VHAPM Part 813.106 Simplified Acquisition Procedures: Soliciting from a Single Source Single Source Justification for SAP under the SAT

DEPARTMENT OF VETERANS AFFAIRS

Justification for Single Source Awards IAW FAR 13.106-1 For

Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)

Acquisition Plan Action ID: 36C246-22-AP-5334

1. Contracting Activity: Department of Veterans Affairs, Network Contracting Office 06, in support of VISN 06 VA Health Care System at Hunter Holmes McGuire VA Medical Center, Purchase Request No. 652-22-3-165-0311.

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount: The Hunter Holmes McGuire VA Medical Center, 1201 Board Rock Boulevard, Richmond, VA 23249-4915 Eye Clinic requires the Picture Archiving and Communication System (PACS) to replace the system, currently at the end of its life cycle and is no longer being supported by the vendor. This proposed action supports the Department of Veterans Affairs in their efforts to replace the legacy electronic health record, VISTA, with the new electronic health record, Cerner. As part of this Cerner implementation, procurement is required to integrate the existing Zeiss FORUM software with the new health record, which requires additional licenses for the Zeiss FORUM Server. The delivery and installation of the software, test server and upgrades must be completed within the coming months. The interface and services are required to implement the new electronic health record for the Hunter Holmes McGuire VA Medical Center. The period of performance is from approximately September 30, 2022 – September 29, 2023. The total estimated price of the proposed action is $211,986.15.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: The Carl Zeiss FORUM eye PACS meets the unique characteristics mentioned above. It is currently approved on TRM, there is an approved MOU/ISA with the VA, and they have an approved risk assessment. FORUM is compatible with all the systems used at the CVHCS Eye Clinic. It is able to send and retrieve data from our patient record system VISTA, VISTA Imaging, and Radiology PACS. FORUM is currently used at Fayetteville, Salisbury, and Salem, which will support the CVHCS goal to assist other sites with patient examinations. Having the same eye PACS would also add to VISN 6’s standardization and allow the veterans an easier transition if they need care at other facilities.

4. Description of market research conducted and results or statement why it was not conducted: Carl Zeiss, TOPCON, and Sonomed Escalon eye picture archiving systems were all researched. Carl Zeiss’ FORUM meets all of the Central Virginia VA Healthcare System’s needs regarding system compatibility, approved security assessments, and VISN standardization goals. Market Research was conducted using VIP, NSSL, SAC, NAC, GSA, and FPDS-NG. VIP search using NAICS 334112 and keywords PICTURE ARCHIVING AND COMMUNICATION SYSTEM revealed no SD/VOSB vendors. NSSL, SAC, NAC, and
