About this file
The document is a Price Appendix FAQ for a Request for Quote (RFQ) providing detailed instructions for completing pricing spreadsheets for three different discount/pricing scenarios: BPA (Blanket Purchase Agreement), ADP (non-FAMP Discount), and ADP (WAC Discount). The document outlines specific guidelines for each pricing scenario, including instructions to:
- Fill in only yellow cells in the spreadsheet, with other cells locked for editing
- Read the Formulary Notes to understand the P&T committee's decision-making process
- Understand auto-population and auto-calculation features for various pricing columns
- Submit tables for up to 10 NDCs (National Drug Codes), with options to request additional tables if needed
- Use specific indicators like red highlighting to show when a quote is not being submitted for a particular formulary category
For each pricing scenario (BPA, non-FAMP, and WAC), the instructions emphasize reading formulary notes, filling yellow cells, inputting initial prices that auto-populate subsequent rows, and calculating refunds or unit prices automatically. Vendors are advised to contact the Points of Contact (POCs) on the RFQ document if they have questions or need to submit more than 10 NDCs.
