PERFORMANCE WORK STATEMENT (PWS)

In Support Of

PERFORMANCE WORK STATEMENT (PWS)
10 March 2020
1. BACKGROUND
1.1 The KC-46 Division will be making an off base move from 2590 Loop Road West
Building 558 Wright Patterson AFB, OH 45433 to 3000 Presidential Dr, Fairborn OH, 45324.

2. PROJECT SEQUENCE AND PERIOD OF PERFORMANCE
2.1 A Contract Kick-Off meeting is required prior to move. Some topics to be covered are:
Submittal Submissions, Site Access, Approved Contractor Lay-Down Areas, Safety, Period of Performance and any Contractor questions. Kick-Off members include the Contractor, Contracting Officer and TPOC. Kick-Off shall be scheduled within 10 calendar days of award.
2.2 General Schedule:

2.2.3 Material delivery 10 days after Kick-Off meeting
2.2.4 Total Period of Performance from notice of TPOC: Contractor must have completed all work in 21 calendar days after notice from TPOC to proceed with work
2.3 Contractor shall be responsible for coordinating personnel moves with office items.
Contractor shall regularly attend weekly scheduled project meetings with the construction, furniture and telecommunications personnel to keep up to date on progress and to coordinate when moves can occur. Project meetings are held on Fridays from 0900 - 1000 in Building 558, Room 210.05 (dial-in will be provided as needed).
3. SCOPE OF WORK
3.1 General Requirements – The Government seeks a professional moving Contractor to move various tenants residing in the KC-46 Tanker building (2590 Loop Road, West WPAFB, OH 45433) to an off base facility located at 3000 Presidential Dr, Fairborn, OH, 45324. The Contractor shall perform all actions during normal business hours; 8:00AM – mailto:nathan.herrmann.1@us.af.mil mailto:matthew.sowards@us.af.mil
5:00PM. The Contractor’s efforts to complete all contract requirements shall include all required pre-move surveys, scheduling, manpower (ex. project management, on-site oversight, and on-site direct labor), vehicles, tools and any other minor non-explicit ancillary support. The Contractor shall ensure all components of the project are to be completed on time. Appendix 1 provides move specifics such as organization, moving from (Building/Floor Area), moving to (Building/Floor Area), number of personnel to move, anticipated days needed to move, and the anticipated move date.
3.2 Pre-Move Surveys – Contractor shall be responsible for protecting all infrastructure, fixtures, finished furniture and equipment in the installation spaces and throughout the facility and adjacent areas. Prior to the move, Contractor shall identify any deficiencies and document them in a “pre-move survey report” and provide to the primary and alternate TPOC prior to the move occurring. Any subsequent damage resulting from execution of this contract shall be identified to the TPOC with a “post-move survey report”. This pre-move survey can occur the morning of the scheduled move. Pre-move survey will also confirm the number of personnel, equipment and material to be relocated, as well as origin and destination of items to be moved.
3.3 Moving Supplies – The Contractor shall, within fifteen (15) business days of contract award, provide all moving supplies. Contractor shall provide moving boxes (approximately 16” x 12” x 12”), packing tape and tape dispensers and packing labels.
Packing labels should be adhesive (and be removed without leaving damage) and shall contain the following information: “Old Location”, “New Location”, “Work Station”, “POC”, and “Box __ of __”. The Government will not be returning any Contractor-provided moving supply items. The Contractor shall deliver these moving supplies to 2590 Loop Road, West WPAFB, OH 45433. The Contractor shall see the primary or alternate TPOC for the specific Government representative to accept the Contractor-provided moving supplies.

