1. Contracting Activity: Department of Veterans Affairs, VISN 12, Jesse Brown VAMC (537) has submitted 537-21-3-

969-0483 for Ophthalmology Equipment to the Great Lakes Acquisition Center (NCO12). Darel Gilmore from the Logistics will be the Jesse Brown VA contact point & Matthew Hohl from NCO12 will be responsible for the Contracting Action.

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

This justification and approval is for a Brand Name HAAG-STREIT procurement which will be set-a-side for Service- Disabled Veteran Owned Small Businesses and awarded as a firm-fixed-priced purchase order. This equipment is utilized during Ophthalmology appointments at the Jesse Brown VAMC.

This order is estimated at

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:

Jesse Brown VA Medical Center (Ophthalmology Department) is requesting the purchase of an Ophthalmology Equipment that will be used to support the Ophthalmology Mission. This equipment will provide the Jesse Brown VAMC continuity as the department is expanding their care. It is critical for the equipment to match what is already currently in use at the department to maintain the current level of care within the department.

No training required as staff have been using this equipment already.

4. Description of market research conducted and results or statement why it was not conducted:

This is a requirement for Brand Name only for the Jesse Brown VAMC as they are expanding their Department. The Original Equipment Manufacturer (OEM) is HAAG-STREIT which is considered a Small Business (SB) per the vendors Sam.gov Reps & Certs. Inquiries were completed to the Vendor information pages to identify any Service-Disabled Veteran Owned Small Business’s, Veteran Owned Small Business’s, or other Small Business’s. Additionally, a Sources Sought Notice (36C25221Q0723) was posted to identify any additional vendors to determine if it can be set-a-side(SDVOSB, VOSB, SB). Both actions revealed five (5) Service-Disabled Veteran Owned Small Business’s interested and capable of providing a Small Business Manufactured product.

Based on the information provided above, it is determined this procurement shall be set-aside for Service-Disabled Veteran Owned Small business (open market), and that the award can be made at a fair and reasonable price that offers the best value to the United States IAW 13 CFR § 121.406(b), and VAAR 819.7005.

5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
