PAST AND PRESENT PERFORMANCE INFORMATION/QUESTIONNAIRE

Purpose: The Pretrial Services Agency for the District of Columbia (PSA) has issued a solicitation for the services identified in SECTION I below. As part of our evaluation process, PSA takes into consideration an

Offeror’s present and past performance history providing similar services. The purpose of this questionnaire is to obtain relevant performance history to assist in this evaluation effort.

SECTION II: OFFEROR INFORMATION

This Section is to be completed by the Offeror who intends to submit a proposal in response to the above solicitation. Once Sections I and II have been completed, the Offeror should forward this questionnaire to their reference point of contact, requesting the reference complete the questionnaire and forward it directly, via email, to the Contract Specialist/Contracting Officer identified in Section I above. The Contract Specialist/Contracting Officer will provide receipt acknowledgement to both the Offeror and Respondent.

Using the adjectival rating table below, choose the appropriated adjective that most accurately describes the contractor’s performance or situation to the questions provided.

Adjectival Rating Definition

Exceptional Performance meets contractual requirements with many exceeded to your firms benefit The contractual performance of the element or sub-element being assess was accomplished with no more than a few minor problems for which corrective action taken by the contractor were highly effective.

Good Performance meets contractual requirements with some exceeded to your firms benefit. The contractual performance of the element or sub-element being assessed was accomplished with no more than some minor problems for which corrective actions taken by the contractor were effective.

Satisfactory Performance meets contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the contractor were satisfactory.

Marginal Performance does not meet some contractual requirements. The contractual performance of the element or sub-element being assessed reflects a serious problem for which the contractor has not yet identified corrective actions or the contractor’s proposed actions appear only marginally effective or were not fully implemented.

Unsatisfactory Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance of the element or sub-element contains a serious problem(s) for which the contractor’s corrective actions appear or were ineffective.

Neutral Performance was not observed or not applicable to the current effort being reported against.