3.4 Move Logistics – The Government will notify the Contractor within ten (10) business days of a scheduled move date(s). This notification allows the Contractor to posture resources (ex. labor) to perform the move on the scheduled date(s). The Government will go in the new office space. The Contractor shall move all predetermined Government-filled moving boxes, furnishings and equipment from the existing to the new office areas.
The Government estimates that each workstation or office shall have approximately three
(3) boxes worth of personal possessions. The Contractor shall move the aforementioned items and anticipate any one of the aforementioned quantities changing by as much as 25%.
Common office items to be moved include one (1) Plotter (HP Design Jet Z6200 Size – 96 x 34 x 54 in, Weight 430 lbs.), Ten (10) Printers, four (4) Safes Large (ex. 500 – 1200 lbs.) safes that contain classified materials*, two (2) free-standing paper shredders (HSM
411.2 Size – 18 x 15 x 28 in, 40 lbs.). Every item moved shall be tagged with a Contractor-provided moving label. The Contractor shall account for all equipment to be moved in their moving procedures and checklist. The Contractor shall move the aforementioned items and anticipate any one of the aforementioned quantities changing by as much as 15%.
*TPOC or Alt-TPOC must be contacted before moving safes. A security member must accompany Contractor when transporting confidential material.
The Contractor shall refer to the Appendix 1 and 2 provided with this PWS to ascertain where the offices are moving from and to so the level of effort can be determined.
4. CONTRACTUAL REQUIREMENTS
4.1 Quality Assurance. The Government will monitor the Contractor’s performance. The Government reserves the right to review services being provided.
4.2 Quality Control. The Contractor is solely responsible for the quality of services provided. The Contractor is also liable for the Contractor employee negligence, and any fraud, waste, or abuse. Re-performance of any work for failure to perform in accordance with the specified requirement shall be completed at the Contractor’s own expense and at no additional cost to the Government. In instances where the moving boxes are delivered free of exterior defects or other obvious exterior signs of mishandling, the Contractor shall not be held liable for condition of contents of Government-packed boxes. The contractor shall be held liable for the condition of Government-packed boxes in instances where the moving boxes are delivered with exterior defects or other obvious exterior signs of mishandling. Exterior box defects or obvious exterior signs of mishandling include but are not limited to: Bent, crushed, discolored, warped, waterlogged, and/or cut/punctured.
4.3 Place of Performance: AFLCMC/WK
4.3.1 Building 558, 2590 Loop Road, West WPAFB, OH 45433
4.3.2 Off-Base Location, 3000 Presidential Drive, Fairborn, OH 45324
4.4 Normal Hours of Operation. Normal hours of operation are 8:00AM through 5:00PM, Monday through Friday and exclude Federal holidays. Deviations from the hours specified above may be requested by the Contractor to be submitted to the primary and/or alternate TPOC. The Contractor shall give notice of the deviation of hours to the primary and alternate TPOC at least five (5) business days in advance of the proposed change.
Billable hours are limited to the performance of services as defined in the PWS.
Government surveillance of contractor performance is required to give reasonable assurance that efficient methods and effective cost controls are being used. Work in excess of the standard 40 hour work week requires prior written approval by the Contracting Officer.
4.4.1 Federal Holidays – The following are Federal holidays:

