2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount: This contract is for preventive maintenance and service coverage for two phacoemulsification units. The specific equipment covered is displayed below and is detailed in the Scope of Work (SOW). Specific maintenance coverages for this contract are also detailed in the SOW.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: Alcon Laboratories, Inc. is the manufacturer of the equipment. ALCON software and updates are proprietary to Alcon. No other vendor has access to this software and thus can provide adequate services to the equipment covered by this contract.

4. Description of market research conducted and results or statement why it was not conducted:

During the market researching phase, the OEM (ALCON) provided its sole source letter, which is attached with this procurement.

5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
