About this file

This document is a Massachusetts Substitute Form W-9 (Revised April 2022) used for requesting taxpayer identification number and certification for vendors doing business with the Commonwealth of Massachusetts. The form is designed for businesses to provide their tax identification information, including options to specify federal tax classification (such as individual/sole proprietor, C Corporation, S Corporation, Partnership, or Trust/estate), and to certify their taxpayer identification number and status regarding backup withholding.

The form requires vendors to provide key details including their legal business name, address, taxpayer identification number (either Social Security Number or Employer Identification Number), and includes specific requirements for vendors receiving federal funds. As of April 4, 2022, vendors receiving federal grant funds must submit a Unique Entity Identifier (SAM) registered in the System of Awards Management. The form also includes a certification section where the business confirms the accuracy of their tax information and attests to their compliance with IRS reporting requirements.
