About this file

This document appears to be a Past Performance Questionnaire template related to a federal contract opportunity with the United States Forest Service (USFS), Northern Region 1, for road and bridge construction services.

The USFS is seeking an Indefinite Delivery-Indefinite Quantity (IDIQ), Multiple Award Task Order Contract (MATOC) for road, bridge, and related civil construction projects with task order values ranging from $2,000 to $3,000,000. The solicitation number is 1240LU24R0011. The questionnaire instructs offerors to send it to their clients for completion, which will assist the government's evaluation of the offeror's past performance. The questionnaire covers ratings on quality, schedule, client satisfaction, management, cost/financial, safety, and overall performance, and asks whether the client would hire the contractor again. Completed questionnaires must be returned to the Contracting Officer by a specified date to be evaluated with the offeror's proposal.
