About this file

This is a Single Source Justification document for a simplified acquisition under FAR 13.106-1 for the Department of Veterans Affairs.

The Network Contracting Office 1, Regional Procurement Office East is acquiring Midmark 626 Barrier-Free Examination Chairs and related services on behalf of three VA Medical Centers: Jamaica Plain (MA), Brockton (MA), and West Haven (CT). The total procurement includes 1 chair for Jamaica Plain, 6 chairs for Brockton, and 11 chairs total for West Haven (8 model 626-001 and 3 model 626-005), along with Premiere Delivery Services and Medical Exam/Procedure Table Removal Services, with an estimated total amount of $185,826.66. The acquisition is being pursued as a single-source award because the Midmark 626 is the only exam chair offering the required combination of accessibility, clinical accuracy, safety features, and ergonomic performance. The chairs have been standardized across VISN 1 since 2010, and prior identical purchases were made under contracts 36C24122F0231 and 36C24124P0871. Switching manufacturers would disrupt workflows, require retraining, compromise ADA/MDE compliance, and violate VHA Directive 1611 for Safe Patient Handling. Market research identified multiple SDVOSB and VOSB distributors capable of providing the chairs, including Sustainment Technologies, LLC; however, GSA and other scheduled sources cannot fully meet the requirement because installation and de-installation services are not available through their contracts. The Contracting Officer recommends a small business set-aside with SDVOSB/VOSB non-manufacturer distributors and has certified approval in accordance with FAR 13.106-1(b).

Over Micro-Purchase Threshold but Not Exceeding the Simplified Acquisition Threshold

Acquisition Plan Action ID: 36C241-26-AP-3644 / 36C241-26-AP-3648

1. Contracting Activity: Network Contracting Office 1, Regional Procurement Office East on behalf of the Department of Veterans Affairs, Veterans Integrated Services Networks (VISN) 1, on behalf of Jamaica Plain VA Medical Center 150 S. Huntington Avenue Jamaica Plain, MA 02130-4817, Brockton VA Medical Center 940 Belmont Street Brockton, MA 02301-5596 and West Haven VA Medical Center 950 Campbell Avenue West Haven, CT 06516-2770. Funding 523-26-4-4807-0116, 523-26-4-4807-0113 and 689-26-4-7821-0166.

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount: This is a new, brand name requirement. The Contracting Activity is looking to acquire Midmark 626 Exam chairs for Jamacia Plain, Brockton, and West Haven VAMC. Jamacia Plain VA Medical Center is looking to acquire (1) Midmark 626‑005 Barrier‑Free Examination Chair along with accessories, Brockton VA Medical Center is looking to acquire (6) Midmark (626‑005 ) Barrier‑Free Examination Chairs with accessories and West Haven VA Medical Center is looking to acquire (8) Midmark (626-001) Barrier-Free Examination Chair and (3) Midmark (626‑005 )Barrier‑Free Examination Chairs both with accessories. These purchases for each station will come with Premiere Delivery Services (CHRG-SVC305) and Medical Exam/Procedure Table Removal Services (CHRG-SVC315) estimated Total Amount $185,826.66.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: The Midmark 626 Barrier-Free Examination Chair is the only exam chair on the market that provides the required combination of accessibility, clinical accuracy, safety features, and ergonomic performance needed by the VA Connecticut and Boston’s Healthcare System.

Midmark is the sole manufacturer of the 626-series chairs, which offer a unique 15.5-inch low transfer height, flat transfer surface, integrated safety features, and standardized exam-room ergonomics essential for accurate blood-pressure measurement and compliant patient handling.

These chairs have been fully standardized across VISN 1 since 2010, and VA Connecticut and Boston have already procured identical Midmark 626 chairs under contract numbers 36C24122F0231 and 36C24124P0871. Maintaining brand-name consistency is required to ensure interoperability with existing equipment, staff familiarity, continuity of clinical workflow, and alignment with VHA Directive 1611 for Safe Patient Handling. No other manufacturer offers equipment that meets these established specifications; therefore, only the Midmark brand satisfies the agency’s needs.