If the federal holiday falls on a Saturday, the holiday is observed on the preceding Friday. If the federal holiday falls on a Sunday, the holiday is observed on the following Monday.
4.5 Government Furnished Equipment/Information.
4.5.1 General Services – The Government will not provide any general services.
4.5.2 Facilities & Utilities – The Government will not provide any facilities or utilities.
4.5.3 Materials and Equipment – The Government will not provide any materials or equipment.
4.5.4 Housekeeping – The Government will not provide any housekeeping. The Contractor shall remove all waste generated in the performance of this PWS.
4.5.5 Records, Documents and Work Papers – The Government will not provide any records, documents and/or work papers.
4.6 Security Requirements. The Contractor shall take appropriate measures to ensure the protection of data to specified levels for the duration of the contract. The Contractor shall follow all appropriate security procedures regarding identification badges, entry control cards, and vehicle passes. The Government will direct the Contractor of the necessary avenues required acquiring the appropriate identification badges, entry control cards, and vehicle passes. Contractor shall follow all rules and regulations prior to entering any secure spaces, including restrictions on cameras, Bluetooth-capable devices, cellular phones, etc. All secure spaces require sign in and sign out during entrance and exit.
Contractors shall display identification badge (e.g. rapid gate badge) at all times. The Contractor must return all issued badges and passes to the Government at the end of the contract, before final payment will be made.
4.6.1 Safety: Contractors shall wear all required personal protective equipment (PPE).
At minimum, required PPE includes high-visibility vests and steel-toed boots.
5. CONTRACTOR QUALIFICATIONS/REQUIREMENTS
5.1 The Contractor shall provide sufficient trained personnel fully capable of providing the deliverables requested in the PWS and have the knowledge, skills and expertise to meet the requirements specified.
5.2 All work performed shall be in accordance with Federal, State of Ohio, AFLCMC/WK and local laws, regulations and procedures.
5.3 U.S. Citizenship is required for facility access.
5.4 Confidentiality – Services rendered and all materials including records, files, documents, and work papers generated by the Contractor as part of this PWS or by the Government shall be considered Government property. The information shall not be disclosed, copied, modified, used (except in the completion of this contract) or otherwise disseminated to any other person or entity at any time to include, but not limited to, inclusion in any database external to the Government without the Government’s expressed consent.
5.5 Contractor Point-of-Contact/Manager – The Contractor shall provide a manager who shall be responsible for the performance of the work. The name of this person and an alternate who shall act for the Contractor when the manager is absent shall be provided to the primary and alternate TPOC. The contract manager or alternate shall have full authority to act for the Contractor on all contract matters relating to daily operation of this contract.
6. CONTRACTOR PERSONNEL
6.1 The Contractor shall furnish sufficient personnel to perform all work specified within this contract.
6.2 When performing work associated with this PWS, the Contractor shall make clear to all individuals they deal with that they are Contractor employees and not DOD personnel when working in any situation where their Contractor status is not obvious to third parties. Contractor employees shall clearly identify themselves as a Contractor (i.e., company shirt, pin, visible company identification, etc.) in a visible location as to who their employer is, to avoid creating an impression that they are government officials. All Contractor-produced documents and/or reports shall be suitably marked as Contractor products or that Contractor participation is appropriately disclosed. Contractor personnel shall not manage, supervise, direct, or task U.S. military, DOD civilians or other personnel not associated with the Contractor while performing the tasks within this PWS.
6.3 Contractor personnel shall not make any commitment which commits the expenditure of
U.S. Government resources.
6.4 The Contractor performing services shall be required to comply with all U.S. Air Force rules, regulations and training that is applicable to conduct, safety, security and procedures governing site entry and exit.
6.5 Contractor personnel shall not establish an employee-employer relationship with the primary or alternate TPOC under this PWS.
6.6 The Contractor shall not provide personal services to the primary or alternate TPOC. If
Contractor employees are directed by any Government employee at any time to perform services not covered under this contract, the Contractor shall immediately notify the Contracting Officer along with the primary and/or alternate TPOC.
6.7 The Contractor shall not take direction from any AFLCMC/WK building tenants without first getting written approval from the Contracting Officer to ensure all work is within scope.
6.8 The Contractor shall maintain accountability for and control of any keys provided and shall return them to the AFMC/DS upon completion of work. Duplication of keys is not authorized.
6.9 Installation Access – Obtain access to Air Force installations through participation in the
Defense Biometrics Identification System (DBIDS). Requirements for Contractor employee registration are available at https://dbids-global.dmdc.mil/portal/. No fees are associated with obtaining a DBIDS credential.
Participation in the DBIDS is not mandatory and Contractor personnel may apply for one-day passes at the Base Visitor Control Office to access an installation.
6.9.1 Registration for DBIDS: Registration for DBIDS is available at https://dbids-global.dmdc.mil/portal/.
Procedure includes:
a. Present a letter or official award document (i.e., DD Form 1155 or
SF 1442) from the Contracting Officer that provides the purpose for access to the base Visitor Control Center representative.
b. Present valid identification, such as a passport or Real ID Act-compliant state driver’s license.
c. Provide completed SECNAV FORM 5512/1 to the base Visitor Control Center representative to obtain a background check. This form is available for download at https://www.secnav.navy.mil/dusnp/forms/forms/allitems.aspx.
d. Upon successful completion of the background check, the Government will complete the DBIDS enrollment process, which includes Contractor employee photo, finger prints, base restriction and several other assessments.
e. Upon successful completion of the enrollment process, the Contractor employee will be issued a DBIDS credential and will be allowed to proceed to worksite.
6.9.2 DBIDS Eligibility Requirements – Throughout the length of the contract, the Contractor employee must continue to meet background screen standards.
Periodic background screenings are conducted to verify continued DBIDS participation and installation access privileges. DBIDS access privileges will be immediately suspended or revoked if at any time a Contractor employee becomes ineligible.
An adjudication process may be initiated when a background screen failure results in disqualification from participation in the DBIDS and the Contractor employee does not agree with the reason for disqualification. The Government is the final authority.
6.9.3 DBIDS Notification Requirements
a. Immediately report instances of lost or stolen badges to the Contracting Officer.
b. Immediately collect DBIDS credentials and notify the Contracting Officer in writing under the following circumstances:
1. An employee has departed the company without having properly returned or surrendered their DBIDS credentials.
2. There is a reasonable basis to conclude that an employee, or former employee, might pose a risk, compromise, or threat to the safety or security of the Installation or anyone therein.
6.10 All commercial trucks must enter through Gate 26A, WPAFB, Ohio, which is located off St. Rt. 235, WPAFB, Fairborn, Ohio, for inspection. In accordance with the Installation 88 ABW/SFS base entry procedures must adhere to delivery vehicle process requirements mandated by the WPAFB AFI 31-101. Once at Gate 26A the driver must have a current pass AF Form 75, Visitor Pass, requested by AFLCMC/PZI, Area A, Building 1, POC SSgt Paul Denton, and be able to show:
• A current Vehicle Registration
• A current Proof of Insurance
• A valid and current Driver's License for the driver
• A valid and current Driver's License or other State Identification for any passenger(s) in the vehicle.
6.10.1 Gate Procedures:
• Once at Gate 26A be prepared to exit the vehicle and allow for vehicle inspection. At this time you may ask for directions to your final destination.
• If the final destination requires the driver to leave Gate 26A and re-enter the base through an alternate gate, the pass received will be sufficient documentation for entry into both gates, within a pre-established time directed by the Security Forces representative.
• Arrival at the appropriate gate after the designated allotted time will result in non-admittance to the installation.
• This procedure will occur each time a commercial truck requests access to the base. Current Bill of Lading (BOL) to get delivery driver and the vehicle on base. BOL must include the AF installation name, date of anticipated delivery, and specific destination (including building number, street address, and point of contact information).
Personnel should bring the following items when entering an installation:
• Current Registration for all vehicles
• Current Proof of Insurance for all vehicles
• Valid driver’s license for each driver
• Valid driver’s license or other state identification for each passenger
• The sponsor must state the guest's purpose and destination. It is the responsibility of the contractor to validate this need with their government POC prior to their arrival so as to coordinate the arrival time accordingly. Sponsors may request that a pass be applicable for multiple days if needed.
• To sponsor 10 or more people, a typed list of those people must be submitted by the Government Acceptor in Word or Excel format, stating LAST NAME then FIRST NAME of each person. The sponsor must submit that list to Pass & ID Area A, Building 286, no fewer than three days prior to the anticipated arrival.
Names listed must be complete names, (no nicknames), and correspond to the names on the state ID for each person.
Prohibitions: Firearms may not be brought on base under any circumstances.
Individuals with criminal records are not allowed on base. Contractor personnel are prohibited from sponsoring guests onto the installation.
6.11 Current rules and regulations applicable to the premises where the work will be performed shall apply to the Contractor and its employees while working on the premises. These regulations include, but are not limited to, presenting valid identification for personnel and vehicular entrance, smoking restrictions and any safety procedures as outlined in the installation regulations. Before work is started, the Contractor shall arrange with the primary and/or alternate TPOC a means of installation and building access, space for storage of materials and equipment and the use of the building approaches, corridors, and pathways for the purposes of executing work. The Contractor shall ensure ingress and egress of Contractor vehicles is limited to the Gate 19B off of National Rd. The Contractor shall keep within the established limits of work and avenues of ingress and egress. The Contractor shall ensure their personnel working on Wright-Patterson AFB become familiar with and obey all Wright-Patterson AFB requirements and regulations, including, but not limited to, safety, fire, traffic and security. The Contractor shall not enter any restricted areas unless required to do so and until cleared for such entry. The Contractor shall ensure Contractor equipment is conspicuously marked for identification.
6.12 The Contractor shall ensure project scheduling and work execution will be done in coordination with the AFLCMC/WK TPOC.
6.13 The Contractor shall immediately notify the primary and/or alternate TPOC and
Contracting Officer if any problems arise which might require a work stoppage or if any scheduling may be affected. The Contractor shall then provide the primary and/or alternate TPOC and Contracting Officer with a written account of the problem and its impact to the delivery schedule and period of performance.
6.14 The Contractor shall be liable for any damage to the building and/or any Government personnel office items to be moved by the Contractor which include, but is not limited to, televisions, monitors, computers, pictures, etc. The Contractor shall identify any existing damage to the building and/or Government personnel office items by means of a pre-move checklist. Proper measures (ex. protective floor runners, pads, temporary wall coverings, protective wrapping, etc.) shall be utilized by the Contractor to protect the building (elevators, walls, floors, etc.) as well as the Government personnel office items.
The Contractor shall conduct the pre-move checklist with the Government POC prior to starting work. Copies of all pre-move checklists shall be provided to the Government POC prior to the move.
7. SERVICES SUMMARY

