and be clearly discernable from the refuse curbside containers. The contractor shall pick up debris including grass clippings and tree branches placed at the collection stations in boxes, cans, tied bundles, or other containers in the same manner as if it were a specified container, provided no single item exceed six (6) feet in length. Collection at the public/common area shall normally be from 36 gallon metal containers with plastic inserts
Upon emptying these containers, the contractor shall line the inserts with a leak proof 1.01 Mil X 30 Gallon capacity plastic trash bag. Contractor shall report to COR when a container needs replaced, and COR will validate that replacement is needed.
1.1.9. Maintaining Containers and Collection Area. The Contractor shall return all MSW and recycling containers to their original location after servicing, in an upright position with the lids securely in place (closed). The Contractor is responsible for cleaning up any spills, debris, etc., on the ground within 10 feet of the container when servicing a location. The contractor shall collect debris placed at collection stations; for example, tree branches, sacks, cartons, boxes, cans, tied bundles, or other containers. It is intended that the majority of debris will be placed in containers. The Contractor shall notify the COR of locations where debris is frequently placed outside of containers so the COR may notify facility managers and, if necessary, the Contractor can adjust the container size or pickup frequency with CO or COR approval. The contractor shall also notify the COR if it is discovered that a particular receptacle is being utilized for the wrong material. The contractor shall also position containers to minimize interference with adjacent parking areas, sidewalks, roadways, overhead utilities, and other potential obstructions.
1.1.9.1 Apparent Serviceable Material. The contractor shall notify the COR within two (2) hours of any materials which have obvious value and appear to have been inadvertently placed near the collection point that should not be disposed of, unless designated as refuse by the COR.
If the contractor is in doubt as to the value of the material, the contractor shall check with the COR. The contractor shall be liable for intentionally disposing of serviceable items or material.
1.1.10. Vehicle Weighing. All vehicles used to collect MSW, C&D, organics and recyclable materials shall be weighed on state-certified public scales/weigh station, on base. When use of off-base scales are the only option, a predetermined public state-certified scale/weight station will be identified for use. The COR may be present at the public scale for empty and loaded weighing with no notice to the Contractor. All weight discrepancies will be resolved prior to departing the scale site. The Contractor and COR (if present) shall sign all weight tickets. The Contractor shall submit a monthly report (PWS para 1.5) that includes weight tickets that reflect the disposed tonnage, disposal date, vehicle type, and landfill tipping fee cost.
The Contractor shall be reimbursed for tipping fees according to submitted invoices. Wright Patterson’s Weigh Station is located at the Recycling Center at Building 10293 in Area A.
1.1.11. Missed Pickups. The COR must be notified in writing and by phone if a schedule picked up was missed. Refuse at missed pickup points shall be collected at no additional cost to the government within two (2) working hours from notification by the CO/COR.
1.2. RECYCLABLE MATERIALS PROCESSING. The base (WPAFB), operates a Qualified Recycling Program (QRP) to reduce the volume of non-hazardous solid waste generated. It is the intent of the base to conduct direct sales of recyclable commodities through a QRP. The USAF SSPP can be found at http://www.safie.hq.af.mil/esoh/. For these services, the contractor will not sell any recyclable commodities. All commodities on this contract are picked up and delivered to Bldg. 10293 the Wright Patterson Recycling facility, for WPABF to sell.
1.2.1. Materials. Recyclable items shall be collected and transported using 30 yard open top containers. All recyclable items will be transported to the Base Recycling Center, Building 293 located in Area A. The following items shall be recycled: metal, cardboard, and wood.
The contractor may collect and transport materials not listed above with prior coordination of the CO or COR. All recyclable items shall be stored, shipped and disposed of in accordance with federal and local environmental regulations.
1.2.2. Pickup Points. Contractor shall provide recycling containers at the locations specified in Appendix B, Table B-2. Containers shall be emptied as specified in section 1.1.2.2.
1.2.3. Base Recycling Center. The government will provide the contractor use of the base recycling center as shown in Appendix B-1 to be used as the drop off point for all recycling on base.
