About this file
This document is a Past Performance Questionnaire template from the U.S. Army Mission and Installation Contacting Command (MICC) at Fort Eustis, associated with Request for Proposal (RFP) W9124G24R0001. The questionnaire is designed to collect detailed performance evaluations from previous contract partners, allowing the government to comprehensively rate a contractor's capabilities across multiple dimensions including staffing effectiveness, workforce stability, personnel surge capabilities, and customer satisfaction.
The questionnaire provides a structured evaluation framework with rating scales ranging from Exceptional to Unacceptable, requiring detailed rationale for each performance category. Respondents are requested to complete and return the document within 3 days of receipt, specifically by 10:00 AM EDT on a specified date, with submissions directed to Contacting Officer David Clark and Contract Specialist Hunter Langdon via email. The form captures critical contract information such as contract type, value, award dates, and requires comprehensive performance assessments that will be used in the federal contract award decision-making process.
