Past Performance Questionnaire

Thank you in advance for filling out this questionnaire for the Defense Contract Management Agency (DCMA) Procurement Center. We understand that your time is very valuable, and we appreciate your input. Your rating is essential in successfully evaluating the contractor’s past performance.

GENERAL INSTRUCTIONS:

1. DCMA Requests the Government Contracting Officer, Contracting Officer’s Representative or Technical POC (or Corporate Official for Commercial Experience) complete this questionnaire and submits directly to DCMA by 12:00 p.m. Eastern, November 28, 2023.

2. Provide an adjectival rating for the following questions (the adjectival rating is defined on page 2).

3. Please provide comments to substantiate the assigned rating. At a minimum, provide comments to substantiate any rating that is checked Exceptional (E), Very Good (VG), Marginal (M), Unsatisfactory (U) or Not Applicable (N).