1.3 DISPOSAL
1.3.1. Off-Base Disposal. The contractor shall transport and dispose of all municipal solid waste at a permitted off-base disposal site. A copy of the landfill(s) operating permit material is transported shall be provided to the CO. The contractor shall transport recyclable commodities from Military Family Housing (MFH) to an off-base material recovery facility. The contractor is responsible for disposal being in accordance with existing local, state, and federal regulations. The contractor shall be responsible for any permits or fees associated with the use of off-base disposal locations and invoice according to the applicable
1.3.1.2 On–Call Collection (Asbestos). Asbestos shall be transported in a government supplied covered container. Asbestos shall be buried in an Ohio EPA landfill. A certified copy of the landfill permit for asbestos disposal shall be submitted prior to award of the contract. Asbestos removal shall be coordinated through the office of Environmental Management WPAFB
(937) 257-5531. Asbestos container shall be picked up, emptied, and returned within 24 hours of notification by the COR.
1.4. EQUIPMENT MAINTENANCE. The Contractor shall maintain all Contractor and Government provided containers so that they have a professional appearance and ensure they are free of unpleasant odors, dirt, debris, and pests.
All containers must remain in good, workable condition, with no leaks. They must remain easily accessible to customers.
The Contractor shall perform all cleaning, repair and other maintenance tasks off-base. Trucks and trailers used for hauling and collecting shall be free of odor, dirt, debris, and pests. Contractor vehicles and equipment shall not have any waste or equipment leaks such as oil or hydraulic fluid.
Should any contractor vehicles or equipment have any leaks, the contractor is responsible for cleanup.
1.4.1. Maintenance and Repair of Government Furnished Containers. An original inventory will be done by the government and given to the contractor upon award of contract. By the end of the first month, a joint inventory between the COR and contractor will be completed. The contractor shall provide an updated inventory report, which will list all broken or damaged government containers to the COR. The COR shall then designate the containers as (1) condemned, (2) serviceable, or (3) repairable. All broken or damaged government containers identified shall be repaired by contractor at the government's expense. The contractor shall provide documentation showing that the repair costs are fair and reasonable.
1.4.2. Bulk Container Cleaning. The contractor shall maintain bulk containers to ensure they are free of unpleasant odors, dirt, debris, and pests. Cleaning should include thoroughly washing containers with steam, soap or detergents and water. Bulk containers used for food waste shall be cleaned at least monthly
1.4.3. Other Municipal Solid Waste Container Cleaning-MFH. Smaller refuse (e.g., 15 through 36 gallon containers at the common area (Turtle Pond) shall be maintained free of unpleasant odor, dirt, debris, and pests. Cleaning of MFH containers is not a contract requirement.
1.4.4. Condemned Containers. Condemned containers shall be removed from service and replaced with serviceable government-furnished containers within 24 hours of notification by the COR. The replacement container shall be placed at the collection station at the same time the condemned container is removed. Containers designated as condemned shall be emptied and transported to the Recycling Center uilding 10293 in Area A.
1.4.5. Repairable Containers. Repairable container shall be removed from service and replaced with a serviceable government-furnished container within 24 hours of notification by the COR. The substitute container shall be placed at the collection station at the same time a container is removed.
1.4.5.1. The contractor shall repair containers at the expense of the government, and return containers to original location. Repair shall be completed within seven (7) working days from the time of notification by the COR. Verbal estimate for the repairs shall be provided within 24 hours of a notification by the COR. A follow-up written estimate of repair shall be submitted to the COR within 72 hours of the original request.
1.4.5.2. All damaged repairable containers not noted on the original or joint inventory and determined by the COR to be caused by contractor abuse, negligence, or misuse, shall be repaired at the expense of the contractor.
1.4.5.3. Repairable Compactors. The COR will be notified if there are any compactors that need repaired. The government will be responsible to tag compactors using appropriate AF Tags. Upon approval from the COR, the contractor will be responsible to ensure Lockout/Tag Out procedures are performed while compactors are being serviced.
1.4.6. Preventive Maintenance-Government Provided Compactors. The contractor shall implement a planned preventive maintenance program, consistent with manufacturer’s intervals and specifications, IAW ANSI Standard No. Z245.2, and OSHA 29 CFR, Part 1910.147. A preventative maintenance program will help prevent and correct deficiencies with compactors to minimize breakdowns and service interruptions, extending equipment life, and maximizing operating efficiency. The contractor shall notify the COR before proceeding with any preventive maintenance, and advise the COR after their initial inspection of any supplies or parts required in addition to oil, belts, and filters to justify and obtain government reimbursement. At the contractor’s conclusion of such maintenance the contractor shall, provide the COR with documentation showing the supplies and parts used for each PM, and the additional costs if applicable. The Contractor shall keep accurate records of all preventive maintenance performed, and make those records available to the COR at any time requested. Contractor shall provide the manufacturer’s recommended performance schedule for each model compactor to the COR at the start of the contract.
