Justification for Single Source Awards IAW FAR 13.106-1 For

Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K) Brand Name

Acquisition Plan Action ID:

1. Contracting Activity: Department of Veterans Affairs, Network Contracting Office 06, located at 100

Emancipation Drive, Hampton, VA 23667, in support of VISN 06 Fayetteville VA Coastal Health Care System (565) at 2300 Ramsey Street, Fayetteville, NC 28301.

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

The Fayetteville Costal Health Care System and Ramsey Main operating rooms is in need of two additional Zoomscope with Trulight Colposcope with Digital USB Camera to be used during women’s surgical procedures.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:

The original equipment manufacturer is CooperSurgical Inc (LB). The Wallach Zoomscope is currently being used in the clinic and the Operating rooms at both the Fayetteville HCC and Ramsey HCC. The Wallach Zoomscope colposcope is currently compatible with the existing software at the facility and will be interchangeable and interoperable with the current system in place.

4. Description of market research conducted and results or statement why it was not conducted:

Market research was perfomed to identify sources and to evaluate the availability of the marketplace to meet the requirement. The VA Rule of Two cannot be met. Market research revealed the requirement can be fulfilled by the OEM CooperSurgical Inc (LB) and by authorized distributors After Action Medical and Dental Supply, LLC (SDVOSB), Medline Industries, Inc. (LB), Henry Schein (LB), and Cardinal (LB).

5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
