Justification for Single Source Awards IAW FAR 13.106-1 For

Over Micro-Purchase Threshold but Not Exceeding the Simplified Acquisition Threshold

1. Contracting Activity: Network Contracting Office 1, Regional Procurement Office East on behalf of the Department of Veterans Affairs, VISN 1, West Roxbury VA Medical Center 1400 VFW Parkway West Roxbury, MA 02132-4927. Funding 523-26-4-7918-0069.

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount: The Contracting Activity is looking to establish a new, sole source, follow-on contract to 36C24121P0754 with one (1) Base Year and four (4) Option Years for EMS Radio Subscription services required by the VA Boston Healthcare System (HCS). These services can only be provided by Metropolitan Boston Emergency Medical Services Council, Inc. (UEI: UND1LPDRNGP3), a non-profit large business located at 25 B Street, Suite A, Burlington, MA 01803. The Metropolitan Boston CMED EMS Communications Network provides essential emergency communication services that directly support VA Boston Healthcare System�s coordination with pre-hospital emergency personnel and inter-facility partners during routine operations and multiple-casualty or disaster incidents. The current contract, 36C24121P0754, will reach its final period of performance end date on 08/06/2026.

Total Amount for all 5 years will be $108,640.91.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: The current contract provides VA Boston with essential access to the Metro Boston CMED EMS communication network, necessary for real-time coordination with regional municipalities, EMS responders, and hospitals during emergency medical events. The contractor, METROPOLITAN BOSTON EMERGENCY MEDICAL SERVICES COUNCIL, INC., has supplied this service under 36C24121P0754 since 2021. VA Boston HCS requires continuous EMS radio network subscription service.

Discontinuation would leave the facility unable to communicate with EMS responders, which constitutes an unacceptable patient-care and safety risk. EMS coordination is a chronically recurring medical necessity. Any lapse would immediately affect emergency response capability. If a new contract is not established VA Boston would lose EMS communication capability after 08/06/2026 and ambulances cannot coordinate with dispatch, other hospitals, or municipal responders. Emergency operations, including mass-casualty response, would be impaired and VA would be non-compliant with state EMS communication requirements. The Metropolitan Boston Emergency Medical Services Council (MBEMSC) is a nonprofit organization established under a mandate from the

Massachusetts Department of Public Health. Its mission is to coordinate, maintain, and enhance the EMS system across the Boston region. The Council has operated for many years and remains the sole entity responsible for providing this essential service, ensuring seamless communication among all area medical facilities. The EMS communication network used by the Commonwealth of Massachusetts is proprietary and singular�the vendor operates the regional CMED system. Attempting to use another vendor is not technically possible, because the CMED EMS network is a regionally assigned public safety communication system, not an open commercial platform. Only the EMS Council controls and maintains the dedicated EMS medical radio channels, dispatcher coordination, interoperability links, EMS-to-hospital patches, and emergency incident communication protocols. No other business has legal authority, radio infrastructure, regional EMS integration, or public-safety governance status to operate CMED.

4. Description of market research conducted and results or statement why it was not conducted: Limited market research was conducted because the requirement involves participation in the Metropolitan Boston CMED EMS Communications Network, a regional emergency-communications system operated solely by the Metropolitan Boston EMS Council (MBEMSC). As the only organization mandated by the Massachusetts Department of Public Health to coordinate and maintain this network, MBEMSC is the exclusive provider of CMED communications services, and no alternative commercial sources exist. Because the service is unique, proprietary, and required for VA Boston Healthcare System to remain integrated with the regional EMS communications infrastructure, traditional market research could not identify any other capable vendors. Therefore, market research was not performed, and this procurement proceeds under a sole-source determination supported by the absence of competition. The contractor, METROPOLITAN BOSTON EMERGENCY MEDICAL SERVICES COUNCIL, INC., has supplied this service under 36C24121P0754 since 2021. Given the essential mission impact, recurring need, and contract expiration, it is recommended that VA establish a new 5-year contract (Base + 4 Option Years) to ensure uninterrupted EMS Radio Subscription services.