1.5. REPORTS AND RECORDS. The Contractor shall provide a monthly report, by working day of the month, detailing the previous months total contract costs incurred, cumulative weight tickets for total tonnage of solid waste collected (MSW, C&D, organic, and recycling after disposal for landfill), tipping fees, weight tickets, in a way that differentiates MFH costs, costs, Hospital, and AF Museum costs. ALL RECYCLABLE SALES RECEIPTS must accompany the report. Format for these monthly reports shall be coordinated with the CO or COR prior to the start of the contract
1.5.1 MFH Refuse/Recycling. The Contractor shall provide weight slips for each load (full, three-quarters, one-half, or one-quarter) carried on each trip in order to calculate the generation fee for Greene County. Weight slips shall be in accordance with CDRL A001.
1.5.1.1. MFH Weight Slips. The Contractor shall have all vehicles used in collection of recyclable material weighed prior to and after the end of each collection run. Incoming and outgoing vehicle weights shall be recorded for each vehicle processed. The Contractor shall compile the data, convert load size to tons, tally weight slips and submit to the COR, showing total tons. Weight slips must clearly differentiate MSW from MFH loads. The generation fee report shall be submitted to the COR no later than the twelfth (12) working day of the month, following the month in which the work was completed.
1.5.1.2 Tonnage Conversion. To convert load size to tons, the Contractor may establish a conversion factor (so many tons per load) for purposes of reporting. The contractor shall verify and appropriately adjust this conversion factor at least once every three (3) months.
The contractor’s method of arriving at this conversion factor shall be subject to review and acceptance by the COR.
1.5. Documentation and Records. All documentation, records, schedules, as described in this PWS, that the Contractor provides, becomes the property of the Government and shall be turned over to the Government at completion of the contract. The Contractor shall keep those items current at all times. If requested by the Government, original documentation and records shall be available within two (2) business days.
2. SERVICE SUMMARY. The contract service requirements are summarized in performance objectives that relate directly to mission essential items. The performance threshold briefly describes the minimally acceptable levels of service required for each requirement.
The Services Summary (SS) and the Contractor’s Quality Control Plan provide information on contract requirements, the expected level of Contractor performance, and the expected method of government validation and confirmation of services provided. These thresholds are critical to mission success. Procedures as set forth in the FAR 52.212-4 (a), Contract Terms and Conditions - Commercial Items, Inspection/Acceptance, will be used to remedy all deficiencies. During the first initial 30 days of the contract, two additional errors on each Performance Objective shall be allowed in an effort to identify normal phase-in problems.

2.1. QUALITY CONTROL
The contractor shall develop and maintain a quality program to ensure integrated solid waste management is performed in accordance with commonly accepted commercial practices.
The contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. At any time during contract performance, the government has the right to require revisions of the Quality Control Plan at no cost to the government should the incorporated plan fail to control the quality of service provided. The plan shall include, but is not limited to the following:
a. A description of the inspection system, addressing all services listed in Service Summary (SS).
b. Frequency of inspections.
c. Title of the individual(s) who shall perform the inspections and their organizational placement.
d. A description of methods for identifying, correcting and preventing defects in the quality of service performed before the level becomes unacceptable.
e. On-site records of all inspections conducted by contractor. The inspection records format shall include:
1. Date, time and location of inspection.
2. Signature block for the person who performed the inspection.
3. Rating of acceptable or unacceptable.
4. Area designated for deficiencies noted and corrective action taken.
5. Total number of inspections.
As a minimum the contractor shall develop quality control procedures addressing the areas identified in paragraph 2, Service Summary.
2.2 QUALITY ASSURANCE
The Government shall inspect and evaluate the Contractor’s performance to ensure services are received in accordance with requirements set forth in this PWS. The contractor shall provide work schedules to the COR in accordance with paragraph 4. .1 of this PWS. The COR shall inspect by validating actual work performance, physically checking an attribute of the completed task, checking a management information report, investigating customer complaints, conferring with facility managers, or otherwise inspecting the task or its results to determine whether or not performance meets the standards contained in this PWS. The COR will use the Contractor’s work schedule or modified version thereof, to record validation results. Results of the validation then become the official Air Force record of the Contractor’s performance. When a performance threshold has not been met or Contractor performance has not been accomplished, the COR will initiate and provide the CO a Corrective Action Report (CAR) DD Form 2772 for issuance to the Contractor. The Contractor shall respond to the CAR IAW instructions provided and return it to the CO within calendar days of receipt.
