About this file
This is a Price Appendix FAQ document providing detailed instructions for submitting pricing information for pharmaceutical agents under a Department of Defense Pharmacy Uniform Formulary opportunity. The document outlines three pricing submission methods: BPA (Blanket Purchase Agreement), ADP non-FAMP Discount, and ADP WAC Discount.
The FAQ explains specific requirements for completing pricing spreadsheets, including instructions for filling in yellow cells, handling NDC tables, and auto-calculation formulas. For BPA submissions, vendors must provide Package Size and Price per NDC, with Price per Unit automatically calculated. The ADP non-FAMP section requires non-FAMP pricing, Current Annual Federal Ceiling Price, and Additional offered refund percentages. The ADP WAC Discount section specifically applies to products without FCPs (like diabetic supplies) and requires WAC pricing and discount percentages. For all submission types, vendors can submit up to 10 NDCs initially, with the option to request additional NDC tables if needed. The document emphasizes that vendors should contact the RFQ POCs for any questions or if additional NDC tables are required.
