Single Source Justification for SAP under the SAT

Revision: 01 Effective Date: 11/15/2024 Page 1 of 1 Acquisition ID#

DEPARTMENT OF VETERANS AFFAIRS

Justification for Single Source Awards IAW FAR 13.106-1 For Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)

Acquisition Plan Action ID: 36C246-25-AP-4854

1. Contracting Activity: Department of Veterans Affairs, Network Contracting Office 6, 100

Emancipation Dr., Hampton, VA 23667 in support of the Salisbury VA Medical Center, 1601 Brenner Ave., Salisbury, NC 28144. Purchase Request: 659-25-3-048-0200.

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

The Salisbury VA Medical Center requires brand name only Medtronic Valley Lab FT10 Electrosurgical Generators with cart, foot pedals, and adaptors. This equipment is utilized during surgical procedures. The estimated cost of the equipment is $34,646.04.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:

This is a brand name justification. The Medtronic Valleylab FT10 Electrosurgical Generator is compatible with technology used by surgeons for surgical procedure across facilities in the Salisbury VA Health Care System utilizing LigaSure Impact and LigaSure Atlas vessel sealing devices. Continuity across the Salisbury VA Health Care System is paramount to safety as staff travel between facilities to perform procedures and familiarity with the machine, settings, functions, and controls are critical for patient safety. Standardization for maintenance, parts, and accessories is in the best interest of the government. Disposables required by the devices are in current inventory at the health care system locations. Other alternatives would require additional inventory to be purchased and create potential for error in patient care.

4. Description of market research conducted and results or statement why it was not conducted:

Searches of SBA Small Business Search, GSA, NAC, and SAC were performed. Two sources were identified that can provide the required equipment.

5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106- 1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
