About this file

This document is a U.S. Department of Housing and Urban Development (HUD) Environmental Assessment (EA) template for documenting environmental review requirements for HUD-assisted projects under 24 CFR Part 58. The standardized form provides a comprehensive framework for responsible entities to systematically evaluate and record environmental impacts, compliance factors, and mitigation measures for proposed projects. The template includes sections for project information, funding details, compliance with various environmental statutes and regulations, impact assessments across multiple domains such as land development, socioeconomic factors, community facilities, natural features, and climate considerations.

The form is designed to guide environmental reviewers through a detailed assessment process, requiring them to document potential environmental impacts using impact codes ranging from minor beneficial impact to significant impacts potentially requiring an Environmental Impact Statement. The document emphasizes the need for credible, traceable documentation of environmental review findings, including lists of sources consulted, permits obtained, and any required mitigation measures. The final sections provide space for determining whether the project will have a significant impact on the human environment, with signature lines for the preparer and certifying officer, and instructions for maintaining the environmental review record in accordance with HUD program requirements.
