About this file

This document contains a pricing schedule for nonpersonal services to be provided by an Internal Medicine/Family Practice physician at the Chinle Comprehensive Health Care Facility. The pricing schedule outlines all-inclusive hourly rates for regular and overtime hours, as well as flat rates for on-call standby shifts on weeknights and weekends/holidays. Estimated quantities and unit prices are provided for each service item. The pricing schedule signature line is blank. Additional notes indicate the all-inclusive rate excludes holiday or overtime pay differentials and covers costs such as labor, benefits, transportation, per diem, supervision and applicable taxes. The related federal contract opportunity notice is for an Internal Medicine/Family Practice physician position needed at the Chinle Indian Health Service facility.