9. Plans and schedules 3.2, 3.3, 3.4
Provide detailed, workable plans and schedules with less than 25% re-work and on date as requested 90% of the time with remaining 10% within 3 working days.
8.0 Voluntary Protection Program
Applicability: This section of the PWS applies to “Applicable Contractors” as defined below.
Definitions:
Applicable Contractor. A contractor whose employees worked at least 1,000 hours at the site in any calendar quarter within the last 12 months and is NOT directly supervised by the applicant (installation).
Days Away, Restricted, and or Transfer Case Incident Rate (DART). The number of recordable injuries and illness cases per 100 full-time employees resulting in days away from work, restricted work activity, and/or job transfer that a site has experienced in a given time frame.
Total Case Incidence Rate (TCIR) - Total number of recordable injuries and illness cases per 100 full-time employees that a site has experienced in a given time frame.
Voluntary Protection Programs - The Voluntary Protection Program (VPP) promotes effective worksite-based safety and health. In the VPP, management, labor, and OSHA establish cooperative relationships at workplaces that have implemented a comprehensive safety and health management system. Approval into VPP is OSHA’s official recognition of the outstanding efforts of employers and employees who have achieved exemplary occupational safety and health.
Requirements
Wright-Patterson AFB is in the process of pursuing VPP recognition or has already been recognized under the OSHA Voluntary Protection Program (VPP). VPP impacts all “applicable contractors” operating on Air Force Installations. The contractor shall be responsible for ensuring their employees and managers have a comprehensive understanding of VPP as well as full compliance with OSHA requirements. The contractor shall follow the safety and health rules of the installation or VPP site.

