About this file
This is a Public Contract Code Disclosure form required by the State of California for bidders on public works contracts with the City of Pasadena. The document establishes mandatory certification requirements under California Public Contract Code Sections 10285.1, 10162, and 10232 that all bidders must complete under penalty of perjury prior to bid submission. Bidders must declare whether they have been convicted within the preceding three years of fraud, bribery, collusion, conspiracy, or violations of state or federal antitrust laws in connection with public works contracts. Additionally, bidders must disclose whether they or their officers or employees with proprietary interests have ever been disqualified, removed, or prevented from bidding on or completing federal, state, or local government projects due to violations of law or safety regulations. The form also requires contractors to certify that no more than one final unappealable finding of contempt of court by a federal court has been issued against them within the immediately preceding two-year period for failure to comply with National Labor Relations Board orders.
All certifications require the bidder's signature, printed name and title, and date. Bidders are advised that false certifications may result in criminal prosecution. The form serves as a binding part of the bid submission for the City of Pasadena's Design-Build Services for a Hydrogen Fueling Station project, which involves designing, building, operating, and maintaining a stand-alone hydrogen-fueling station to support the city's fleet of fuel cell electric buses. This disclosure ensures compliance with state law and establishes the legal and ethical qualifications required of contractors bidding on California public works projects.
