Performance Requirements Summary
The contractor service requirements are summarized into performance objectives that relate directly to mission essential items. The performance threshold briefly describes the minimum acceptable levels of service required for each requirement. These thresholds are critical to mission success.

5.1 The contractor shall
perform all required preventative maintenance procedures
IAW the manual, Spectral Dynamics

Maintenance Program for Multi-Shaker
System.
100% Incentive: Consistently meeting the
100% AQL will be documented as
"Satisfactory" in CPARS. Proactively identifying and reporting potential equipment issues not covered in the standard PM, with recommendations, will be noted as "Very Good" or
"Exceptional," strongly supporting the exercise of option years.
Disincentive: Failure to meet the AQL will require re-performance at no cost to the government. This will be documented as "Marginal" or
"Unsatisfactory" in CPARS. Repeated failures may result in the issuance of a
Cure Notice and jeopardize future option year considerations.
The contractor shall provide only qualified experienced
Spectral Dynamics Shaker
System Technician to perform disassembly and assembly of the shaker system and to perform the required service test, inspections and services as required by the preventative maintenance program.
5.1.1 Provide qualified and
experienced technicians for all work.
100% Incentive: Performance by qualified technicians will be noted as
"Satisfactory." If the contractor's technicians demonstrate exceptional skill that leads to faster-than-expected service or early fault detection, this will be recorded as "Exceptional" in the CPARS
Quality and Personnel ratings.
Disincentive: Use of an unqualified technician is a significant failure and will result in an immediate
"Unsatisfactory" CPARS rating. The government may issue a stop-work order until a qualified technician is present, and all work must be re-certified at the contractor's expense.
The contractor shall perform the preventative maintenance and service after 1000 hours of operation or six (6) months whichever comes first.
5.1.2 The contractor shall
perform all required preventative maintenance procedures

100% Incentive: On-time performance will be rated "Satisfactory" under the Schedule portion of CPARS. Consistently scheduling and completing maintenance well in advance of the deadline demonstrates superior planning and will be noted as "Very Good."
Disincentive: Failure to perform maintenance within the required window will be rated as "Marginal" in CPARS.
Significant or repeated delays will be rated "Unsatisfactory" and will negatively influence the government's decision to exercise future option years.
The contractor shall provide all parts and materials required to perform the required preventative maintenance. The parts include but not limited to all filters, current leads, hoses, clamps, oils, lubricants, fluids, pumps, wires, seals, and connectors.
The contractor shall provide all purchased spare parts per contract list.
5.1.3 The contractor shall
provide all required parts and materials as required IAW the manual, Spectral
Dynamics Preventative
Maintenance Program for Multi-Shaker
System.
100% Incentive: If the contractor identifies and uses an alternative part that provides equal or better performance at a lower cost, the contractor will receive 50% of the cost savings.
Disincentive: A deduction from the invoice equal to the cost of any missing parts, plus a 10% administrative fee. The government may procure the missing parts and charge the cost to the contractor

100% Incentive: Timely and accurate submission will be rated "Satisfactory."
Submitting flawlessly accurate reports ahead of schedule will be noted as "Very
Good" for administrative and reporting compliance in CPARS.
Disincentive: Inaccurate or late submission of the inventory list reflects poor administrative control and will be documented as "Marginal" in CPARS.
The contractor will be required to correct and resubmit the report until it meets the 100% AQL.
Acceptable surveillance methods include:
100 Percent Inspection: With this method, performance is inspected/evaluated at each occurrence. This method is often costly but can be necessary due to health, safety and other considerations.
Random Sampling: Random sampling works best when the number of instances of the services being performed is very large and a statistically valid sample can be obtained. http://www.sqconline.com/mil-std-105.html (web link to

Periodic Inspection: This method, sometimes called “planned sampling,” consists planned sampling at specific dates or time intervals. It may be appropriate for tasks that occur infrequently, and where 100 percent inspection or random testing is not desired.
Customer Input: Although usually not a primary method, this is a valuable supplement to more systematic methods.
For example, in a case where random sampling indicates unsatisfactory service, customer complaints can be used as substantiating evidence. In certain situations where customers can be relied upon to complain consistently when the quality of performance is poor, e.g., dining facilities, building services, customer surveys and customer complaints may be a primary surveillance method, and customer satisfaction an appropriate performance standard.
In all cases, complaints should be documented, preferably on a standard form.
STATISTICAL SAMPLING
Since the cancellation MIL-STD-105E, the availability of statistically sound, sampling tables to personnel performing in-process and end item product audits has been scarce. Most people performing these audits today are still using tables from this canceled document, or use Contractor's Sampling Tables. The table below is approved for use by DCMA QA personnel performing zero-based sampling. If no AQL is contractually specified, an AQL of 1.0% is suggested.

1. ZERO ACCEPTANCE NUMBER SAMPLING PLANS fith edition by Nicholas L. Squeglia.
(ASQ.ORG provides many useful tools for the Quality toolbox. Must be a member to enjoy the discounts.) This revised edition emphasizes the use of the updated military standard, MIL-STD-105E, as a guide in developing inspection strategies. A small lot sampling plan has also been added. These plans have found wide use in many industries where lot-by-lot attribute sampling exists, regardless of product.. This document is made available by ASQC, Quality Press, 611 East Wisconsin Avenue, Milwaukee, Wisconsin 53202. (About 44.00 for nonmembers, 26.00for Members)
2. MIL-STD-1916, DoD PREFERRED SAMPLING PLANS FOR ACCEPTANCE OF PRODUCT This document contains a set of statistically sound sampling plans and procedures for planning and conducting the inspection of product to assess quality and provide information of conformance to contract requirements. This new military standard complies with the Department of Defense policy of eliminating acceptable quality levels (AQLs) and associated practices. The standard is currently available, however, the handbook addressing this standard and providing clarification is still being written. Also, some of these plan may be too stringent to use, based on contract requirements.
3. Electronic Industries Association (EIA) Standard, ZERO ACCEPTANCE NUMBER SAMPLING
PROCEDURES AND TABLES FOR INSPECTION BY ATTRIBUTES BY A CONTINUOUS
MANUFACTURING PROCESS Since conventional attribute sampling plans based on nonzero acceptance are no longer desirable, this industry standard places an emphasis on zero based sampling plans as they relate to Lot Tolerance Percent Defective (LTPD) value or the limiting quality protection of MIL-STD-105. The OC curves of this document are for the most part equal to or better than their associated MIL-STD-105 curves. Remember, in order to use the Contractor's sampling plan, that plan shall afford the government equal or better protection that the requirements of the contract specifies.
