About this file

This is a Pesticide Certification document issued by the Kentucky Department of Transportation for contractors performing work on state contracts. The document requires all contractors engaged in pesticide activities that could impact waters of the United States to certify compliance with the KPDES Pesticide General Permit (KYG99) and the Pesticide Discharge Management Plan (PDMP). Contractors must read and understand the PDMP terms and conditions prior to performing pesticide-related work, including herbicide, insecticide, and fertilizer applications. The certification must be signed by an authorized company representative and submitted for the specific contract identified. A copy of the PDMP is available through the Kentucky Transportation Cabinet's website for contractor review prior to execution.

Contractors who violate any condition of the PDMP are subject to substantial penalties or contract termination. The certification requirement applies to all contractors performing pest management activities as part of their contract deliverables. This document serves as the contractual mechanism to ensure compliance with federal water quality regulations and state pesticide management requirements across all Kentucky Department of Transportation contracts involving pesticide applications.
