About this file

This document is a Price Appendix FAQ for a Request for Quote (RFQ) that provides detailed guidance for submitting pricing information for pharmaceutical products. The FAQ covers three pricing methodologies: BPA (Blanket Purchase Agreement), ADP (non-FAMP Discount), and ADP (WAC Discount). For each methodology, vendors are instructed to fill in yellow cells on a spreadsheet, with specific auto-population and calculation features. Key instructions include reading formulary notes, entering prices in designated columns, and understanding how different pricing inputs (such as package size, non-FAMP price, or WAC price) will be processed. The document emphasizes that vendors should only complete tables for their specific National Drug Codes (NDCs), with a default provision of 10 NDC tables, and provides guidance on requesting additional tables if needed. If vendors do not wish to submit a quote for a specific formulary category, certain cells will highlight red to indicate this to the P&T (Pharmacy and Therapeutics) committee.
