About this file

This is a Foreign Country of Concern Attestation Form (PUR 1355) required by the Florida Department of Transportation for the District Seven Storm Drain Video Inspection Services procurement. The attestation form must be completed and signed by an officer or representative of any entity submitting a bid, proposal, or entering into a contract with a governmental entity that would grant access to personal identifying information. Bidders must attest under penalties of perjury that their entity is not owned by the government of a Foreign Country of Concern, is not organized under the laws of a Foreign Country of Concern, does not have its Principal Place of Business in a Foreign Country of Concern, and that the government of a Foreign Country of Concern does not have a controlling interest in the entity. All prospective bidders must be registered in Florida's MyFloridaMarketplace system, and out-of-state corporations, limited liability companies, and limited partnerships must be authorized to do business in Florida. Additionally, bidders must maintain proper licensure as required by the Florida Department of Business and Professional Regulation if applicable to their business operations.

The attestation requirement is a compliance and eligibility threshold that does not directly impact pricing, set-asides, or contract terms but serves as a mandatory certification for participation in the procurement process. Respondents are subject to communication restrictions during the 72-hour period following the agency's posting of the notice of intended award, excluding weekends and state holidays, with violations potentially resulting in rejection of the response. All payments to vendors resulting from this competitive solicitation are subject to MyFloridaMarketplace transaction fees.
