Justification for Single Source Awards IAW FAR 13.106-1 For

Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)

Acquisition Plan Action ID: 36C245-25-AP-2348, GlideScope

1. Contracting Activity: Department of Veterans Affairs, NCO5, Martinsburg VA

Medical Center located at 510 Butler Ave, Martinsburg, WV, 25405 in support of the Martinsburg VA Medical Center.

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

Every area with emergent intubating needs its own GlideScope for safety reasons.

Emergency Room and Operating Rooms have their own GlideScopes. 4A&4B share a GlideScope because of physical proximity. When Acute Care Medicine moves to 3A&3B the Acute Care ward will require its own GlideScope for emergent intubating.

The two sizes requested will add the capability to perform/assist with removal of foreign objects, large clots or mucus plugs from the lung as well as assist difficult intubations, including nasal intubation.

Estimated cost: $43,264.20

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:

The Martinsburg VA Medical Center already has Glidescope through out the facility (anesthesia, emergency department and ICU) and adding a different type of equipment will require additional training and accessories which constitutes extra unnecessary cost. Also, maintaining multiple systems for the same application could increase the risk of errors, potentially compromising patient safety during airway management situations. They also already have supplies (Disposable Laryngoscopes and Bronchoscopes) for this brand, and they will not be compatible with other brands. This would require additional costs for supplies to replace our current stock.

4. Description of market research conducted and results or statement why it was not conducted:

According to market research performed on GSA, NAC, SBA/DSBS, VETCert and FPDS there are not two or more SDVOSBs or Small Businesses that can fulfill the requirement.

Searched GSA with product ID# 0270-1022-FHD and was not found. However, the consumables are available on GSA such as product number 0570-0447 and 0570- 0450. Those 2 items are under contract# V797D-50352, Verathon Inc.

(Manufacturer).

Searched the NAC and came back with the same information as GSA, product ID# 0270-1022-FHD was not found, however, 0570-0447 and 0570-0450 were found under NAC contract. Those 2 items are under contract# V797D-50352, Verathon Inc. (Manufacturer).

Searched DSBS with NAICS code 339112 and found 4,281 vendors. Another search with keyword “airway” to narrow results and came up with 7 vendors that match that description.

Searched FPDS with keyword “glidescope” and found 600 results of the item, all awards made to Verathon inc. or Veterans Healthcare Supply Solutions, Inc.

(VHSS). An award was made to Verathon Inc. on August 4th, 2023, for the amount of $27,917.10 on contract# V797D50352.

Emailed Manufacturer about items on GSA and NAC contract, previous findings were correct. Also emailed about authorized distributors, VHSS is the only authorized distributor for Verathon products.

5. Contracting Officer's Certification: Purchase is approved in accordance with

FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
