LIMITED SOURCES JUSTIFICATION DOCUMENT

This is a Limited Sources Justification for a sole-source acquisition by the Department of Veterans Affairs, Veterans Integrated Services Network (VISN) 10, Louis Stokes Veterans Affairs Medical Center (CLE VAMC) in Cleveland, Ohio. The acquisition seeks to establish a firm fixed-price Blanket Purchase Agreement (BPA) with Hill-Rom, Inc. for the rental of Envella Air Fluidized Therapy Beds. The contract covers a base year plus three one-year option periods, with delivery required from March 1, 2026, through February 28, 2030. The estimated dollar value is $195,314.00 for the base year and $781,256.00 for the entire period including options. The beds are necessary for patient care of complex, advanced wounds, including Stage III/IV pressure injuries, grafts, flaps, burns, and intractable pain, with two configurations specified: 1,000 Day Envella Bed System and 800 Day Envella Bed System with Trapeze.

The justification for sole-source procurement is based on GSAR 538.7104-3(b)(1)(ii), establishing that Hill-Rom/Baxter is the only source capable of providing the Envella Air Fluidized Therapy Bed at the required quality level due to its proprietary, patented design and superior clinical capabilities. Market research was conducted across GSA eLibrary, SBA databases, and direct OEM confirmation, revealing no alternative vendors or small business distributors offering equivalent air-fluidized therapy systems. Competitive alternative evaluation of Arjo products (Citadel, Skin IQ, and Synergie Pearl) determined they lack the superior evaporative capacity, seamless ecosystem integration with existing Hill-Rom infrastructure at the facility, and compatibility with NaviCare nurse call systems required for operations. The acquisition is under the Multiple Award Schedule Program (FSS Contract 36F79721D0160, ending April 30, 2026), with pricing pre-evaluated for fairness and reasonableness. The VA and SBA Rule of Two cannot be met, with no waiver issued. Required certifications and approvals were obtained from the Health System Specialist, Contracting Officer, and Branch Chief.

(4) Identify the Authority and Supporting Rationale (see below and if applicable, a demonstration of the proposed contractor’s unique qualifications to provide the required supply or service.

GSAR 538.7104-3(b)(1)(ii): Only one source is capable of providing the supplies or services required at the level of quality required because the supplies or services are unique or highly specialized; Hill-Rom/Baxter is the only source that can provide the Envella Air Fluidized Therapy Bed. The beds are specific to the wound level and alternate contractors cannot provide an acceptable healing environment. The contractor was selected based on past history of successful treatment and healing of patients with Stage III/IV wound ulcers.

The Envella Air Fluidized Therapy Beds are proprietary and patented to Hill-Rom/Baxter:

• Hill-Rom/Baxter has no SB distributors for the Envella Air Fluidized Therapy Bed Rentals;

• Envella Air Fluidized Therapy Bed supports skin integrity by providing statistically lower interface pressure and superior microclimate management™ surfaces over traditional surfaces;

• The Envella Air Fluidized Therapy Bed has medical-grade, silicone-coated bead fluidization that promotes a flotation environment to help improve patient comfort;

• Hi/Lo adjustment can make patient positioning and egress easier and can provide a safer working height for caregivers;

• Electric Head of Bed articulation can help accommodate patients with respiratory complications;

• Provides a combination of AFT and low airloss therapies on an articulating frame;

• Optional system Patient Helper accessory can help patients change positions in bed and aid in transfers; and

• Head-of-Bed elevation for optimal pulmonary position and patient comfort.

The following are the patents for the Envella Air Fluidized Therapy Bed:

(5) Describe Why You Believe the Order Represents the Best Value consistent with GSAR 538.7102- 2(b)(4) aid the contracting officer in making this best value determination: The Envella Air Fluidized Therapy Bed provided by Hill-Rom/Baxter represents the best value to the Government because it delivers a clinically proven, medically necessary wound-care solution that https://www.acquisition.gov/fss-ordering-procedures

VHAPG Part 808.405-6 Limiting Sources Page 4 of 7 Revision: 01 Effective Date: 09/01/2024 Acquisition ID#: 36C250-26-AP-1080 cannot be replicated by alternative surfaces, while ensuring continuity of care, patient safety, and cost efficiency under a pre-competed Schedule contract with fair and reasonable pricing.

Clinical and Technical Value:

• The Envella Air Fluidized Therapy Bed is specifically designed for the treatment of patients with complex, advanced wounds, including Stage III/IV pressure injuries, grafts, flaps, burns, and intractable pain.

• The system provides true air-fluidized therapy using medical-grade, silicone-coated beads, creating a flotation environment that significantly reduces interface pressure and shear, promoting optimal wound healing.

• Envella offers superior microclimate management, helping control heat and moisture at the skin surface, which is critical for preventing further tissue breakdown.

