About this file

This is a Past Performance Evaluation Questionnaire template for solicitation 36C24226Q0290, issued by the Department of Veterans Affairs Veterans Health Administration Veterans Integrated Service Network 2. The questionnaire is designed to be completed by references or past clients to evaluate contractor performance on previous contracts related to reusable surgical instrument sharpening and repairs services.

The questionnaire consists of two parts: Part I collects background information about the past contract, including customer/client details, contract number, period of performance, facility type, gross square footage, contract value, and a description of services performed. Part II contains a structured evaluation form to be completed by the entity that received services, using a five-point adjectival rating scale (Outstanding, Good, Acceptable, Marginal, Unacceptable, or Not Applicable). Evaluation categories assess quality of service (including ISO 9001:2015 and ISO 13485 certification requirements and ANSI-AAMI ST-79 standards compliance), recruitment and retention capabilities, timeliness and schedule adherence, business relationships and communication, customer satisfaction, and overall contract performance. The questionnaire also includes yes/no questions regarding contract re-award eligibility, termination for default, and receipt of cure notices or suspension of payments. Respondents must provide written explanations for any Outstanding, Marginal, or Unsatisfactory ratings. This is a template document with no specific response dates or evaluation criteria beyond the structured rating framework provided.
