PERFORMANCE WORK STATEMENT

FOR

GROUP FITNESS

1. DESCRIPTION OF SERVICES.

1.1. SCOPE OF WORK. Contractor shall provide/furnish all labor, uniforms, supervision and transportation necessary to provide Group Fitness Instruction services, a minimum of 10 classes a week (number of classes adjustable at government discretion), at Little Rock Air Force Base, Arkansas.

1.2. EVALUATION CRITERIA. Contractor shall have experience in group fitness instruction, training of instructors and scheduling. Contractor shall have retained ability equal to the period of the contract. Contractor shall submit a resume of qualifications with solicitation.

All instructors shall be certified by AFAA, ACE, NAFTA, ACSM, AAAI/ISMA, NETA, Coopers Institute, or any of the organizations listed in by HQ AFPC/SV as accepted organization and certifications.

1.2.1. Contractor shall supply a copy of each individual’s respective certification/membership; to the Little Rock AFB Fitness Program Manager one week prior to the individual’s working/instructing his or her first class.

1.2.2. Additionally, contractor will ensure that all instructors maintain a current CPR certification. Upon awarding of contract, Contractor will provide proof of certification to the Fitness Program Manager. Contractor retains responsibility for ensuring compliance.

1.3. AREAS OF RESPONSIBILITY

1.3.1. Classes to be instructed could include but are not limited to step, interval, high/low impact, kickboxing, strength, boot camp, functional fitness, ECPs, parent and child classes, Yoga, Spinning and Pilates. The contractor shall provide the required number of instructors.

1.3.2. Instructors will comply with all Air Force safety requirements when instructing on Little Rock AFB, and enforce these requirements while performing services.

1.3.3. The instructor(s) shall pick up the required equipment/keys and return them back to the Fitness Center after their class. Fitness Center staff members and instructors will conduct an inventory upon return of equipment/keys.

1.3.4. The head instructor shall notify the Fitness Center if a member is removed from class, or is injured. Specific procedures for instructors to take will be outlined by the Fitness Program Manager. Instructors must report any member being removed due to poor conduct or inappropriate behavior, both verbal and physical, towards other members, instructors or Fitness Center staff. Instructors will report any injuries. A detailed description of the situation, to include name, rank, unit and what/why/how will be relayed to the Fitness Center front desk, and in the case of an ejection, a written statement must be submitted to the Fitness Program Manager.

2. GOVERNMENT FURNISHED PROPERTY AND SERVICES. The government shall provide the facilities, fixtures, equipment and services listed here or in Appendix A.

2.1. Property.

2.1.1. Facilities. The government will make available facilities for conducting the fitness classes. The government will provide the maintenance and repair of all instruction areas. The government with insure the room is closed for ongoing classes, unless the instructor(s) deem otherwise. No alterations shall be made to furnished facilities without specific written permission from the Contracting Officer. The government will provide adequate lighting for all classes.

2.1.2 Equipment. The government will furnish the equipment required and all equipment shall be returned to the Fitness Center upon completion of classes or as directed by the Little Rock AFB Fitness Program Manager.

2.2. Schedules. The government will provide the contractor with a schedule of classes at least one week prior to commencement of contract term.

2.2.1. Changes. The Fitness Program Manager reserves the sole right to change days and times of classes with a two (2) week notice to the contractor.

2.2.2. Arrival Time. Instructors will arrive early enough to ensure that classes start at the scheduled time.

3. CONTRACTOR FURNISHED ITEMS AND SERVICES.

3.1. Except for those items or services specifically stated in section 2 as government furnished, the contractor shall furnish everything needed to perform this contract according to all its terms and conditions.

4. GENERAL INFORMATION.

4.1. CONTRACTOR PERSONNEL. The contractor shall provide a contract manager and alternate(s) that shall have full authority to act for the contractor on all contract matters relating to daily operation of this contract. The contract manager and alternate(s), who shall act for the contractor when the manager is absent, shall be designated in writing to the Contracting Officer prior to the start of the contract.

4.1.1. The contract manager or alternate shall be available within 60 minutes during normal duty hours to meet on the installation with government personnel designated by the Contracting Officer, to discuss problem areas.

4.1.2. Telephone numbers of the contract manager or alternate shall always be updated and provided to the Fitness Center for short notice cancellation notification due to inclement weather, military-related commitments, or acts of God.

