PART II. EVALUATION (TO BE COMPLETED BY EVALUATOR)
For each of the seven (7) performance elements identified below, PPQ evaluators should rate the performance of the company and/or individual identified in Part 1(A) of this PPQ as identified above.
The PPQ evaluator who completes this PPQ must be knowledgeable of the Contractor’s day-to-day operations and overall performance. However, the PPQ evaluator is also encouraged to supplement personal knowledge of the Contractor’s performance with the independent judgment of others within the organization, as applicable.
Where the PPQ evaluator lacks sufficient information to provide an evaluation for a particular performance element, this should be noted accordingly in the Remarks section. Where any deficiency or unfavorable information is identified (Marginal rating or Unsatisfactory rating), the PPQ evaluator should denote any mitigating factors (i.e. problems due to causes outside the Contractor’s control) that may weigh in favor of the Contractor that this reference is provided.

Please provide an explanatory narrative in the designated Remarks section. The explanatory narrative need not be lengthy, however, detailed as possible. If more space is required for the explanatory narrative, then use the back of the PPQ or attach an additional page. Handwritten responses to this PPQ are acceptable. If responses are handwritten, please print legibly. Illegible handwriting will not be evaluated as part of the PPQ submission.
