About this file

This is a Past Performance Reference Information Form template designed for offerors to submit documentation of their relevant contract experience as part of proposal evaluation. The form requires offerors to complete one form per reference contract, limited to three pages per reference, and must include the offeror's name and Unique Entity Identifier (UEI), the reference contract client information, the entity that was awarded the reference contract with its UEI, the contract number and title, and the client point of contact (including the Contract/Subcontract Lead and optionally the Project Director or Technical Representative with name, title, telephone, address, and email).

The form further requires documentation of the reference contract's period of performance, contract type (such as Fixed-Price, Time and Materials, Cost Reimbursement, IDIQ, BPA, or BOA), total contract value with approximate average annual value, and the value performed to date as of the RFP release. Offerors must also provide a complexity summary addressing the scope, work difficulty, similarity to the current work scope, problems encountered, and creative solutions implemented. The form explicitly requires demonstration of the offeror's commitment to safe work practices, including a summary of the safety program, relevant policies, safety performance indicators such as Experience Modification Rates (EMR) for the past three years (with explanations for any EMR exceeding 1.0), and disclosure of any regulatory citations from OSHA, Cal-OSHA, or equivalent international safety authorities within the past three years, where readily available. The form may be amended in format provided the exact information, font, and size requirements and page limitations are maintained.

Where applicable, the Offeror may also provide general information regarding the safety practices of any proposed lower-tier subcontractors.

To support an understanding of the Offeror’s safety approach, the Offeror is encouraged to provide available safety performance indicators, such as Experience Modification Rates (EMR) for the most recent three years, where applicable to the business unit(s) performing the Work. If provided, any EMR exceeding 1.0 should be accompanied by a brief explanation.

Disclosure of regulatory citations (including OSHA, Cal-OSHA, or equivalent international safety authorities) within the past three years is requested only to the extent such information is readily available and deemed relevant by the Offeror. Where disclosed, a high-level summary of the nature of the citation and its resolution status is sufficient. Detailed reporting is not required.

Failure to provide the above information will not, in itself, render a proposal non-responsive. However, the Laboratory reserves the right to request additional clarification if necessary to evaluate the Offeror’s ability to perform the Work in a safe and responsible manner.

Note: The Offeror may amend the format for Attachment A, as long as the exact information, font and size and page limitations are followed.
