About this file
This is a pricing schedule template for nonpersonal nursing services at Northern Navajo Medical Center (NNMC). The template requests an all-inclusive hourly rate for an estimated 160 hours per month of regular/overtime services, including holiday pay. The rate must encompass all labor, fringe benefits, transportation, per diem, supervision, housing, and other expenses, including all applicable federal, state, and local taxes.
The schedule is associated with a solicitation (75H71025Q00055) from Indian Health Service seeking two Inpatient Pediatric Registered Nurses for the Inpatient Pediatrics Department at NNMC in Shiprock, NM. The template includes notes specifying that the hours are estimates only and variations from estimated quantities will not constitute grounds for price adjustments. Contractors must provide their signature, date, and UEI number on the form.