2.3. PERFORMANCE ASSESSMENT
2.3.1. Periodic Validations. This method requires the COR to employ a “spot check” style of evaluation based on the Contractor’s schedule. Periodic validations will be conducted on a scheduled basis (daily, weekly, monthly, quarterly, semi-annual or annually) and may be adjusted, based on quality trends.
2.3.2. Unsatisfactory Performance. Any unsatisfactory inspection (defect) result shall be recorded, and the Contractor shall re-perform the service after notification by the COR. Failing to meet the performance threshold as outlined in the SS for any of these performance objectives in any one month period shall result in a warning or letter of concern from the CO. Failing to meet the performance threshold as outlined in the SS of these performance objectives in any combination for any two, or more consecutive or non-consecutive months during a contract period shall constitute an immediate Progress Meeting with the Multi-functional Team. All remedies shall be in accordance with the FAR 52.212-4 (a), Contract Terms and Conditions - Commercial Items, Inspection/Acceptance.
2.3.3. Customer Complaints. The COR will receive and investigate complaints. The COR shall be responsible for initially validating customer complaints. The CO shall make final determination of the validity of customer complaint(s) in cases of disagreement with the COR and the Contractor. Validated customer complaints shall be re-performed after notification by the CO or COR. Exceeding the performance threshold for performance objective in any one month period may result in a warning or letter of concern from the CO.
2.4. PERIODIC PROGRESS MEETINGS
2.4.1. Meetings. The CO, Functional Commander, COR, QRP Manager and/or Solid Waste Manager, other government personnel as appropriate, and the Contractor shall periodically meet to discuss the Contractor’s performance. The following issues shall be discussed: opportunities to improve the contract, required contract modification(s), unsatisfactory inspections and trends against each performance objective observed, positive performance and steps taken by the Contractor to prevent unsatisfactory occurrences in the future. The Contractor shall provide a summation of unsatisfactory inspections and customer complaints and provide insight into any identified trends.
2.4.2. Meeting Minutes. The minutes of these meetings will be reduced to writing, signed by the CO, and any other signatures as deemed appropriate, distributed to the functional area and the Contractor. Should the Contractor not concur with the minutes, the Contractor shall provide a written notification to the CO identifying areas of non-concurrence for resolution.
3. GOVERNMENT FURNISHED PROPERTY AND SERVICES.
3.1. Government-Approved Containers. Collection of refuse shall be from government-provided, government-approved containers. The government shall provide without cost, containers, equipment, materials and services listed herein or in Appendix C.
3.1.1. mergency Medical Service. The government shall provide emergency medical treatment and emergency patient transportation service for contractor personnel who are injured or become critically ill during the performance of this contract. The contractor shall reimburse the government the cost of medical treatment and patient transportation service at the current inpatient or outpatient treatment rate, as appropriate.
3.1.2. Security Police and Fire Protection. The government shall provide general on-base security police service, fire prevention and protection, inspection and maintenance of government-furnished fire extinguishers and systems, pest control, and grounds maintenance. While on WPAFB for Emergencies, call 257-9111 for Fire Department and Security Police support. For WPAFB Fire Department routine support call 257-3033. The Security Police routine call number is 257-6516.
4. GENERAL INFORMATION.
4.1. MISSION
The overall Civil Engineer mission at Wright Patterson AFB is to plan, maintain, operate, and protect the infrastructure, facilities, and environment for assigned Airmen, families, and civilian employees.
4.2. HOURS OF OPERATION. Perform integrated solid waste management services during normal duty hours and be available (by telephone, cell phone, text message) during the Government Program Office’s hours (0 :00 a.m. to 05:00 p.m., Monday through Friday) and for after-hours response to refuse/recycling problems or emergencies. This excludes federal holidays, unless otherwise required in the contract or approved by the CO. There may be situations that require the Contractor to work at times other than normal duty hours. When the Contractor’s work schedule conflicts with an Air Force mission requirement, advise the CO and reschedule the work to minimize disruption.
. Military Family Housing (Brick Quarters) and Turtle Pond. The Contractor shall perform refuse and recycling collection required under this contract during the following hours: 7:30 a.m. to 5:00 p.m., Monday through Friday, except Federal holidays. The Contractor may work, with prior approval of the CO, CA or COR, extended hours to ensure timely completion of work at no additional cost to the Government.
4.4. HOLIDAYS & CLOSURES
4.4.1. Recognized Federal Holidays. There may be federal holidays when the Contractor is not required to perform service, except in emergency situations or to support specific mission needs.
