PERFORMANCE WORK STATEMENT

FOR

RAYMOND MODEL 9600 AND 9700 SWING REACH LIFT TRUCK REPAIR

9 Sept 2021

1. SCOPE OF WORK.

1.1 The Contractor shall provide all personnel, equipment, tools, test equipment, transportation, preventative maintenance parts, and other items and services necessary to perform maintenance and calibration (on-site) for the listed equipment. These units are located at the 354th Logistics Readiness Squadron, Building 3426, Eielson AFB, AK.

2. CONTRACTOR- SERVICES

2.1 MAINTENANCE: The Representative will service each of the four lift trucks.

Regulation Number 520-17-05238 has a hydraulic leak towards the top portion of the lift. The representative will need to replace the hose and check to see if the leak is resolved.

Regulation Number 520-17-05461 and 520-17-05462 will not ascend and would stay grounded. Their power no longer turns on. The contractor would need to troubleshoot as to why the assets do not rise and correct the error. Finally, the four charging stations no longer have an auto shut off when charging the lift trucks. It will continue to charge the batteries past its recommended voltage. This can cause damage to the batteries and the trucks. The contractor will need to troubleshoot the charging stations to reset it to factory settings.

RAYMOND MODELS

2.2 MODELS: The model that requires the hose replacement is: 9600 Swing Reach Lift Truck. The model requiring maintenance on its hydraulics and batteries is: 9700 Swing Reach Lift Trucks. Lastly, there are two charging stations for the 9600 model and two for the 9700 model.

3. SCHEDULE OF WORK

3.1 SCHEDULE: The contractor will provide services Monday through Friday. The hours of operation will be between 0730 and 1630, depending on the amount of work required to troubleshoot the lift trucks. If overtime is needed, an additional proposal will be provided as an additional quote before going over the already approved work hours. The contractor will coordinate work with the operation owner to minimize disruption. The contractor will work on the vehicles in building 3426 (354th Logistics Readiness Squadron Warehouse). Lastly, the contractor will coordinate with the Vehicle Control Officer(s) if the lift trucks need to be taken to an alternate location.

4. GENERAL INFORMATION

4.1. INFORMATION: The government reserves the right to restrict employment of any contractor employee, or prospective contractor employee, who is identified as a potential threat to the health, safety, security, general well‐being or operational mission of Eielson AFB and its population.

4.2 All personnel employed in the performance of this contract, or any representative of the contractor entering the government installation, shall abide by the security and health regulations of the installation.
