About this file
This is a Price Appendix FAQ document providing detailed instructions for completing pricing spreadsheets for a Department of Defense Pharmacy Uniform Formulary solicitation. The document outlines three pricing structures: BPA (Blanket Purchase Agreement), ADP non-FAMP Discount, and ADP WAC Discount.
For each pricing structure, vendors must fill in yellow cells with NDC pricing information, with other cells locked and auto-calculating. The BPA section requires Package Size and Price per NDC, automatically calculating Price per Unit. The ADP non-FAMP section requires non-FAMP price, Federal Ceiling Price, and Additional Offered Refund percentage. The ADP WAC section is specifically for products without FCPs like diabetic supplies, requiring WAC price and WAC Discount percentage. All sections provide 10 NDC tables by default, with instructions to contact POCs if more are needed. Red highlighting indicates when a vendor chooses not to quote for specific formulary categories. The document emphasizes reviewing Formulary Notes for understanding P&T committee decision-making processes.