The contractor shall submit a Safety and Health Plan and corresponding site safety checklist to the Contracting Officer not later than 10 calendar days after contract award. The contractor’s plan shall include appropriate measures to ensure the contractor reacts promptly to investigate, correct, and track alleged safety and health violations and/or uncontrolled hazards in contractor work areas. The plan shall:
Demonstrate a management commitment to employee safety and health;
Identify the application of the safety and health plan to subcontractors;
Identify the roles and responsibilities of the following individuals with respect to safety and VPP:
Management, Supervisors, Employees, and Safety Coordinator;
Identify applicable safety rules and regulations;
Include a worksite hazard analysis to include base-line hazard identification and required control measures;
Include a job site analysis to include hazards of tasks required to control measures;
Identify employee safety and health training requirements and the documentation process;
Include a workplace inspection frequency, to include identifying the individual conducting the inspections;
Include employee hazard reporting procedures;

Identify individual(s) responsible for corrective action of hazards;
Identify first aid/injury procedures;
Identify procedures for accident investigation and reporting;
Identify emergency response procedures; and
Identify the process for tracking controlled hazards in contractor work areas.
The contractor shall submit their Total Case Incidence Rate (TCIR) and Days Away, Restricted, and or Transfer Case Incident Rate (DART) rates and OSHA Form 300A annually to the contracting office for consolidation and submission as part of the installation’s annual VPP Safety and Health Management report. TCIR and DART rates are due by the 15th of January of each year.
The contractor’s Quality Control Plan shall identify the processes and procedures the contractor will use to track compliance with the Safety and Health Plan, and the process and procedures that will be used to correct violations.
The contractor shall establish these VPP requirements for all subcontractors who qualify as applicable contractors under the resulting contract.
9.0 CONTRACTOR MANPOWER REPORTING LANGUAGE FOR CONTRACT
PERFORMANCE WORK STATEMENTS
The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the Air Force via a secure data collection site.
The contractor is required to completely fill in all required data fields using the following Air Force link web address: http://www.ecmra.mil/
Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through September 30.
While inputs may be reported any time during the FY, all data shall be reported no later than October 31 of each calendar year.
Reporting Period: Contractors are required to input data by 31 October of each year. Uses and Safeguarding of Information: Information from the secure web site is considered to be proprietary in nature when the contract number and contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the contractor name and contract number associated with the data.

Combating Trafficking in Persons Combating Trafficking in Persons (CTIP) is a worldwide problem posing a transnational threat involving violations of basic human rights. The DoD has a zero tolerance policy for TIP. As part of the 2013 http://www.ecmra.mil/ http://www.ecmra.mil/ http://www.ecmra.mil/ http://www.ecmra.mil/
National Defense Authorization Act, the Pentagon is required to take action against any contractor engaging in any activities related to labor trafficking, sex trafficking and child soldiering. The contractor should familiarize its employees with TIP by going to the DoD CTIP site at:
http://ctip.defense.gov/Home.aspx. The contractor should immediately report all suspicious activity of its employees to the COR or CO.