4.1.3. The contract manager and all employees shall be able to read, write, speak and understand English. The government reserves the right to restrict employment of any contractor employee, or prospective contractor employee, who is identified as a potential threat to the health, safety, security, general wellbeing, or operational mission of Little Rock AFB and its population.

4.1.4. Instructors shall maintain a professional attitude and wear appropriate professional teaching attire. Contractor personnel shall present a neat appearance and be easily recognized. This may be accomplished by wearing distinctive clothing item bearing the name of the company and employee name.

4.1.5. The Contracting Officer may, in writing, require that the contractor personnel be removed from the work site, area or base due to objectionable behavior.

Objectionable behavior includes, but is not limited to, wearing inappropriate attire, using profane language, and abusing or disobeying base rules or regulations. No instructor will be allowed to instruct if it is apparent he/she has been consuming alcoholic beverages. At no time shall an instructor consume alcoholic beverages or use tobacco products in, on or around the classes or use cell phones while performing duties during classes.

4.2. SCHEDULE OF WORK.

4.2.1. The contractor will be notified 14 days in advance, by the Fitness Center, concerning class schedules and approximate start dates. The Fitness Center will provide the contractor with a schedule of classes, not later than one week prior to the first class.

4.3. HOURS OF OPERATION.

4.3.1. Fitness Center hours of operation are as follows:

4.3.1.1. Monday – Thursday: 0500 – 2200

4.3.1.2. Friday: 0500 - 1900

4.3.1.3 Saturday: 0900 - 1800

4.3.1.4. Sunday and Holidays. Contractor is not required to provide services on any Federal Holiday, defined as New Year’s Day, Martin Luther King Day, Washington’s Birthday, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran’s Day, Juneteenth, Thanksgiving Day and Christmas Day.

4.3.2. Instructors may be required to work on weekends as scheduled in advance.

Cancelled or postponed classes may also be added to the end of the schedule, extending the schedule. Classes will not be scheduled on Federal Holidays.

4.4. QUALITY ASSURANCE.

4.4.1. The government will periodically evaluate the contractor’s performance.

4.4.2. The government is responsible for the inspection and acceptance of services authorized. All work shall be accomplished in a timely and professional manner. Work, which does not conform to the specifications contained herein, will be justification for rejection and non-payment after due process in accordance with the Federal Acquisition Regulation by the Contracting Officer. Correction of any deficiency shall be at no cost to the United States government or the Little Rock AFB Fitness Center.

4.4.3. The contractor will respond to written concerns or complaints received by the contracting office. (Note: Response is an action designated to correct or clarify a situation under question.) The contractor will provide the Contracting Officer a written response within 5 working days after receipt of the concern or complaint has been received by the contractor.

4.4.4. GOVERNMENT REMEDIES. The Contracting Officer shall follow FAR 52.212.4, Contract Terms and Conditions – Commercial Items (MAR 1999), for contractor’s failure to perform satisfactory services or failure to correct non-conforming services.

4.4.5. Payment will be made monthly upon receipt of a properly certified invoice for services rendered under this order. Contractor will only be paid for officials provided in accordance with the contract. The Fitness Center Director, Fitness Director (or designated representative, in their absence) and Contracting Officer Representative (COR) will be the only persons authorized to certify this pay document.

4.5. CANCELLATIONS.

4.5.1. Cancellations of scheduled classes will be handled as follows:

4.5.1.1. If the Fitness Center cancels a class, the Fitness Center will notify the head instructor not later than 60 minutes before start of the class.

4.5.1.2. Instructors shall also have the authority to postpone any class due to inclement weather, suitability of the instruction area, or emergencies such as military recalls/alerts.

5.6. SECURITY REQUIREMENTS.

5.6.1. All personnel employed by the contractor in the performance of this contract, or any representative of the contractor entering the government installation shall abide by the security regulations of the installation.

5.6.2. The contractor shall be responsible for safeguarding all government property provided for contractor use. At the end of each work period, all government equipment and materials shall be secured as directed by the Fitness Center Director after each game.

5.6.3 Base Access will be coordinated with the contracting office to allow services to be performed on the installation.

APPENDIX A

GOVERNMENT FURNISHED PROPERTY

1. Government Furnished Property. The government shall provide the following equipment and supplies. All equipment and supplies hall be returned to the Fitness Center, unless directed otherwise by the Fitness Center Director.

1.1. Classroom, Bosu Balls, Stereo, Hand Weights, Mats and Steps, barbells, weights, kettle bells.
