About this file

This is a Past Performance Questionnaire template used to evaluate contractor performance on federal contracts. The questionnaire is specific to Solicitation Number 1305M326Q0178 for Southern Journey Repairs, a research vessel maintenance project under the National Marine Fisheries Service/National Oceanic and Atmospheric Administration (NOAA).

The template is designed for completion in two stages: offerors complete Sections I and II with basic solicitation and contract information, then provide the questionnaire to government assessors who had direct experience with the contractor on previous relevant contracts. Assessors validate the submitted information and complete Sections III through IV, which capture assessor identification and role, and provide an overall performance rating. Performance ratings are classified into five categories: Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or Not Applicable. Assessors evaluate contractors based on performance of contractual requirements, quality and timeliness of delivery, cost management, communications, proactive management, and customer satisfaction. Completed questionnaires must be submitted to the Contracting Office at jenna.taulman@noaa.gov. Offerors are required to submit copies of Sections I and II from all questionnaires with their proposals by the solicitation closing date.