The holidays are:
New Year’s Day - 1 January Martin Luther King Day - 3rd Monday in January Washington’s Birthday - 3rd Monday in February Memorial Day - last Monday in May
Independence Day - 4 July Labor Day - 1st Monday in September Columbus Day - 2nd Monday in October

If a holiday falls on a Saturday, the preceding Friday will be observed. If a holiday falls on Sunday, the following Monday will be observed. If a holiday falls on a scheduled service day, the Contractor will be responsible for rescheduling services for the first day after the holiday observance.
4.4.2. Other Base Closures. Work scheduled but not accomplished because of base closure to exercises or real-world events will be accomplished as soon as possible after base reopening.
4.5. PERFORMANCE OF SERVICE DURING CRISIS OR HEIGHTENED
SECURITY The services under this contract are essential for performance during crisis. The CO will determine

4. .1. Schedule Submission and Approval. The Contractor shall submit a schedule for all services described in this PWS and Appendices to the CO for approval at the post-award conference. The schedule shall include routes, pickup points, times, and days that pickups will occur. The Contractor shall not deviate from the approved schedule without prior approval from the CO or COR. Permanent changes to the schedule must be submitted 10 days before implementation and receive CO approval before the Government will allow the proposed changes. The schedule must be submitted electronically using a file format compatible with Government software.
4. .2. Schedule Changes. Occurrence based (non-permanent) schedule changes due to customer requests or base exercises shall be submitted to the COR and do not require CO approval.
4. . CONTRACT PERSONNEL
4. .1. Contract Manager. The contractor shall establish and maintain an office through which the contract manager or alternate(s) can be contacted during work hours. The contract manager or alternate shall be available during normal duty hours to meet on the installation within 1 hour with the government personnel designated by the CO to discuss problem areas.
The contractor shall provide the CO telephone number(s) where validation results and complaints can be reported. The contractor shall also provide to the CO the names and phone numbers of contractor POCs for after business hours including nights, weekends, and holidays.
This information will be kept updated by the contractor whenever personnel changes occur.
The contract manager or alternate shall have full authority to act for the contractor on all contract matters relating to the daily operation of this contract.
4. .2 Personnel. Contractor personnel shall present a neat appearance. Contractor personnel shall be easily recognizable while on the installation in conjunction with this contract.
This shall be accomplished through the wear of distinctive clothing, overcoats, or hats, bearing the company name or logo. The coloring or design of the items selected should be such that identifies personnel easily and quickly for reasons of safety and personal protection.
4. .3. Employee Restrictions. The government is authorized to restrict employment under the contract of any contractor employee or prospective contractor employee who is identified as a potential threat to the health, safety, security, general well-being, or operational mission of the installation and its population.
4. . CONTRACTOR MANPOWER REPORTING
he Contractor shall report all contractor labor hours (including sub-contractor labor hours) required for performance of services provided under this contract for “U.S. Air Force” via a secure data collection site. 4. . SECURITY REQUIREMENTS. Contractor employees shall comply with Wright Patterson AFB security requirements imposed by the Installation Commander at all times while on Wright Patterson AFB premises. The Contractor shall obtain all necessary passes, decals, badges, or other items required for access to perform contract services at Wright Patterson AFB.
The Contractor is responsible for accountability of all such media, which shall be surrendered to the Security Forces Squadron Pass and Registration Section upon completion or termination of the contract or upon termination of an individual’s employment for the Contractor. The Contractor shall comply with all Wright Patterson AFB policies regarding entry to the installation, rules of the road, contingency operations compliance, and photography prohibitions.
Contractor employees are authorized entry to the installation for the purpose of work under this contract only, and are not authorized in other facilities or areas without prior government authorization. Due to safety/security requirements, Contractor personnel shall be required to fluently speak, comprehend, read, and write English.
4. .1. Base Access. Contractor/Employee Base Pass and Identification, Special Clearances and Vehicle Passes. The contractor shall comply with all requirements and procedures IAW FAR 5352.242-9000, Contractor Access to Air Force Installations. All documentation shall be submitted at the pre-performance conference.
4. .1.1. The Contractor shall complete a Request for Identification Credential for each employee of the contractor requiring access to Wright Patterson Air Force Base. The request shall be submitted to Pass and Registration (Bldg. 10286).
