About this file

This is a Price Appendix FAQ document providing detailed instructions for completing pricing spreadsheets for the Department of Defense Pharmacy Uniform Formulary procurement. The document covers three pricing scenarios: BPA (Blanket Purchase Agreement), ADP non-FAMP Discount, and ADP WAC Discount.

The FAQ outlines specific requirements for completing pricing tables, including filling in yellow cells for editable fields, auto-population features for package sizes and pricing calculations, and handling up to 10 NDCs (National Drug Codes) per submission. For the BPA section, vendors must enter Package Size and Price per NDC, with Price per Unit automatically calculated. The ADP non-FAMP section requires input of non-FAMP prices and Federal Ceiling Prices, with Total Calculated Refund determined by formula. The ADP WAC Discount section applies specifically to products without FCPs (like diabetic supplies) and requires WAC pricing input. For all sections, vendors indicating no quote submission will have cells highlighted in red. Additional NDC tables can be requested through the RFQ POCs if vendors need to submit more than 10 NDCs.
