About this file
This past performance questionnaire requests information about a contractor's performance on a previous physical access control system installation and maintenance contract to support the evaluation of proposals for a new Department of Veterans Affairs solicitation seeking the same services. Specifically, the incumbent is seeking preventative maintenance for an existing PACS at the Veteran Benefits Administration Headquarters. References are asked to rate the contractor's quality of service, timeliness, business relations, management of key personnel, compliance with various requirements, cost management, and overall performance on a scale of outstanding to unsatisfactory. The contracting officer should be provided the completed questionnaire to assist with awarding a follow-on contract for PACS maintenance services at the agency.