To ensure ADA/MDE compliance, patient transfer safety, and continuity of care across all seven campuses, the Medical Center must standardize on the Midmark brand currently deployed throughout the facility. Using any other manufacturer would introduce prohibited equipment variance, disrupt existing accessibility workflows, require retraining, and fail to maintain brand‑specific compatibility already established under previous acquisitions. VA Boston and VA Connecticut have procured identical Midmark 626 chairs under contract numbers 36C24122F0231

Effective Date: 12/30/2025 Page 2 of 3 Acquisition ID# 36C241-26-AP-3644 / 36C241-26-AP-3648 and 36C24124P0871, and clinical services rely on these exact models for safe‑patient‑handling consistency. No other manufacturer offers equipment meeting the combined specifications for transfer‑height accessibility, support‑rail design, safety integration, and interoperability with installed infrastructure. Therefore, only the Midmark 626 exam chair meets the Medical Center’s clinical requirements and ensures full compliance with the forthcoming 2026 ADA MDE accessibility mandate.

The Medical Center cannot transition to a different manufacturer because doing so would break the established standardization of exam-room equipment across the facility. As documented in previous brand-name justifications, introducing a second, non-standard product line increases training burden, creates operational inconsistencies across multiple campuses, and introduces clinical risk because staff frequently rotate between sites and rely on uniform equipment for safe and accurate patient handling.

The VA currently uses Midmark 626 exam chairs—previously procured under contract numbers 36C24122F0231 and 36C24124P0871—and clinical workflows, room layouts, maintenance procedures, and safe-patient-handling protocols are built around this equipment. Switching to another brand would require re-training clinical staff and SPD/SPS personnel, updating safety procedures, re-establishing maintenance programs, and reconfiguring exam rooms at all seven campuses, resulting in increased risk and substantial operational disruption. For these reasons, only the Midmark brand can meet the Medical Center’s needs for compatibility, safety, and continuity of operations.

4. Description of market research conducted and results or statement why it was not conducted:

The Contracting Activity initiated comprehensive market research to determine the availability of Service-Disabled Veteran-Owned Small Businesses (SDVOSBs) and other small businesses capable of fulfilling the requirement for these Midmark Exam Chairs. The first step was a search of the SBA database using NAICS code 339113 (Surgical Appliance and Supplies Manufacturing) with a filter for SDVOSB certification. This general search generated a broad pool of 1,223 SDVOSB contractors associated with this NAICS code. However, this only established the potential for SDVOSB involvement at the NAICS category level and did not confirm capability or authorization to distribute the specific brand and model of evacuation chair required.

A search of the SBA DSBS database using NAICS 339113 and the keyword ‘Midmark’ identified three small businesses, including SDVOSB Sustainment Technologies, LLC, which is an authorized distributor capable of providing these chairs. A review of ADDS UP using the keyword ‘Midmark 626’ under Awarding Office 36C241 and Funding Offices 36C523 and 36C689 identified eight contracts for Midmark 626 exam chairs and related surgical equipment, including two prior purchases for the Boston and Connecticut VA Healthcare Systems under contract numbers 36C24122F0231 and 36C24124P0871.

A GSA e-Library search using the keyword “Midmark” identified eight schedule contractors holding MAS contracts that list Midmark medical equipment. This confirms that multiple scheduled vendors are authorized to provide Midmark products. However, the MAS results only cover supply items and do not include the installation and de‑installation services required for this procurement, which are not listed on the vendors’ GSA contracts. Therefore, while eight scheduled sources exist for the supplies alone, GSA cannot meet the full requirement because the necessary services are not available through MAS.

Effective Date: 12/30/2025 Page 3 of 3 Acquisition ID# 36C241-26-AP-3644 / 36C241-26-AP-3648

A GSA Advantage search for product number ‘626‑001’ identified six vendors offering the requested Midmark chairs; however, procurement through GSA is not feasible because the requirement includes installation and de‑installation services that are not available on the vendors’ GSA contracts, and without these services only the supplies—not the full requirement—can be purchased.

Market research has identified several SDVOSB’s that are capable of providing the required exam chairs. The NAICS code being used for this requirement (339113) is not covered by a Non- Manufacturer Rule (NMR), however the SDVOSB/VOSB distributor(s) identified qualified as non-manufacturers therefore a small business set-aside is recommended.

5. Contracting Officer's Certification: Purchase is approved in accordance with FAR 13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
