About this file

This is a bid package and forms attachment document issued by the California Department of Corrections and Rehabilitation (CDCR) and California Correctional Health Care Services (CCHCS) for the procurement of 50 behavioral health beds and accompanying mattresses for delivery to California Health Care Facility (CHCF) in Stockton, California. The document serves as an Invitation for Bid (IFB) and includes mandatory special terms and conditions, security requirements, and compliance certifications. The procurement requires suppliers to adhere to strict CDCR security regulations, including restrictions on clothing resembling inmate attire, compliance with Prison Rape Elimination Act (PREA) standards, vehicle gate clearance procedures, and prohibition of electronic devices on institution grounds. Suppliers must complete gate clearance checks including Department of Motor Vehicles verification, wants and warrants checks, and criminal history checks a minimum of ten working days prior to delivery or service commencement.

The bid package includes multiple compliance and certification forms that suppliers must complete, including the CalRecycle Postconsumer Recycled-Content Certification, Generative Artificial Intelligence (GenAI) Disclosure and Factsheet forms, Commercially Useful Function (CUF) Certification, Target Area Contract Preference Act (TACPA) forms, California Civil Rights Laws certification, and state payee data record forms (STD 204 and STD 205). These forms require suppliers to certify compliance with environmental, non-discrimination, and state tax reporting requirements. All suppliers entering into transactions with CDCR must provide valid federal tax identification numbers and complete all required documentation under penalty of perjury. The document emphasizes that suppliers and their representatives must comply with all institutional security policies and procedures while on CDCR facility grounds.

To be completed by the State agency

State Agency:

Purchasing Agent: PO #:

Phone: E-mail:

The State Agency Buy Recycled Campaign (SABRC) is a state mandated program that requires the reporting of all purchases made within 16 specified product categories. All state agencies are required to verify the recycled-content of all products purchased within each of these categories.

All businesses shall certify the minimum percentage in writing to the contracting officer or his or her representative, if not the exact percentage, of postconsumer recycled-content (PCRC) material in the products, materials, goods, or supplies offered or sold to the State regardless of whether the product meets the minimum content requirements specified in law (see reverse side). The certification shall be furnished under penalty of perjury. The certification shall be provided regardless of content, even if the product contains no recycled material. A State agency may waive the certification requirement if the percentage of postconsumer material in the products, materials, goods, or supplies can be verified in a written advertisement, including, but not limited to, a product label, a catalog, or a manufacturer or vendor Internet website.
