STATEMENT OF WORK (SOW)
A. BACKGROUND
This is a 5-year, firm fixed-priced, Indefinite Delivery Indefinite Quantity Contract (a Base Year with four Option Years) for Rapid Sorting, Authoritative Identification, and Specimen- Based Documentation of Exotic Wood-Feeding Beetles to Facilitate Early Detection, Eradication, and Pest Management for the upcoming field season with USDA-APHIS-PPQ.
B. SCOPE OF WORK
The Contractor shall supply all the labor, materials, safety equipment, facilities, licenses, and special permits in accordance with federal, state and local regulations. Services are required for sorting, screening, and identifying trap catches for USDA, APHIS to be able to early detect and rapidly respond to any new exotic insect find. In particular, bark beetle/wood-boring beetles and siricids collected in multi-funnel traps are largely associated with our early detection CAPS surveys. To ensure that APHIS detects a national target or other unknown exotic pest at the earliest point in a new infestation, APHIS requires expertise and trained staff to have samples rapidly sorted and identified.

TASK #1: The contractor will be responsible for performing Rapid Sorting, Authoritative Identification, and Specimen-based Documentation of Samples collected, following quality assurance policies and procedures set forth by USDA APHIS, and reporting those results to the TPOC.
DELIVERABLE #1: The contractor will submit the results to USDA APHIS PPQ Field Operations Raleigh Office, and State PPQ (see contact list) within 90 days or sooner for EBB/WBB and Sirex samples. The results will be used to confirm total number of samples processed before related invoices are approved for payment.
NOTE: No other State or PPQ Program is authorized to utilize the contract without approval from the APHIS PPQ Field Operations Raleigh Office.
C. QUALITY CONTROL (QC)
The contractor shall develop and maintain an effective pro-active quality control program to ensure services are performed in accordance with the SOW. The Contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of a defective service. The Contractor shall ensure management oversight and supervision, training to employees, error-free work, early problem detection and corrective action, increased productivity and efficiency, and superior public relations.
D. QUALITY ASSURANCE (QA)
PPQ will follow the methods of surveillance specified in the SOW or elsewhere in the contract. When an observation indicates defective performance, PPQ will submit a nonperformance report to the contract manager or representative at the contractor’s facility to initial the observation. The initialing of the observation does not necessarily constitute concurrence with the observation, only acknowledgment that he or she has been made aware of the defective performance. Any action taken by the Contracting Officer as a result of surveillance will be in accordance with the terms of this contract.
E. DELIVERY TICKETS
All sample results shall be reported to (See contact lists) by the 30th of each month in (Excel spreadsheet or Word document). The results will be used to confirm total number of samples processed before related invoices are approved for payment. The following minimum information shall accompany the results report:
1. Contract Number
2. Delivery Order Number
3. Itemized list of services furnished
4. Quantity, unit price
5. Date of services performed
F. INVOICES
The contractor shall submit a monthly itemized invoice online at www.IPP.gov by the 30th of each month for all services provided during a billing period, and for which payment has not been received.