The government shall provide a completed Identification Credential (AFMC Form 387), which shall be issued, displayed, and surrendered as directed in AFI 31-101. Also DD form 254 (Contract Security Classification Specification) shall be filled out by vendor (attachment to solicitation) and returned to the CO/COR for access to controlled areas at Wright-Patterson Air Force Base.
4. .1.2. The Contractor must obtain favorable National Agency Check (NAC ) investigations for its employees who require entrance to restricted areas. The Wright-Patterson Air Force Base Security Police shall be the point of contact for the NAC .
The NAC shall be obtained through the Defense Investigative Center, P.O. Box 1083, Baltimore, MD 21203-1083, and shall provide inquiries on employee’s background worldwide. (Note:
Completion of a NAC requires approximately to nine months. A follow-up on the NAC shall be performed every 60 days until completed. Proof of follow-up shall be forwarded to the CO/
COR).
4. .1.3. All contractor employees shall comply with all security requirements imposed by the local commander at all times while his employees are on the installation and shall follow instructions of the local organizational commander pertaining to security.
4. .1.4. Contractor shall provide adequate training and instruction for personnel to perform designated tasks. Flight-line driver training shall be required for personnel who must drive on flight-line. Training is coordinated through Base Operations (937-257-6206) on the 2nd floor of 30206 (Area A) the 1st and 3rd Wednesday of each Month.
4. .1.5. The contractor shall safeguard all government property provided for contractor use. At the end of each work period, government facilities, property, equipment and materials shall be secure. Any damage caused by contractor personnel to the building, finishes, furnishings, equipment, etc., the contractor shall repair, clean, replace, or restore damaged items to the condition prior to the item being damaged. The contractor shall conform to the provisions of AFI 31-101, Integrated Defense, 08 Oct 09, for safeguarding the government-furnished facilities and material contained therein.
4. .2. Main Base Access. Current security access restrictions at the Commercial Delivery Gate requires all contractor vehicles (which are the size of a standard bread delivery truck or larger) to enter through (Gate 26A). Contractor vehicles are subject to search prior to gaining base entry.
The Contractor should plan and allow for delays as a result of increased and ever-changing security procedures. Times required to complete the inspection process can be unpredictable, the contractor should be prepared for possible delays.
4. .3. Security Manager. Upon contract award, the Contractor shall designate, in writing to CO, a Security Manager who shall be the Contractor’s point of contact for all security matters.
Within 5 working days of contract award, the Contractor’s Security Manager shall contact the Civil Engineer Squadron (CES) Security Manager for assistance on accomplishing Contractor personnel security requirements.
4. .3.1. Security Training. Contractor personnel are required to attend security and OPSEC orientation under Homeland Security Presidential Directive (HSPD) Title 12. The Contractor shall schedule initial and annual refresher training (for all assigned contractor personnel) through the CES Security Manager. All contractor personnel shall be thoroughly briefed by the CES Security Manager on established security requirements and procedures as stipulated in Air Force Policy Directive (AFPD 31-1, Integrated Defense).
4. .3.2. Security Authority. The Contractor shall brief all employees they shall immediately leave any area upon notification by the CO, COR, constituted authority, Security Forces, or when directed as occasional operational requirements, real world activities, or emergencies dictate.
4. .4. Restricted/Controlled Areas. Contractor performance within restricted/controlled areas shall require Contractor compliance with various levels of security and established personnel security entry control procedures. The Contractor shall ensure all contractor employees performing services in secured areas are processed for a controlled area, limited access, or visitor badge by the Entry Controller of the restricted/controlled area.
4. .4.1 Access Coordination. The facility manager/user/occupant/requester will be responsible for providing access, escorts, or temporary visitor access badges for contractor personnel while performing ISWM duties. The Contractor shall be responsible for scheduling access/escorts/badges by contacting the facility manager/requester at the Entry Control Point of the restricted/controlled area.
4. . .2 Escorts. At all times while in restricted/controlled areas, contractor employees shall be under direct surveillance of a person from that facility who is cleared to wear a controlled area badge and perform escort duties. Visitor badges shall be returned to the Entry Controller each time the employee completes the duties and leaves the area. This requirement is waived for contractor personnel who have unescorted access privileges and display a controlled area badge for the restricted/controlled areas.
4. . . Employee Background Checks. All Contractor employees who do not have a current security investigation acceptable to the Security Forces, and who require a National Agency Check NAC ) for the performance of their duties, shall complete an Electronic Personnel Security Questionnaire (EPSQ), SF 85P, Questionnaire for Public Trust Positions, in three copies (original and two photocopies); and an AF Form 2583, Request for Personnel Security Action.
