2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

Philips Ultrasound Probe; Transducer, X8-2t / $

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: Brand Name Only

This requirement is for a brand name Philips Ultrasound probe. It is to replace a broken existing probe utilized by VA Palo Alto, utilizing an exchange program with the OEM Philips Healthcare (Philips North America LLC). This is an urgent requirement, a replacement probe is required in order to resume normal operations in service of VAPAHCS patients and stakeholders. The probe must be compatible with existing equipment at the station, the X8-2T probe is the only model/brand that can fulfill the needs of the government.

4. Description of market research conducted and results or statement why it was not conducted:

FPDS, SAM, SEWP, GSA Advantage, GSA eLibrary, the NAC, and the SAC were reviewed. Additionally, the OEM was contacted via phone where it was determined that there was only one brand.

FPDS produced past procurements similar to the current requirement, particularly on 10/14/2021, 36C24122P0060 was awarded to Philips America for an ultra sound probe in the amount of $ . On 05/02/2022, a telephone call was made to Philips Healthcare where a sales representative was contacted. It was initially determined that in order for the station to obtain the exchange price (trading broken equipment for new equipment) Philips was thought to be the only provider. During solicitation of a sole source RFQ 36C26122Q0560 it was determined that Philips did in fact have at least 2 SDVOSB authorized distributors, and

. Other market research efforts regarding the aforementioned sources confirmed that Philips was the only brand for the exchange of the needed ultrasound probe.

5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify nd complete to the best of my knowledge and belief.
