2. Description of Action: The Lebanon VAMC has a new brand name only requirement for the purchase of 9 Codonic Brand SLS 550I Printers.

4. Description of Supplies or Services:

The estimated value of the proposed action is $85,526.00

The Codonics Safe Label System (SLS) 550i printers are medical label printers that are compatible with and will mount on the medical center’s Pyxis Workstations and Supply Dispensing Cabinets. The Codonics Safe Label System communicates with the Pyxis Workstations to confirm the drug removed from the Pyxis against the medical center’s formulary, provide a visual and audible confirmation of the drug name and concentration, allows for dilutions and then prints a full-color compliant lable for the clinician to attach to the medication.

The following are the minimum equipment and accessories required, Codonics (Brand Name) Safe Label System 550i Printer:

5. Authority:

Order Against FSS: Authority of the Multiple Award Schedule Program, Title III of the Federal Property and Administrative Services Act of 1949 (41 USC 251, et seq.); Title 40 USC 501, Services for Executive Agencies; and Section 803 of the National Defense Authorization Act of 2002 (PL 107-107) and implemented by FAR 8.405-6. Mark the applicable exception:

An urgent and compelling need exists, and following the procedures would result in unacceptable delays(FAR 8.405-6(a)(1)(i)(A);

Only one source is capable of providing the supplies or services required at the level of quality required because the supplies or services are unique or highly specialized;

In the interest of economy and efficiency, the new work is a logical follow-on to an original Federal Supply Schedule order and the original order was placed in accordance with the applicable Federal Supply Schedule ordering procedures. The original order was not previously issued under sole-source or limited-sources procedures (FAR 8.405- 6(a)(1)(i)(C)); or

Items peculiar to one manufacturer. An item peculiar to one manufacturer can be a particular brand name, product, or a feature of a product, peculiar to one manufacturer). A brand name item, whether available on one or more schedule contracts, is an item peculiar to one manufacturer (FAR 8.405-6(b)).

Rationale/explanation to support cited authority The Codonics Safe Label System will complement the current Pyxis system workflow by automating syringe labeling in the OR and procedural areas, which increases workflow efficiency and eliminating risk associated with handwritten medication labels. Lebanon VA Medical Center currently utilizes the Pyxis dispensing systems and servers, which is tailored to the VA network. Market research was completed on the open market and Federal Supply Schedules to determine if there are any other printers similar to the Codonics SLS 550I on the market that can provide the same features and connectivity and compatibility with Pyxis. Market research resulted in finding no other printers of this kind on the market. The Codonics SLS 550I is Printer is FDA-cleared and utilizes bar code technology to electronically confirm medical removed from Pyxis and then confirms the medication information visually and verbally before printing the medication labels. The Codonics SLS 550I is the only products of this type on the market.

The implementation of the printer at the Lebabnon VAMC will reduce the risk of medication errors and improve patient safety.

Market research did reveal that there are multiple vendors who are authorized to distribute the Codonics SLS 550I and the procurement is suiteable as a brand name only requirement.

6. BRAND NAME ONLY: This buy is estimated to exceed $25,000 and this documentation will be posted along with the RFQ on e-Buy, as required by FAR 8.405-1(e).

7. Note that this determination satisfies the requirements of FAR 8.405-6. The requirement to prepare a written justification IAW FAR 6.303 does not apply to Federal Supply Schedules.