• The bed combines air-fluidized therapy with low-air-loss capability on an articulating frame, allowing for clinical positioning while maintaining therapeutic effectiveness.

• These features are proprietary and patented to Hill-Rom/Baxter and are not available in commercially comparable products.

Operational Efficiency and Patient Care:

• Use of the Envella system supports standardized wound-care protocols already in place at CLE VAMC, reducing clinical variability and ensuring consistent outcomes.

• The articulating frame, Hi/Lo functionality, and optional patient helper accessories enhance patient mobility, caregiver safety, and ease of care delivery.

• Proven performance at VA facilities demonstrates reduced healing times, fewer complications, and improved patient comfort for high-risk wound populations.

• Rental access ensures immediate availability for emergent clinical needs without the capital costs or maintenance burdens associated with owned equipment.

Cost and Procurement Advantages:

• Pricing is established under Hill-Rom/Baxter’s Federal Supply Schedule contract and has been pre-evaluated for fairness and reasonableness.

• The rental model avoids large upfront expenditures and includes service, support, and equipment replacement, reducing lifecycle costs.

• Utilizing a single OEM source eliminates the need for multiple surface types, additional training, and duplicative support contracts.

• Alternative products would require clinical compromises, additional labor, and potentially longer lengths of stay, resulting in higher total cost of care.

Taken together, this order achieves best value by delivering a clinically indispensable therapy, ensuring continuity of care, reducing operational burden, and leveraging pre-competed Schedule pricing. No alternative product offers equivalent therapeutic capability for this patient

(6) Describe the Market Research Conducted among schedule holders and the results or a statement of the reason market research was not conducted. Market research was conducted by the Contracting Officer and the service to determine if there were other vendors capable of providing mattresses that meet CLE VAMC's specific requirements. The following sources were reviewed:

On 01/08/2026 Hill-Rom/Baxter, the OEM, confirmed that there are no distributors for leasing the Envella Beds.

The following sources were reviewed on October 30, 2025.

• GSA eLibrary, National Contracts, ECAT, SAM.gov, Interagency Contract Directories, NAC, SEWP, FPDS, SCMC, OMB, BIC and FDA:

a. Keywords Used: "bed"

(301) 698-5798 cfolland@l1enterprises.com Justification: Market research evaluated comparable products offered by Arjo, including the Arjo Citadel, Arjo Skin IQ, and Arjo Synergie Pearl systems. Of these products, only the Arjo Synergie Pearl qualifies as a true Air-Fluidized Therapy (AFT) bed system; the Arjo Citadel is an acute care bed frame designed for use with advanced wound support surfaces utilizing low-pressure air redistribution rather than air-fluidized therapy, and the Arjo Skin IQ is a powered microclimate management coverlet intended to be placed over a pressure-redistribution mattress and does not provide AFT capability. Although the Arjo Synergie Pearl provides AFT therapy, technical mailto:cfolland@l1enterprises.com

VHAPG Part 808.405-6 Limiting Sources Page 6 of 7 Revision: 01 Effective Date: 09/01/2024 Acquisition ID#: 36C250-26-AP-1080 evaluation determined that the Envella system offers superior evaporative capacity for moisture and heat management, which is critical for treating complex wound care patients. Additionally, the Envella integrates seamlessly with the existing Hill-Rom bed ecosystem currently deployed at the Cleveland VAMC, including compatibility with Centrella and Progressa beds and integration with NaviCare nurse call systems, thereby supporting established nursing workflows and clinical operations. Adoption of an alternate system would require workflow modification, additional training, and potential interoperability challenges. Based on these clinical, technical, and operational considerations, the Arjo product line does not meet the Government’s requirements at the level of quality and integration required for this acquisition.

The NMR does apply to this procurement under NAICS 532289, and the SBA has not issued an NMR waiver. The VA and SBA Rule of Two cannot be met, the OEM does not have distributors for rentals. This will be a sole source to the OEM Hill-Rom/Baxter

(7) Any Other Facts Supporting the Justification: A FITARA review is not required for this procurement because the acquisition involves supplies/services and does not include the purchase or development of information technology hardware or software.

(8) A Statement of the Actions, if any, the agency may take to remove or overcome any barriers that led to the restricted consideration before any subsequent acquisition for the supplies or services is made: On-going research of the industry marketplace will be conducted to determine if there are new entrants into this product arena.

(9) Requirements Certification: I certify that the requirement outlined in this justification is a Bona Fide Need of the Department of Veterans Affairs and that the supporting data under my cognizance, which are included in the justification, are accurate and complete to the best of my knowledge. I understand that processing of this limited sources justification restricts consideration of Federal Supply Schedule contractors to fewer than the number required by RFO Subpart 8.4.
